Program Revision Form

To all faculty members seeking to modify an existing program (including degree and certificate programs and options to existing programs) to the Curriculum Committee, please make sure the following items have been met before submitting your program revision proposal. For all revised programs, please submit an electronic copy of your proposal to the Curriculum Committee

Chair, with a copy to the New Course/New Program Subcommittee Chair. All program

revisions proposals must follow these guidelines. If these guidelines are not followed completely, your program revision proposal may be delayed in the acceptance process. Please refer to the

Curriculum Committee website for all deadlines. Please Note:

a) These guidelines for submitting program revision proposals to the New Course/New

Program Subcommittee were taken from the Union County College Curriculum Committee By-laws found on the committee’s web page.

b) Faculty members who seek to revise an existing program must first consult with all departments (including Library and IT) affected by the proposed program revisions. c) A program revision proposal must be approved by the Department, the New Course/New

Program Subcommittee, the Curriculum Committee, the Faculty at one of the scheduled Faculty meetings, and the VP for Academic Affairs.

d) This form must be submitted to the New Course/New Program Subcommittee Chair with your program revisions proposal.

Current Requirement – the program now requires ENG 122 and MAT 113 or MAT 143 and 144. Rationale to Support Change: Students transfer to Union County College haven taken the

equivalent of ENG 102 or a higher level mathematics course (above MAT 113) – this will make graduating and completing the Sport Management Degree less complicated for these students – In addition, certain colleges recommend ENG 102 or a higher level mathematics (above MAT 113) – this will also make graduating and completing the Sport Management Degree less complicated for these students

☒ Department Approval. Attach email from department chair indicating department review and approval of the program revision proposal.

☒ Degree Requirements. Not required for program name or catalog description changes. Provide an outline of the curriculum, semester by semester, including a list of the proposed courses and credits per course, including lecture and lab hours. Indicate the total number of credits in the degree program.

☒ Program Review Grid. Not required for program name, catalog description, or

recommended sequence changes. Provide a Program Review Grid for the applicable degree

(see Curriculum Committee website) that follows the state requirements for General Education. The Program Review Grid must clearly show the courses offered with accompanying credit in each of the General Education Categories: Communications; the Social Sciences; Mathematics, Science, & Technology; and Humanities. The Program Review Grid must also indicate how the diversity course requirement is met.
