The Funding Guide for

Young Children and Families

 This guide was developed by Infant and Child Development Services Durham to give parents/caregivers and service providers information about funding opportunities that may be available to families.

 The guide includes sources of funding from various levels of government, non-profit organizations, charities and wish foundations.

 The contact information is listed for each agency and/or source which provides the funding, with a description of the purpose of the funds, a summary of the eligibility criteria, and instructions on how to apply for or get more information on the funding.

ACHILD’S VOICE FOUNDATION

This non-profit foundation provides financial assistance to children who are physically and/or mentally challenged, after all other sources have been exhausted. The foundation has two programs: Kids Life-Line and Angel Hair for Kids.

Kids Life-Line provides one-time financial assistance for necessary products and services such as transportation costs, limited in-home nursing, counselling or therapy, groceries, parking permits, medication or medical/dental supplies that are not covered by the government or insurance. Angel Hair for Kids provides wigs and hair-loss solutions to financially disadvantaged children who have lost their hair due to a medical condition or treatment.

To access these programs, a community and/or healthcare professional must submit an intake form on behalf of the child.

This foundation grants wishes/dreams to children who are critically, terminally, and/or chronically ill. A World of Dreams grants wishes/dreams, such as trips to Disney World®, computers, electronic devices and meetings with celebrities.

To be eligible, a child must be under the age of 18 years and have a confirmed diagnosis (in writing) from a doctor.

To make a referral, the parent or healthcare professional can call, mail or e-mail the request to the foundation. The direct link to the referral from is:

ACCESS 2ENTERTAINMENT

Access 2 Entertainment is a non-profit organization that gives individuals with a disability a card that provides a support person with free admission or a significant discount on

admission to various entertainment venues in Canada. The individual with a disability will pay regular admission. Venues include movie theatres, Metro Toronto Zoo, Ontario Place,

Ontario Science Centre and more. There is a $20 fee to acquire the card and it will be valid for 5 years.

To be eligible, a person with a permanent disability must require a support person when attending a movie or other entertainment venues. No age restrictions apply.

Parents are able to claim the 15% non-refundable Children's Arts Tax Credit on up to $500 of qualifying expenses per child. Registration costs for participation in an eligible artistic,

cultural, recreational or developmental program may be claimed. Examples of eligible programs can be found at the following link:

To be eligible, the child must be under age 16 at the beginning of the year in which the activity fees are paid. If a child qualifies for the disability tax credit, they may be eligible to claim $1000 until the child is 18 years of age. Eligible programs must be ongoing (a minimum of once a week for at least eight weeks or five consecutive days), suitable for children, and supervised. The program must not be eligible for the Children’s Fitness Tax Credit. Claim this tax credit by completing the corresponding line of your Schedule 1, Federal Tax form. ACSD funding is a benefit that is paid monthly to families with a child with a disability. It is meant to help with the extraordinary costs that families have, due to the disability.

Extraordinary costs may include: special clothing and diets, special learning or developmental equipment, babysitting, transportation to the doctor, clinic, or hospital, and parent relief. The benefit also provides funds for eye glasses, dental and drug benefits for the child, as well as hearing aids and repairs to hearing aids.

To be eligible, parents or guardians must have a child with a disability who is under the age of 18 years and lives at home. The amount of funding a family may receive is calculated based on the total family income and the ongoing extraordinary costs and difficulties related to the care of the child with the disability.

The ADP covers up to 75% of the costs of equipment and other devices that will help people with a long-term physical disability increase their independence and quality of life. Social assistance benefits under Ontario Works (OW), Ontario Disability Support Program (ODSP), Assistance for Children with Severe Disabilities (ACSD), private insurance and other sources may cover the additional 25%. Types of equipment covered include: communication devices, diabetes equipment, hearing aids, home oxygen, orthoses, ostomy supplies, prosthetic devices, visual aids, wheelchairs, etc.

