and available in sufficient quantities. There was a tea party taking place on the first day of the inspection and residents appeared to very much enjoy the event. These are monthly occurrences and the chef prepares a variety of foods for the residents, including pastries, as a special treat for residents. Residents requiring assistance at meal times are assisted appropriately by staff and a special table is available for residents that have large speciality chairs. While there was an adequate system for nursing staff to communicate speciality diets with catering staff, it was noted by the inspector that for one resident the diet sheet in use did not reflect the prescribed diet.

There were a number of policies and procedures to guide practice in relation to the management of nutrition, including the provision of therapeutic and modified diets, management of hydration, enteral feeding and meals and mealtimes. There were systems in place to ensure residents' nutritional needs were met, and that they did not experience poor hydration. Residents were assessed for the risk of malnutrition on admission and at regular intervals thereafter. Residents' weights were checked on a monthly basis, and more frequently when indicated. There was an effective system of communication between nursing and catering staff to support residents with special dietary requirements. Inspectors found that residents on weight reducing, diabetic, high protein and fortified diets, and also residents who required modified consistency diets and thickened fluids received the correct diet and modified meals were attractively served. Based on a sample of records viewed by inspectors, residents' needs were met to a high standard of nursing and medical care as they approached end-of-life. This was supported by feedback obtained from relatives of deceased residents who completed questionnaires on the care provided to residents at end-of-life. Nine questionnaires had been sent to relatives of deceased residents and eight had been returned to the Authority. All were very complimentary of the care provided.

It is located on the outskirts of a town where community facilities, resident’s general practitioners (GPs) and pharmacy services were available. A mature garden around the centre and a secure courtyard and garden area adjoining it was available. A spacious car park was available at the front of the centre. Entry was via the main front door in to the reception area.

However, residents with a do not resuscitate (DNR) status in place did not have the DNR status regularly reviewed to assess the validity of clinical the judgement on an on going basis. Most residents had a plan of care for end-of -life. The care plans contained detail of personal or spiritual wishes. Decisions concerning current and future healthcare interventions and advance care planning required review. Resident’s preferences with regard to transfer to hospital if of a therapeutic benefit were not documented in all of the end-of -life care plans reviewed.
