This Statement of Work # 1 (“Statement of Work”) describes services to be performed by CS
STARS LLC (“CS STARS”) for Broward County Board of Commissioners(“County”). This
Statement of Work is effective upon the execution of the Agreement and will remain in effect through Migration Closure (as defined herein), unless terminated sooner in accordance with the Agreement.
STARSTM Enterprise Edition is software for claims administration of Workers’ Compensation
and various lines of Liability claims; for making payments on claims; to transmit mandated data via EDI to the State for Workers’ Compensation records; to transmit mandated Medicare Section 111 EDI; for property tracking to include values, Construction, Occupancy, Protection, and Exposure (COPE) data; and for reporting.
The implementation/migration project involves migrating County’s locally-hosted STARS
database to CS STARS’ ASP servers. In addition, the project involves migrating County from
STARS Professional Edition to STARS Enterprise. The screens, fields, codes, reports, and event triggers that County currently utilizes will be present in STARS Enterprise. Furthermore, the file import/export configurations existing in Stars PE will need to be reconfigured within Stars Enterprise.
This SOW includes annual training days and outlines the way service hours will be billed back to the County.
Any capitalized terms not defined in this Statement of Work shall have the same definitions as set forth in the Agreement.

1. Project Scope and Deliverables
The following describes all initial Services to be performed by CS STARS under this Statement of Work.
A. Required Meetings

Kickoff Meeting Initial meeting to kickoff project Kickoff meeting will be conducted via
conference call.
At the Kickoff meeting, a draft
migration plan will be created, including timeframes, by County and Stars. Stars will provide the final document within two (2) weeks after this meeting. Should County prefer an onsite Kickoff
Meeting, travel costs, if any, will be billed as incurred to comply with County’s Travel and expenses policies, including as provided in Article 4.
B. Migration of Standard System Configuration
The following deliverables will be configured using standard configuration tools in the STARS software.

This will include all fields currently set up in Center for Medicare & Medicaid Services (CMS) Admin and existing data in records.

The configuration of STARS Universes1 and
labeling of custom fields

Fundamentally, the universe enables the end-user of business intelligence to interact with data without having to understand the complexities of database logic or be familiar with where the physical data is stored.

C. Technical Implementation Services

NOTE: County is responsible for complying with the minimum technical requirements as provided in the Client Machine Requirements Hardware and Software Documentation attached to this Statement of Work.

Migration of existing STARS PE reports as follows:

Report definitions for reports that have been generated within 18 months from the migration start date will be migrated.
Existing custom templates
Custom report templates to be migrated are outlined in the Custom Reports Appendix
Custom and Ad Hoc templates identified for replication are those generated within 18 months from the migration start date. Previously configured My Reports and

Live instructor led training for
large groups of County users. 2 days (8 hours maximum per day) of onsite instructor led training for up to 15 colleagues per class at County’s Fort Lauderdale, FL location or at a local Fort Lauderdale CS STARS training facility. Acceptance Testing and Migration Closure
County User preliminary Acceptance Testing is required for approval of all deliverables outlined in the Project Scope and Deliverables section. If County does not provide CS STARS with feedback, the deliverable will be deemed accepted thirty (30) days after delivery. Migration Closure is defined as the following:
County User Final
Acceptance Testing period has elapsed.
The deliverables outlined above are provided in substantial conformity to the specifications set forth in this Statement of Work:
County is provided a migration services closure letter and any supporting data validation documentation created as part of the deliverables.
Access to STARS Professional Edition is discontinued
CS STARS will provide County with project deliverables for User Acceptance Testing and will provide County guidance on the testing process.
County has attached Final Acceptance Test Plan to be used for Final
Acceptance testing
Final closure when the County’s Final Acceptance Test Plan criteria, which is attached, has been met as determined solely by County and CS STARS has been notified in writing by County of the system’s Final Acceptance.