To be eligible, Ontario residents must have a valid Health Card and have had a physical disability for six months or longer. There are no limits to funding based on personal income. A doctor or health care professional will make a referral to a registered authorizer who will

Autism Ontario provides funding through the Family Funding Program (Durham Region Office), the March Break Reimbursement Fund (Provincial Office), and the One-to-One Summer Support Worker Reimbursement Fund (Provincial Office) to families with children who have been diagnosed with Autism Spectrum Disorder.

The Family Funding Program provides up to $200 for an individual with ASD to participate in a summer recreational program or obtain a respite care worker, depending on the individual’s abilities. There is no age restriction to be eligible for this program.

The March Break Reimbursement Fund provides up to $400 for families who either retain the services of a one-to-one support worker or pay for a March Break camp/program for their child/youth with ASD. To be eligible, an individual must be 18 years or younger and have an official diagnosis of ASD.

The One-to-One Summer Support Worker Reimbursement Fund provides up to $600 to Ontario families of children with ASD who attain the services of a one-to-one support worker for their child to attend a community summer camp or summer program. To be eligible, a child must be under the age of 18 years and have a documented diagnosis of ASD by a professional.

All application forms can be obtained online at the website above. Please note: Autism Ontario does not accept applications all year for these programs. Please refer to their website for specific dates and deadlines to apply.

CANADA CHILD TAX BENEFIT (CCTB) Canada Revenue Agency

The CCTB (formerly known as the “Baby Bonus”) is a non-taxable benefit paid monthly to help eligible families with the costs of raising and meeting the daily needs of a child. To be eligible, you must be a Canadian Citizen and primarily responsible for the care and upbringing of a child who lives with you and is under the age of 18 years. The benefit a family may receive is calculated based on the total family income.

You should apply when: your child is born, you become a Canadian Citizen, and/or a child begins living with you. If you are a mother of a newborn, you may apply through the

Automated Benefits Application when you sign your child’s birth registration. Otherwise, you must fill out Form RC66 (Canada Child Benefits Application) and return it to your local tax office. This form can be obtained from this link: http://www.cra-arc.gc.ca/E/pbg/tf/rc66/rc66-fill-10e.pdf.

Canada Disability Savings Bond:

The Government deposits money into the Registered Disability Savings Plans (RDSPs) of low-income and modest-income Canadians. If you qualify for the bond, you could receive up to $1,000 a year from the Government, with a limit of $20,000 over your lifetime.

Contributions do not need to be made to the RDSP in order to receive the bond. To be eligible, you must meet the following criteria: be 49 years of age or under, be a Canadian resident, have a Social Insurance Number, be eligible for the Disability Tax Credit and have a family income of less than $41,544. Parents/guardians opening an RDSP for their child (under 18 years of age), must have filed their taxes for the past 2 years and all future years.

Canada Disability Savings Grant:

The Government deposits money into your RDSP. Matching grants of up to 300%,

depending on the amount contributed and the Beneficiary’s Family Income, may be provided. The maximum is $3,500 each year, with a limit of $70,000 over your lifetime.

To be eligible, you must meet the following criteria: make contributions to your RDSP, be 49 years of age or under, be a Canadian resident, have a Social Insurance Number and be eligible for the Disability Tax Credit. Parents/guardians opening an RDSP for their child (under 18 years of age), must have filed their taxes for the past 2 years and all future years. You must apply for the RDSP, Canada Disability Savings Bond and Canada Disability Savings Grant at a participating Financial Organization where you want your RDSP to be opened. CANADIAN TIRE JUMPSTART CHARITY

Canadian Tire Jumpstart® is a community-based charitable program that helps children in financial need participate in organized sports and recreational activities. Activities can

include hockey, dance, soccer, and swimming, etc., but do not include summer or day camps. A maximum of $300 per child is funded to eligible families.

To be eligible, a child must be between the ages of 4 to 18 years. A financial assessment, including details about your family and desired choice of activity, may also be required. Applications are accepted in two sessions per year: for spring and summer apply between January 15th and June 1st and for fall and winter apply between July 1st and November 15th. Call the toll-free number provided above to apply.

