Items 60, 611 and 612: Bankruptcy
The bankruptcy documentation you are required to provide depends on:
• If you ever filed for bankruptcy (item 610)
• If you are an undischarged bankrupt (Item 611).
• Whether or not OSAP is a creditor in your bankruptcy or related event.
• Date you filed for bankruptcy or initiated a related event (Item 612)

(Item 610 is “Yes” and item 611 is “Yes”)
You must provide documentation from your trustee in bankruptcy indicating the following:
• Date the bankruptcy was filed.
• That the following conditions have been met:
• No student financial assistance programs funded by the Province of Ontario and/or the Government of Canada are creditors in the bankruptcy.
• None of the funding given to you through any OSAP program in the 2014-2015 OSAP year (August 1, 2014 to July 31, 2015) will be seized to repay any creditor(s) listed in the bankruptcy.
Undischarged bankrupt, filed for bankruptcy after May 11, 2004, and OSAP is a creditor (Item 610 is “Yes”, item 611 is “Yes”, and Item 612 before May 11, 2004)
You must provide:
• Proof that you have no outstanding balance on any student loans.
• If you have been released from your obligation to repay your student loans by reason of a court-ordered absolute order of discharge which states that section 178(1)(g) of the BIA no longer applies to your student debt, you provide proof that an order of discharge was made and that three years have elapsed since the day the order was made.
• You must provide proof that you meet the following four conditions:
• At the time you filed for Bankruptcy or initiated a related event, you were enrolled in an approved program of study at an approved school and you were taking the minimum required course load.
• You continue to be enrolled in the same approved program of study in which you were enrolled at the time that you filed for bankruptcy or initiated a related event.
• You have not had a break in studies of longer than six months since the date of Bankruptcy or related event.
• It has not been more than 3 years since the date of Bankruptcy or related event.
In addition, you must provide proof from your trustee indicating the date you filed for bankruptcy or initiated a related event and that the following two conditions have been or will be met:
• Neither Ontario nor Canada is a creditor in the Bankruptcy or related event as a result of financial assistance given to you through OSAP.
• No financial assistance given to you through OSAP in the 2014-2015 year (August 1, 2014 to July 31, 2015) will be seized to repay any creditor(s) listed in the Bankruptcy or related event..
Discharged bankrupt/initiated a related event, filed for bankruptcy after May 11, 2004, and OSAP is a creditor (Item 610 is “Yes”, item 611 is “No”, and Item 612 before May 11, 2004)
Note: Initiate a related event refers to a consolidation order under Part X of the Bankruptcy and Insolvency Act (BIA) or a consumer proposal.
You must:
• Provide proof that you have no outstanding balance on any student loans.

Section B: Current Status Information
Item 200 Married
You must provide a copy of your marriage certificate or an affidavit signed by you and your spouse confirming the date of your marriage.
Item 210 Common-law relationship
You must provide an affidavit signed by you and your spouse confirming that:
• you have lived together continuously for a period of not less than three years, or
• are in a relationship of some permanence and are the natural or adoptive parents of a child.
Item 220 Sole-support parent
The following documentation is required based on your response in item 221:
Separated:
You must provide a copy of one of the following documents:
• Separation agreement
• Court order
• An affidavit of separation as proof of your marital status.
The documentation must include the date of your separation, details of your custody arrangements that confirm your child(ren) will be residing with you full-time during your study period, and the birth dates of those children.
Divorced:
You must provide a copy of one of the following documents:
• Divorce judgement
• Court order
• An affidavit of divorce as proof of your marital status.
The documentation must include the date of your separation, details of your custody arrangements that confirm your child(ren) will be residing with you full-time during your study period, and the birth dates of those children.
Widowed:
You must provide an affidavit confirming that your child(ren) will be residing with you full-time during your study period and the birthdates of those children.
Never Married:
You must provide an affidavit confirming that your child(ren) will be residing with you full-time during your study period and the birthdates of those children.
Item 231 Separated, divorced or widowed AND have no dependent children living with you The following documentation is required based on your response in item 231:
Separated:
You must provide one of the following documents:
• A copy of your separation agreement,
• A copy of your court order
• An affidavit of separation as proof of your marital status.
The documentation must include the date of your separation.
Divorced:
You must provide a copy of one of the following documents:
• Divorce judgement
• Court order
• An affidavit of divorce as proof of your marital status.
The documentation must include the date of your separation.
Widowed:
You must provide a copy of your spouse’s death certificate that includes the date of your spouse’s death. If you are unable to provide this document, contact your Financial Aid Office.
