Job Title/Position: Social Services Supervisor

Reports To: Clinical Director

JOB DESCRIPTION SUMMARY:

The Social Services Supervisor is responsible for managing all aspects of the provision of hospice patient social services including managing the members of the social services team as applicable. The Supervisor establishes, implements, and evaluates goals and objectives of the hospice patient and ensures that the social services support team members are meeting goals and promoting standards of quality and excellence which in turn contribute to the organization’s mission and philosophy. The Social Services Supervisor is a member of the core interdisciplinary team (IDT). The Social Services Supervisor functions under the supervision of the Clinical Director.

ESSENTIAL JOB FUNCTIONS/RESONSIBILITIES:

1. Directs all daily hospice patient social service operations including providing direct

oversight of the establishment and implementation of social service policies and procedures. 2. Ensures compliance with all state, federal, and CHAP social service regulatory

requirements.

3. Directs the implementation of improved work methods and procedures to ensure social services support is delivered in accordance with Nurse Force Hospice policy.

4. Ensures seamless home hospice care and service delivery by providing direct oversight of the coordination of care with other clinical service providers.

5. Assists the Administrator in the preparation of an annual budget for the social services department and monitors allocation of resources according to budgetary limitations.

6. Recruits, selects, orients, and directly manages members of the social service support team. 7. Supervises the assessment, planning, implementation, and evaluation of social service

clinical care by members of the social services team.

8. Ensures proper documentation of social services to the hospice patient and family/caregiver in the clinical record in compliance with state, federal, and CHAP standards.

10. Maintains a comprehensive working knowledge of the field of social work and shares information with appropriate organization staff.

11. Maintains a comprehensive working knowledge of Nurse Force Hospice contractual

relationships and ensures that social services are delivered according to contract provisions. 12. Maintains a comprehensive working knowledge of community resources and assists referral

sources in accessing community resources on behalf of the patient and family/caregiver in the event that services will not be provided by Nurse Force Hospice.

13. Plans and implements in-service and continuing education opportunities to meet the development needs of social service staff.

14. Actively participates in IDT meetings as scheduled.

15. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS:

1. Has a Master's degree in Social Work. This requirement meets the employee qualifications stated in CMS Hospice Conditions of Participation §418.114(b) (3).

2. Has at least three (3) years of management experience preferably in home care operations (preferable).

3. Demonstrates an ability to supervise and direct professional and administrative staff. 4. Has the ability to deal tactfully with customers and the community.

5. Has an active knowledge of corporate business management, governmental regulations and CHAP standards as applicable.

6. Demonstrates effective communication techniques and established public relations skills. 7. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in

Job Title/Position: Social Worker

Reports To: Clinical Supervisor or Master of Social Work

Job Description Summary

The social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.

Essential Job Functions/Responsibilities

1. Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required. 2. Carries out social evaluations, including family dynamics, caregiver abilities,

communication patterns, high-risks for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in

relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.

3. Assesses for, and educates interdisciplinary group, on any special needs related to the culture of the patient and family. Includes communication, role of family, space, and any special traditions or taboos.

4. Maintains clinical records on all patients referred to social work.

5. Educates patients and families on, and assists in, preparation of advanced directives. 6. Provides information and referral services for organization patients and families/caregivers

regarding practical and environmental needs.

7. Provides information to patients and families/caregivers and community agencies. 8. Serves as liaison between patients and families/caregivers and community agencies. 9. Maintains collaborative relationships with organization personnel to support patient care. 10. Maintains and develops contracts with public and private agencies as resources for patient

11. Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.

12. Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.

13. Actively participates in quality assessment performance improvement teams and activities. 14. Assists family and patient in planning for funeral arrangements, financial, legal, and health

care decision responsibilities.

15. Other duties as delegated by the Clinical Supervisor.

Position Qualifications

1. A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3). 2. Minimum of one (1) year's experience in health care, hospice experience preferred.

Understands hospice philosophy, and issues of death/dying. 3. Experience in a hospice care preferred.

4. Demonstrates good verbal and written communication, and organization skills. 5. Possesses and maintains current CPR Certification.
