Low-Fluence 585 nm Q-Switched Nd:YAG Laser: A Novel Laser Treatment for Post-Acne Erythema

Results: At 6 weeks after 3 sessions of laser treatment, all patients demonstrated clinical improvement. Erythema lesion counts decreased by 20.1% (versus baseline) after the first treatment (P ¼ 0.004), by 32.7% after the second treatment, by 46.5% at 2 weeks after the third treatment and by 58.7% at the 6-week follow-up (all P < 0.001). Significant improvements were also noted in erythema indices (22.29 2.4 to 17.51 1.8) and mean post-acne erythema scores after the first treatment (both P < 0.001). The mean scores of independent physician assessments were 4.04 0.9 in term of the improvement of post-acne erythema and 3.44 0.9 in the improvement of scaring. In addition, we could observe a significant decrease in inflammatory acne lesion counts after two laser treatments with a decrease in mean lesion counts by 67% at the 6-week follow-up. Treatment was well-tolerated and adverse effects were limited to transient erythema and edema at treatment sites. Conclusions: Low-fluence 585 nm Q-switched Nd:YAG laser treatment is safe and effective for the treatment of post-acne erythema with minimal discomfort and quanti- fiable improvement in the appearance of early acne scarring and inflammatory acne. Abstract: Background: Complicated Infantile Hemangiomas (CIHs) clearance takes over 6 months or incomplete with propranolol treatment alone, and frequently relapses after drug discontinuation. In order to evaluate the ef- ficacy of concurrent versus sequential combination therapy of systemic propranolol with dual-wavelength laser (585 nm and 1064 nm) in the treatment of CIH, sixty-one CIH patients were randomized into two groups. Patients in group A (sequential therapy, 30 cases) were treated with oral propranolol (1-2 mg/kg/d) until maximal reduction of tumor size was achieved, after which laser treatment was initiated. Patients in group B (concurrent therapy, 31 cases) were treated with oral propranolol (1-2 mg/kg/d) for one week before laser was applied concurrently. The size and color of tumors were observed and recorded to assess the treatment efficacy. As a result, the time to cure CIH in group A and B was 14.0±0.87 months and 9.0±1.21 months, respectively (P<0.05). No severe side effects were observed. In conclusions, combination of oral propranolol and dual-wavelength laser was safe and effective for CIH treatment and concurrent therapy was found to be superior to sequential therapy.

As increasing the reaction time from 3 to 10h, intensity of bands attributed to Mn 2+ ions also increase, reaches the maximum at 15h and then decreases as increasing reaction time to 30 h. In contrast, the reflectance is substantially higher at 625 nm, since the electric and magnetic fields couple now to the far field. At this wavelength, the reflectance seems dominated by the back-reflector mode, which at 625 nm exhibits a local maximum. Additionally, the electric and magnetic field amplitudes at 625 nm are smaller than at 585 nm, therefore the light trapping within the nanostructure is weaker. The hybrid nanostructures with 180 nm and 380 nm polymer thickness exhibit also very similar near-field distributions at the nanodisc air and air polymer interfaces (Figure 2d). At 625 nm, the light is completely trapped within the hybrid nanostructure, since the electric fields exhibit multipolar character and the magnetic fields are mostly confined within the polymer layers. However, additional electromagnetic modes can be seen in both; the electric and the magnetic field distributions of the hybrid structure with a polymer thickness of 380 nm. We attribute these modes to the back reflector modes. The near field distributions Figure 2c indicate that the re-occurrence of the spectrally sharp features also depends on the spatial separation of the localized nanosdisc modes with the local maxima and minima of the back reflector modes. The electric and magnetic field distribution at 585 nm reveal another aspect of the interactions between the back-reflector modes and the localized plasmon modes of the Au nanodiscs. As discussed previously, the back reflector modes exhibits a local maximum at around 585 nm for a 180 nm thick polymer film. Due to the interaction between the back reflector modes and the localized modes at 585 nm, the electric field distributions exhibit a dipolar character. Coupling to the far field is now facilitated, which results in a higher reflectance than at 625 nm.

Compared to previous generation of TiAlN coatings, TiAlCrN/TiAlYN exhibit significantly higher resistance to oxidation and to abrasive wear. It has been reported that the onset of rapid oxidation of TiAlN in thermo-gravimetric tests has been increased from 750 0 C to 850 0 C and further to 950 0 C by the incorporation in the coating of 3 at.% Cr and 2 at.% Y respectively [25, 26]. It has been shown in [25] that the incorporation of a large and therefore relatively immobile Y atom (atomic radius 0.18 nm) leads to repeated re-nucleation in the growing film, resulting in a much finer grain size (typically in the range of 5 – 10) compared to that of the TiAlN and a near equi-axed grain morphology. The incorporation of Y in TiAlCrN in a layered manner has been found to be from a key importance for its efficiency to enhance both the oxidation resistance as well as the high temperature tribological behavior of the coating.