CENTRAL EAST COMMUNITY CARE ACCESS CENTRE – ENHANCED RESPITE FUNDING

Enhanced Respite Funding is a program which provides a grant paid to eligible families who are caring for a child who is medically fragile and/or depends on a technological device needing care 24 hours a day, 365 days a year and is paid in addition to other respite

services. Families may be eligible for up to $3,500 per child, per year. Examples are children who rely on medical and technological equipment, such as mechanical ventilators, apnea monitors, renal dialysis, urinary catheters, colostomy bags, children who take drugs

intravenously, and/or children who rely on tracheotomy tube care, suctioning, oxygen support or tube feeding. There are no restrictions on the type of respite a family may purchase. Funds may be used flexibly to purchase in-home or out-of-home services or a combination of in-home and out-of-home services.

To be eligible, a child must be 18 years or younger and have care requirements resulting from medical or physiological condition(s) that require ongoing, frequent or time-consuming caregiver intervention and monitoring on a 24-hour basis for survival. In addition, children must fall into one of the categories of care requirements. Note: children with behavioural disorders alone are not eligible.

A family must be involved with the Community Care Access Centre and apply through their case manager or call the Ministry of Children and Youth Services at their Durham office at 1-877-669-6658.

CHILD CARE SUBSIDY

Subsidy is available to eligible families who are either attending school, working, receiving Ontario Works, or have a child who has a special need. Subsidy is available only in child care programs that have a Purchase of Service Agreement with the Regional Municipality of Durham and meet all provincial licensing requirements.

Eligibility for subsidy assistance is based on income, residence in Durham Region, and the eligible amount of hours of child care. An appointment with a Caseworker is necessary to confirm eligibility for and amount of subsidy.

To apply for subsidy phone the number provided above to be placed on the waitlist.

If the child has a special need, a social need or the parent is unable to care for the child due to illness or disability a Recognized Need for Childcare Subsidy form should be completed and submitted by a professional.

CHILD DISABILITY BENEFIT (CDB)

The Child Disability Benefit (CDB) is a tax-free benefit, paid monthly, for families who care for a child under age 18 with a severe and prolonged impairment in mental or physical functions. The impairment must last, or is expected to last, for a continuous period of at least 12

months.

The amount is determined based on family net income.

To receive the CDB, you must apply for the Disability Tax Credit by completing and submitting form T2201 to your local Revenue Canada office.

CHILDREN FIRST -SCHOOL CHOICE TRUST

This is a privately funded program that assists parents in financial need, to send their children to an independent elementary school. The program provides tuition grants of 50%, up to $3,500 per year.

Eligible children must reside in Ontario and be entering junior kindergarten to Grade 8. Recipients will need proof of financial need by verifying household income and net assets. There are no academic requirements.

To apply, complete an application form online or send the form by mail to Children First. Application forms can be obtained online.

CHILDREN IN NEED OF TREATMENT (CINOT) Ontario Ministry of Health Promotion and Sport

This program provides emergency dental care and out-of-hospital anesthetic coverage. Children can receive urgent dental care free of charge.

To be eligible, a child must be 17 years of age or younger, possess a valid Ontario Health Card Number, and have a dental condition requiring emergency or essential care. The parents must sign a written declaration stating that they have no dental insurance and that the cost of dental treatment for their child would result in financial hardship to the family. To apply, contact your local Public Health Department to request a CINOT Claim Form. The claim form is valid for six months and must be submitted to the dental office before or upon making the appointment. The contact information for Durham Region residents is listed above.

CHILDREN’S RESPITE FLEXIBLE FUND

This funding is available for the following: respite worker at home or camp, camp fees, fees for social or recreational groups, classes or lessons. Please note that this is a

reimbursement funding process; therefore, parents/guardians must pay for the service or camp first, and then submit a receipt. Funds are limited. Approval is needed before

reimbursement is provided.

