The following table contains a list of equipment that was evaluated for this order of approval for the Yahoo! Inc., Data Center (Yahoo!) located at 1010 Yahoo! Way and 1500 M Street NE, Quincy, WA. Engine sizes listed in the tables are in megawatt (MWe) units with the “e” indicating “electrical” based on generator power ratings listed on the engine specifications. Thirteen (13) existing 2.0 MWe MTU Detroit Diesel emergency generator unit identification numbers R through 12 were approved in Notice of Construction (NOC) approval Order No. 07AQ-E241 issued on November 13, 2007. Order No. 07AQ-E241 was rescinded and replaced by NOC approval Order No. 11AQ-E399 issued on March 28, 2011. Order No. 11AQ-E399 included the original 13 engines and also ten (10) 2.0 MWe MTU Detroit Diesel emergency generator units with identification numbers 13 through R3. Twenty five (25) new emergency generator units at the facility were proposed in Yahoo!’s Project Genesis final NOC application submitted to Ecology on December 23, 2015 and will have capacities of 2.0 MWe (20 units) and 2.75 MWe (5 units). Yahoo!’s application provided Ecology with a combination of the following anticipated engine manufacturers and models to be used for the 25 new engines: Caterpillar Models 3516C and 3512C; Cummins DQKAB and DQLF; MTU 16V4000 DS2000 and 20V4000 DS2800.

Effective October 1, 2011, the SDC will be operated by the newly created Consolidated Technology Services (CTS) agency. ESSB 5931 established CTS as the centralized provider and procurer of certain information technology services to maximize benefits to the state from advances in information technology. To successfully meet agency needs and meet its obligation as the primary service provider for these services, CTS must deliver high quality services to state agencies and other customers at the lowest possible cost to taxpayers. This project facilitates the migration and consolidation of IT assets into the SDC in support of this legislation and the governor’s directive to consolidate data centers. It also supports the effective management of enterprise services and the transition of how the state plans, develops, implements, funds, and maintains information technology. This project is financed through funds allocated by the legislature and construction bond fund savings.

against their employers for personal injuries in the workplace resulting from employer negligence arises under the Industrial Insurance Act. W ASH . R EV . C ODE § 51.04.010 et seq. (2006). Washington courts have no original jurisdiction over such actions: “The state of Washington, therefore, exercising herein its police and sovereign power, declares that all phases of the premises are withdrawn from private controversy, . . . and to that end all civil actions and civil causes of action for such personal injuries and all jurisdiction of the courts of the state over such causes are hereby abolished, except as in this title provided.” Id. at § 51.04.010. In the first instance, worker compensation is determined by a State agency, the Department of Labor & Industries, with hearings conducted by administrative judges without a jury, and appeals considered by a three judge panel at the Board of Industrial Insurance Appeals. See, e.g., id. at § 51.28 et seq; id. at § 51.52.010 (2006). Awards are based upon a schedule of damages widely regarded as less generous than those available in the civil justice system, but without having to prove “fault.” Id. at § 51.32.010. For instance, complete loss of hearing in both ears and loss of one eye by enucleation are compensable as permanent partial disabilities for $43,200 and $21,600, respectively, plus adjustments per the Consumer Price Index (CPI). Id. at § 51.32.080. Workers, however, may recover damages over and above scheduled workers compensation benefits if they can establish that the injury was a consequence of the “deliberate intention of his or her employer to produce such injury.” Id. at § 51.24.020.

Patients were mostly likely to file a complaint at 28% with DOH (Department of Health) slightly lower at 26%. It should be noted that the majority of complaints filed by the DOH were on licensees who did not complete their continuing education (CPE) or had misrepresented themselves on their application and received a watch hit on a background check. Other sources of complaints were providers, employers, employees, other Boards and Commissions, peers, and self-reports.

Cause of death in Washington state veterans hospitalized with acute coronary syndromes in the veterans health administration

Between October 2003 and December 2005, there were 524 individuals who were admitted to VA hospitals located in Washington State or Oregon and had a diagno- sis of acute myocardial infarction or unstable angina. These individuals were Washington State residents (iden- tified by residential zip code) admitted to VA hospitals or were non-state residents admitted to 1 of 3 VA hospitals in Washington State. According to VA death records, 136 (26%) died prior to January 1, 2006. Using the social security number, full patient name, and date of birth, these records were linked to death records obtained from the Washington State Department of Health. This process identified 117 (86%) decedents in state death records. Of the 19 who were not found, 8 were Washington State res- idents, 6 were residents of Idaho, 3 – of Alaska, 1 – of Montana, and 1 – of Oregon. Individuals with missing death records were similar to those whose death records were found in that the two groups did not differ with respect to age, gender, race, or year of death.
