The process of completing student performance evaluations is undertaken by both the student and field instructor.

Both the student and the field instructor will independently complete this evaluation and discuss in supervision.

The field instructor will submit her/his evaluation to the faculty liaison.

The student will submit her/his evaluation to the faculty liaison.

If a student receives a rating of “1” on any of the outcomes they cannot pass the field education courses.

B. Rating Scale

4= Exceed Expectations: This score reflects the student has exhibited performance skills of a very high level and has practiced well above the expected level.

3= Meets Expectations: This score reflects the student is performing at the expected level.

2= Below Expectations: This score reflects the student is performing below the expected level and is displaying certain limitations in knowledge, skills, values, and ethics, with these specific practice behaviors.

1= Failed To Meet Expectations: This reflects that the student is performing below the expected level and is displaying significant limitations in knowledge, skills, values, and ethics with these specific practice behaviors.
