BSW STUDENT EVALUATION OF PLACEMENT (FORM C) UNDERLYING PRINCIPLES

As the practicum component is a core element of the curriculum and because students evaluate classroom instruction, similar practices are appropriate in relation to field instruction and faculty consultation. This process is expected to be ongoing between both the student and Field Instructor and the student and Field Consultant throughout placement. During supervision and consultation, the student will be sharing positives and negatives with regard to the placement. This ongoing evaluation also prepares the student for professional life and the performance appraisal process.

The following guidelines are intended to assist the student in developing a near end of Practicum evaluation of both Field Instructor and Field Consultant.

Completion of the evaluation is mandatory.

Please submit completed evaluations to the Field Education Coordinator.
