CLE REPORTING FORM

Please use this form to keep track of your CLE credits throughout your two-year certification

period. This form must be submitted along with your renewal application and the supporting

documenation required for each of the CLE credits being reported. Please feel free to print more

copies of this form as needed to record your CLE credits.

CLE Requirements: a minimum of 12 hours as follows: 10 hours substantive; 2 hours ethics.

Your renewal packet must include the following:

1. The completed Application for Renewal. The application should be signed on the day that

it is submitted for processing;

2. The completed CLE Reporting Form;

3. Supporting documentation relative to all CLE credits indicated on the CLE Reporting

Form. (See next section for specific requirements.);

4. Check or money order for the appropriate fee made payable to Keystone Alliance of

Paralegal Associations. Please note that this fee is non-refundable.

For members of a Keystone Alliance Member Association: $25.00 non-refundable

renewal application processing fee for next two-year certification period.

For individual applicants not affiliated with a Keystone Alliance Member Association:

$100.00 non-refundable renewal application processing fee for next two-year

certification period.

SUPPORTING DOCUMENTATION

IMPORTANT NOTE: The Keystone Alliance oversees the Pa.C.P. Program; therefore, only the Keystone Alliance can award preapproved CLE credits toward the Pa.C.P. credential. Courses which are preapproved by NFPA, PBI, etc. do not qualify for preapproval toward your Pa.C.P. credential. If the course description and/or Proof of Attendance does not specifically state that the course was preapproved for CLE credits by the Keystone Alliance and/or toward the Pa.C.P. credential, then you will need to seek approval for those CLE credits at the time of renewal as set forth below.

The Application for Renewal must be submitted along with the completed CLE Reporting Form, the required fee and supporting documentation relative to the Pa.C.P.’s requested CLE credit hours as follows:

CLE Credits Preapproved By The Keystone Alliance

If the Pa.C.P. attends a course, seminar or program which has been preapproved for CLE credits by the Keystone Alliance, the Pa.C.P. need only submit his or her Proof of

Attendance at the time of renewal. The said Proof of Attendance must specifically state that the course, seminar or program was preapproved by the Keystone Alliance and the number and type (substantive or ethics) of CLE credits for which the the course, seminar or program was preapproved as well as the title of the course and the name of the speaker(s). If the Proof of Attendance does not specifically state that the course was preapproved for CLE

credits by the Keystone Alliance, you will need to include a copy of the course description which indicates same.

CLE Credits Sought at Time of Renewal

Additional documentation is required for any courses, seminars or programs which were not preapproved for CLE credits by the Keystone Alliance. The type of documentation required will vary depending upon the source of the requested credits as follows:

Courses/Seminars/Programs

When applying for CLE credit hours for seminar/courses attended, the Pa.C.P. must submit the following:

 a description of the seminar topic and an outline of the presentation*;  a copy of each speaker's bio or resume which indicates his or her relevant

qualifications;

 for conferences which feature multiple breakout sessions, a copy of the seminar brochure or agenda which provides a detailed timeline of each segment of the conference including lunch and breaks; and

 a copy of your proof of attendance** which indicates the number of hours attended. CLE credit hours shall not be granted for less than thirty (30) minutes of instruction. There is no restriction on the number of CLE credit hours which may be obtained in this manner. *If the speaker does not provide an outline or agenda, the Pa.C.P. should take notes throughout the presentation so that an outline can be drawn up and submitted with your renewal application.

**If the provider does not offer a proof of attendance form for paralegals, please use the proof of attendance form available on the FORMS AND DOCUMENTS page. The form must be completed by you and signed by your supervising attorney or office manager or by the program moderator.

College Courses

When applying for CLE credit hours for college courses, the Pa.C.P. must submit the following:

 a copy of the course title and description;  a list of faculty names and credentials;  the course syllabus;

 for ethics credits based upon an ethical component of the course, proof of the time dedicated to ethics (this may be indicated in the course syllabus, or the Pa.C.P. may need to obtain additional documentation including but not limited to a signed

statement from the instructor);

 a copy of your final grade (a passing grade of B or better is required in order to obtain CLE credit).

CLE Credits will be awarded on an hour-per-hour basis for college courses which consist of substantive and/or ethical legal topics up to the 10-hour minimum requirement for

to qualify for ethics credits, the course must specifically address ethical issues or must contain a component which specifically addresses ethical issues.

There is no restriction on the number of CLE credit hours which may be obtained in this manner.

