19. Call  Back:  

If  less  than  forty  hours  worked,  the  regular  hourly  bill  rate  applies  and  if  over   forty  hours  in  one  week,  1.4  times  the  regular  hourly  bill  rate  with  a  minimum  of   2  hours.

  The  general  and  family  practice  physician  provides  examinations  and   medical  care  for  the  entire  family.  The  physician  is  concerned  with   proper  injury  care  and  disease  prevention,  diagnosis,  treatment,  and   recovery.  The  physician  prescribes  and  administers  medications,   performs  routine  vaccinations,  and  provides  advice  regarding   personal  health  and  hygiene.  The  physician  will  request  necessary   tests  and  follow-­‐up  visits  and  refer  patients  to  specialists,  as   necessary.  

Minimum  Education   Degree  in  medicine  from  an  accredited  school  of  medicine.  

License   Physicians  will  maintain  a  current,  active,  valid,  and  unrestricted   medical  or  osteopathic  medical  license  from  a  U.S.  State.    

Certification   Physicians  who  have  completed  requirements  for  training  and   experience  meeting  the  standards  of  various  member  boards  of  the   American  Board  of  Medical  Specialties  (ABMS)  will  be  board  certified   in  their  respective  specialties.

Job  Description   The  Ophthalmic  Technician’s  duties  include  preparing  examination   and  treatment  rooms,  reviewing  the  appointment  schedule,   performing  basic  and  routine  vision  screening  examinations,  

administering  eye  medications,  cleaning  and  maintaining  ophthalmic   instruments  and  assisting  physicians  during  minor  surgical  and  laser   procedures.

Job  Description   The  licensed  clinical  social  worker  provides  comprehensive  services   through  a  broad  range  of  individual,  family,  programs  and  services  to   sustain,  restore  or  enhance  the  social  well-­‐being  and  functioning  of   individuals,  families,  and  communities.    Licensed  clinical  social   workers  are  members  of  the  facility’s  healthcare  team,  frequently   working  in  social  work  service,  outpatient  mental  health  clinics,   family  advocacy  programs;  and  substance  abuse  treatment  services;   mental  health  services,  combat  stress  detachments  and  correctional   facilities.    Services  provided  shall  comply  with  the  National  

Association  for  Social  Workers  (NASWs')  Standards  for  Social  Work   Case  Management,  Standards  of  the  Joint  Commission;  Standards  of   Practice  of  the  Case  Management  Society  of  America;  the  American   Accreditation  Health  Commission/Utilization  Review  Advisory  

Commission  (URAC);  other  applicable  credentialing  and  accreditation   agencies  and  applicable  provisions  of  law  and  the  rules  and  

regulations  of  any  and  all  governmental  authorities  pertaining  to:   (a)  Licensure  and/or  regulation  of  healthcare  personnel  in  treatment   facilities,  and  

(b)  The  regulations  and  standards  of  professional  practice  of  the   treatment  facility,  and,  

 (c)  The  bylaws  of  the  treatment  facility’s  professional  staff.  

Minimum  Education   Master  of  Social  Work  (MSW)  degree  from  a  school  of  Social  Work   accredited  by  the  Council  on  Social  Work  Education.