Eligible children are 17 years of age or younger and have a diagnosis of Autism Spectrum Disorder.

To apply, parents must register with Kerry’s Place through the following link:

https://secure.kerrysplace.org/reff/ReFF/ValidateEmail.aspx. Once registered an application for respite funds can be submitted online through the website.

CHILDREN’S WISH FOUNDATION OF CANADA

This foundation is dedicated to fulfilling wishes for children suffering from a life-threatening illness. Examples of wishes may include: trips, computer equipment, meetings with celebrities or athletes, and a new puppy.

To be eligible, children must be between the ages of 3 and 18 years at the time the request is received. They must be legal residents of Canada and have an illness, which is categorized as a high risk, life-threatening condition.

Anyone can make a referral for a child they know is eligible. However, the child’s parents or legal guardians must communicate their interest in their child’s wish being fulfilled. Please call or e-mail your referral at 905-427-5353 or on@childrenwish.ca.

Professional counsellors provide a detailed financial assessment (free of charge) to identify possible solutions to your financial concerns. CCS programs include money management counselling, debt management programs, and help with resolving creditor problems.

There is no eligibility criterion for receiving the free financial assessment. The counsellor will look at an individual’s level of income, debt, and monthly living expenses.

This program provides opportunities to persons with cerebral palsy to participate in structured and meaningful activities of their choice in local communities. The Disability Travel Card provides an identification card to a person with a permanent disability, which will allow an adult attendant, travelling with the person with a disability, to travel at no cost by plane, train or boat. The person with the disability pays regular admission. An attendant is an individual who accompanies a person with a permanent disability who cannot travel independently.

Only persons with a permanent disability who require a support person when travelling can apply for the card. There are no age restrictions. (Please note: Via Rail only provides a discount for people with a disability over the age of 13. Anyone under the age of 13 would not be allowed to travel without an attendant).

This program provides low income Durham residents an interest free loan of up to $500/year for utility costs (gas or electricity) due to unexpected circumstances. The energy fund helps those who are usually able to afford their energy costs but due to an unexpected financial burden or unanticipated cost increase, are unable to pay their heat and/or hydro for a short period of time. Financial assistance can also be given for the purchase of home heating oil or chords of wood.

This grant helps Northern Ontario residents who must travel at least 100 kilometers one-way for a medical specialist or designated health care facility that is not locally available. The grant can pay up to $100 for accommodation and 100km of travel costs.

To be eligible, an individual must be an OHIP insured Ontario resident living in one of the following areas: Algoma, Cochrane, Kenora, Manitoulin, Nipissing, Parry Sound, Rainy River, Sudbury, Timiskaming, or Thunder Bay (not Durham Region). A Northern medical

practitioner must complete the referral to the specialized health care.

OCB provides financial support to working and non-working, low-income families. Payments are based on the number of children under the age of 18 years in the family as well as the family’s adjusted net income.

To be eligible, you must be a permanent resident of Ontario and the primary caregiver for a child under 18 years of age. You must have filed your previous income tax return and have registered your child for the federal Canada Child Tax Benefit.

There is no application form for OCB. Once you and/or your spouse/common law partner have filed your income tax returns, the Canada Revenue Agency will automatically review your eligibility. If you are eligible, your payments will be included with the Canada Child Tax Benefit.

This program provides eligible, low income, working families with monthly financial support. Your child does not have to be enrolled in daycare to apply for OCCS.

To be eligible, you must be a resident of Ontario and your child must be under the age of 7 years and born before July 1, 2009. You must meet the qualifying income level or have qualifying child care expenses. In addition, you must be receiving the Canada Child Tax Benefit and are not receiving fully subsidized child care.

To apply, file your annual income tax return. Once you have done this, the Ministry of Revenue will send you the application form for OCCS to complete.

This program covers up to 75% of the pre-approved dental specialist treatment costs not covered by private dental insurance. Patients are entitled to funding for only one treatment. To be eligible, a child must be 17 years of age or younger, have a valid OHIP number, and have a diagnosis of cleft lip and/or palate, craniofacial anomaly, or any other severe dental dysfunction.

