Staff told the inspector they had received a broad range of training which included dementia care and managing responsive behaviours. All of our residents will be involved in the decision-making process and will be kept up to date as the plan progresses. The inspector spoke with a group of residents during their morning coffee and spoke with a small number of individual residents over the remainder of the day. Residents said they find the home comfortable and that it offered the right balance of time alone and time together. They felt that the large communal area was conducive to chatting or sharing views as small groups could sit together for coffee and meals. Residents said they could go out on their own if they wished around the pleasant garden area, but some said they were unable to do that due to limitations in their mobility or their need for supervision from staff.

This was an unannounced inspection to monitor ongoing compliance with the Regulations. The inspector followed up on progress with completion of actions from the last inspection in April 2018. There were non compliances with four regulations found on the last inspection and improvements to bring medicine management into compliance were satisfactorily implemented. Refurbishment work was ongoing to upgrade the first floor of the premises. Non compliance found during the last inspection regarding provision of opportunities for residents to participate in meaningful activities to met their interests and capabilities was not satisfactorily addressed. These non-compliances were found again on this inspection and are restated in the compliance plan with this report. The inspector followed up on information in notifications and unsolicited information received by the Office of the Chief Inspector since the last inspection in April 2018.

Staff were effectively supervised by the person in charge and nursing staff were observed to be available to provide advice and guidance throughout the inspection. As described throughout this report staff engaged positively with residents and to their requests for assistance. Staff the inspector talked to were enthusiastic about their work and said they enjoyed the company of residents and ensured they made a positive contribution to residents’ quality of life each day. They said they were familiar with the standards of care expected and worked hard to achieve high standards every day.

Supervision continues to be provided by CNM’s and Team Leaders, and supported by the ADON and PIC. Supernumerary hours are allocated to the people in these roles to ensure supervision is available on a consistent basis. Feedback is provided to supervisors and staff across all floors to ensure a consistent approach is taken. The PIC meets with the CNMs and Team Leaders every two weeks to provide guidance and agree actions. Every 3 months, the PIC completes a night check including walk around of all units, checking on each resident as well as the check list on each floor. This was last completed on 12th October 2018.

The inspector noted that the dementia training included an item called 'a take home message' which was a change that the staff member was going to undertake as a result of the training. This was then reviewed a week later to check progress. Staff spoken with told the inspector how valuable they found this element of training.

There was a clear complaints procedure in place and information in relation to the complaints procedure was available in the resident's guide and was displayed in the centre's reception area. The person in charge maintained a log of formal complaints that were received in the centre however there was no log of informal complaints received by staff on the units and no record of how these had been managed. Residents and families who spoke with the inspectors were aware that there was a complaints process in place but a number of those spoken with were not aware of who was responsible for managing complaints in the centre. Residents and families told the inspectors that when they raised issues with staff on the units the issues had been dealt with at the time but that the same problems would recur again and they had to report the issue a second or third time. Records showed that one resident council meeting had been held in the centre in January 2018, however these meetings should have been held every quarter in line with the centre’s

The centre is a two storey building containing three distinct lodges located on the outskirts of Longford town. Glencar Lodge is a 40 bed dementia specific unit. Lissadell Lodge is a 34 bed unit and Hazlewood lodge had 40 beds. The majority of bedrooms have full en-suite facilities. The centre is decorated and furnished to a high standard and a variety of sitting rooms and seated areas, dining rooms in each lodge, a spacious oratory/chapel, a meeting room and hair salon is available for residents use. Well-manicured secure and accessible garden courtyards are available along with a number of other surrounding outdoor planted areas. Menus were available and all residents were offered choice at each meal. Residents on modified diets were provided with the same choices and each element of the meal was separately presented on the plate. The inspector observed residents having their lunch in the dining room, where a choice of meals was offered. All of the residents in the dining room were capable of having their meal independently. The assistant director of nursing supervised the dining experience for residents and sat and chatted with them or assisted to source additional condiments, mint sauce or drinks. Residents who required assistance with their meals were in the main sitting room and the inspector observed that appropriate assistance was given and staff were noted to patiently and gently encourage the resident throughout their meal.

The single storey building was built as a nursing home in 2003. It was located near Milltown village in a scenic, country area. There were extensive gardens surrounding the building and residents had access to these for walks with family and staff. Adequate car parking spaces were provided. The person in charge stated that renovations were undertaken on an annual basis and the décor of the centre was consistently upgraded by the full-time maintenance person. Sitting, dining, dayroom and recreation rooms were spacious and provided adequate opportunity to allow private family visits, social events and communal activity sessions.