Self Study Programs

When applying for CLE credit hours for self study programs, the Pa.C.P. must submit the following:

 a copy of the promotional brochure;  the course outline;

 a list of faculty names and credentials;  sample exam questions;

 a copy of your final test score indicating a grade of B or better; and  the certificate of completion.

A self study program is a course of study in which the Paralegal is self-directed, i.e., working at his/her own pace without direct supervision or attendance in a classroom. The self study program must include a final examination consisting of a minimum of ten (10) test questions for each CLE credit hour. The paralegal must correctly answer 80% of the questions to receive CLE credits. The number of CLE credits awarded is at the discretion of the Credentialing Committee based upon the following criteria:

 listening and/or viewing time for audio, video, CD, DVD and/or on-line mediums;  written materials including books and seminar manuals which will be assessed at 50

pages per hour; and

 the test score on the final examination.

NOTE: Self-study programs are not CLE seminars taken on-line instead of in person. On-line seminars are included in Courses, Seminars, and Programs.

A maximum of three (3) CLE credit hours may be obtained in this manner. Teaching, Speaking Or Guest Lecturing

When applying for CLE credit hours for teaching, speaking or guest lecturing, the Pa.C.P. must submit the following:

 a copy of the course advertisement or seminar brochure;

 a description of the seminar topic, including areas of substantive law or professional issues to be presented; or a copy of the seminar outline or agenda;

 a copy of the speaker's resume or description of the speaker's relevant qualifications;

 if offered, the number of credits available to attendees; and  number of hours/minutes of the engagement.

The number of CLE credit hours to be awarded in this category will be based upon the length and content of the presentation and may include credit hours for preparation time.

The number of CLE credits awarded is at the discretion of the Credentialing Committee. A maximum of five (5) CLE credit hours may be obtained in this manner. CLE credit is provided only one time for any seminar/course presentation.

Publishing Articles

When applying for CLE credit hours for publishing an article, the Pa.C.P. must submit the following:

 the article(s) for consideration;  proof the article was published; and  the date of publication.

The number of CLE credit hours to be awarded in this category will be based upon the length and content of the article. The number of CLE credit hours granted is at the

discretion of the Credentialing Committee. To receive credit, the article must be published during the Pa.C.P.'s Certification Period.

A maximum of two (2) CLE credit hours may be obtained in this manner.

ADDITIONAL INFORMATION AND INSTRUCTIONS

Please do not submit any information or documentation beyond the requirements of the

renewal application.

It is the Applicant’s sole responsibility to redact his or her social security number and

month/day of birth from all documentation submitted.

Please assist us with our scanning process by avoiding the use of staples, metal or plastic

bindings, 3 ring binders, metal fold-over straps, etc. One large binder clip or a rubberband is

preferred as your submission (renewal application, supporting documentation and a copy of

your check) will be scanned in as a single document.

IT IS YOUR RESPONSIBILITY TO SUBMIT ALL REQUIRED DOCUMENTS AND

FEES. IF YOUR APPLICATION AND/OR SUPPORTING DOCUMENTS ARE

INCOMPLETE, YOUR APPLICATION MAY BE REJECTED.

DETERMINATION AND NOTIFICATION

*****************************************************

The Credentialing Committee makes every effort to complete its review process within

thirty (30) days of the Chairperson’s receipt of an application or renewal application. All

work is performed by volunteers who are employed as full-time paralegals. Please allow up

to forty-five (45) days for a response in consideration of mailing delays, work loads,

vacation schedules, etc. If you do not receive a response within 45 days, you may request a

status

report

by

emailing

the

Credentialing

Committee

Chairperson

at

Applicants will receive notification of the Credentialing Committee’s determination via

letter from the Keystone Alliance Chairperson.

If your application is approved, you will receive an approval letter enclosing your new

wallet card. The Pa.C.P. will be solely responsible for meeting the requirements for

maintenance of the Pa.C.P. Credential including but not limited to obtaining the requisite

CLE’s within the two-year Certification Period and submitting the Application for Renewal

no more than 60 days prior to the Pa.C.P.’s next anniversary date. See the Keystone

Alliance website for complete information and forms.

If your application is denied, you will receive a denial letter advising you of your right to

appeal within 45 days and enclosing the necessary forms and instructions for submitting the

appeal. The Appellant shall submit the Appeal Form, reason for the appeal and any

supporting documentation. The Appeals Committee shall make the final determination on

the acceptance or denial of the Appellant's certification renewal.