To obtain financial assistance, an individual must have a screening assessment done first at a designated clinic (Holland Bloorview Kids Rehab and the Hospital for Sick Children). All requests for reimbursements must be sent to the Hospital for Sick Children.

ONTARIO DISABILITY SUPPORT PROGRAM (ODSP)

The ODSP provides financial support to adults with a disability to help pay for living expenses such as food, housing, etc.

To be eligible, you must live in Ontario and be 18 years of age or older. You must have a substantial physical or mental disability that is expected to last more than a year and that interferes with your ability to care for yourself. The amount of funding you receive depends on your financial situation.

The Assistive Devices Funding Program provides financial assistance for equipment for persons with cerebral palsy (CP) in Ontario. A full list of funded equipment can be found on the website. A maximum of $2,500 will be funded per person per year.

To be eligible, you must have cerebral palsy and be an individual member of the OFCP with a valid Ontario Health Card. To become an OFCP member, complete the application form found at this link: http://www.ofcp.ca/pdf/mem/OFCP_Individual_Application_Form.pdf. Send the completed form to the mailing address provided above.

To apply for the Assistive Devices Funding Program, you must print and complete the application form found at this link: http://www.ofcp.ca/pdf/adfp/application_form.pdf. PARTNERSHIP IN SERVICE

 

Partnership in Service provides people with special needs, within the Region of Durham, with financial assistance through the support of various Social Agencies, Service Clubs,

Public/Private Sector Organizations. Financial assistance is provided for equipment, medical supplies and home or vehicle renovations when all other sources have been exhausted. A letter from a professional is required outlining the specific needs of the child/family and the reason for the request. Families should speak with their service co-ordinator if they wish to apply.

Precious Minds offers financial assistance to families living in North Durham to help with extraordinary costs such as assessments, specialized equipment, therapies, summer camps, respite, parent training and tutoring. The program pays 50% towards approved requests to a maximum of $500 per child.

To be eligible, the child must be between the ages of 0 and 18 years with a barrier to learning and reside in North Durham Region (includes Uxbridge, Brock, and Scugog).

The PCCC provides grants to families to assist with the costs of mobility equipment, home capital, respite care, camp, environmental modifications, ABA, developmental and physical therapy and accessible vehicles.

To be eligible, your child must be 18 years old or younger and have a physical or

developmental disability. In addition, family income must be $70,000 or less. Families may re-apply for funding every five years.

The application form must be mailed in prior to the equipment and/or service being purchased or received. If you are applying for camp, PCCC asks that you submit your application before May 1st. CMHC offers financial assistance to homeowners/landlords for modifications to a property to make it more accessible to persons with disabilities. These modifications are intended to eliminate physical barriers, safety risks, and to improve the ability to meet the demands of daily living. Examples of eligible modifications include ramps, handrails, chairlifts, height adjustments for countertops, and cues for doorbells/fire alarms. The program does not fund portable aid equipment such as wheelchairs and walkers, supportive care, and therapeutic care.

The eligible property must be occupied, or intended to be occupied, by a low-income person with a disability. Also, the property must be rented or valued below established levels for that area and be without any major structural or system deficiencies.

SSAH supports individuals with developmental and physical disabilities to live as

independently as possible with their own families. This program provides financial assistance to purchase family relief or programming services when they are not available from another source.

To be eligible, individuals diagnosed with a developmental or physical disability must live at home and need more support than most families can provide.

Formulary/Comparative Drug Index. Drug products that are not listed on the Formulary will be considered through the Ministry’s Exceptional Access Program on a case-by-case basis. To be eligible, you must be a resident of Ontario, have a valid Ontario Health Card, and do not qualify for drug coverage from another Ontario Drug Benefit source. Also, you must not have private insurance coverage or if you do, the private insurance does not cover 100% of your prescription drug costs.
