My name is Dr James Okereke. I am a staff of the Department of Paediatrics, University College Hospital, Ibadan. I am carrying out a study on ‘stooling pattern of apparently healthy secondary school children in Ibadan’. I will like to ask you some questions about yourself.

Your answers will be kept very confidential (in secret). The information you and other students will give me will be helpful to Doctors in future to make better decisions on treatment of children who may have problems related to stooling.
