27.1 Multi-Element Non-Recurring Charges (Continued)

A "Line Connection Charge" (L) applies to work done in the Central Office and/or outside plant facility associated with providing each access line (the line from the serving central office up to and including the station protector at the customer's premises), or making changes thereto.

A "Trip Charge" (T) is applicable when a Company employee makes a field trip (including but not limited to the customer's premises) to perform requested work, other than disconnect work.

An “Installation Expedite Charge” (E) is applicable for specific customer requests to have a complex service installed sooner than within the normal interval. Complex service is defined in this instance as any service request that installs or relocates 5 or more single party lines, Centrex lines or NARS, and PBX or DID Trunks. Complex services also include any intra or interexchange private line, digital data, 1.544 Mbps or higher circuits, ISDN services, and FX services.

Regular multi-element non-recurring charges are as follows:

27.2 Link-Up Oklahoma

A. Applicability

The Link Up Oklahoma Service Connection Program is a federally sponsored assistance program designed to make telephone service accessible to low-income residential households who are currently not on the public switched network.

B. Discounts

Customers deemed eligible for Link-Up Assistance will receive a reduction in the eligible customary service charge for commencing telecommunications service for a single telecommunications connection at the customer’s principal place of residence.

The monthly discounted service charge shall be 50% of the customary charge or $30, whichever is less.

A qualifying low-income customer may receive a deferred schedule for payment of the charges assessed for commencing service, for which the customer does not pay interest.

The interest charges not assessed to the customer shall be for connection charges of up to $200 that are deferred for a period not to exceed one year. Charges assessed for commencing service include any charges that the Company customarily assesses to connect customers to the network. These charges do not include any permissible security deposit requirements.

NOTE: These charges do not include an amount for security deposit requirements, any applicable aid to construction charges, inside wire charges and/or other applicable charges.

C. Eligibility Requirements

The Eligibility Requirements for Link-Up Service are the same as those for Lifeline Service. Customers who qualify for Lifeline Assistance automatically receive the Link-Up discounts set forth in this section. See Section 16.1.2 for Eligibility Requirements applicable to both Lifeline and Link-Up Service.

D. General

The credit is available on the installation of a single residential access line to the principal residence of the eligible customer.

A qualifying low-income customer shall receive the benefit of the Link-Up program for a second or subsequent time only for a principal place of residence with an address different from the residence address at which the Link-Up assistance was provided previously.

27.2 Link-Up Oklahoma (Continued)

E. Link Up America – On Tribal Lands

A) The Link Up America on Tribal Lands program is available to eligible applicants who certify residence on tribal lands as defined in Title 25, Code of Federal Regulations, Section 20.1, paragraph (v). (1)

B) The applicant or customer seeking to obtain Link Up Service on Tribal Lands credits must demonstrate their current participation in one of the following assistance programs. The Applicant or Customer shall complete and sign, under penalty of perjury, an authorization and self-certification form provided by the Company.

1) Food Stamps

2) Temporary Assistance to Needy Families (TANF) 3) Supplemental Security Income (SSI)

4) Medical Assistance

5) Vocational Rehabilitation (including aid to the hearing impaired 6) Oklahoma Sales Tax Relief

7) Federal Public Housing Assistance

8) Low Income Home Energy Assistance Program 9) Bureau of Indian Affairs general assistance (2)

10) Temporary Assistance for Needy Families (TAFN) Tribally-administered block grant programs (3)

11) Head Start Programs (only applicant or customer who satisfy the income qualifying eligibility provision) or

12) National School Lunch Program (only applicant or customer who satisfy the income standard of the program for free meals.

C) The applicant must not be a dependent for Federal Income Tax purposes, unless the applicant is over the age of 60 years of age.

D) The applicant must also certify agreement to notify the Company if the applicant no longer participates in the program or programs described in paragraph B, above, for which the applicant certified their participation in.

E) The service installation charge, as described elsewhere in this tariff, will be discounted by fifty percent (50%) not to exceed $30. An additional 100%

discount will be applied for service installation charges between $60 and $130, including any facilities based charges associated with the extension of lines or construction of facilities needed to initiate service.

F) The discount will not apply to charges for facilities or equipment on the customer side of the demarcation point.

(1) The Company shall have no responsibility for the certification of applicant’s or customers eligibility.

(2) Applicant must have sufficient resources to meet the basic and special needs defined by the Bureau Standard of Assistance. 25 C.F.R. Sec. 20.21.

(3) 42 U.S.C. Sec. 612 and 45 C.F.R. Sec. 286.

27.3 Residential and Business Service Promotions

When in the opinion of the Telephone Company, it becomes feasible to promote the use of residential and business service, service may be connected at a reduced rate or without a nonrecurring charge for a specified promotional period of generally 30 days or less. These promotional offerings will only be made when the Telephone Company determines that one or more of the following conditions are met:

a) That Service Order activity is in a temporary off peak period, or

b) That other residential or business service data indicates a need to further penetrate the residential and business service market.

27.4 Waiver of Nonrecurring Charges for Military Personnel

This waiver applies to Military Reservists, National Guard and Full Time Military Personnel who disconnect and/or those requesting reconnection of their service due to military deployment associated with Operation Enduring Freedom, Operation Iraqi Freedom or Operation Noble Eagle (Homeland Defense) or any yet to be named military campaigns.

All residence nonrecurring charges are to be waived at the time the access line is installed. The nonrecurring charges include the service charge for the access line, and additional access lines as found in Section 27 of the tariff, and any optional feature or features installed at the same time as found in Section 30 of the tariff.

Verification of military service will be required for this waiver. Military Personnel will be required to provide the name and telephone number of their Commanding Officer and date of orders. In order to qualify for this waiver, military personnel must meet one or more of the following qualifications:

a. Personnel who were relocated to other countries, states or cities because of Operation Enduring Freedom, Operation Iraqi Freedom, or Operation Noble Eagle or other military campaigns and have returned to Windstream territory.

b. Personnel who were moved from military bases in other telephone company areas because of Operation Enduring Freedom, Operation Iraqi Freedom, or Operation Noble Eagle, or other military campaigns and have relocated to Windstream territory.

c. Only Personnel who were moved as a direct result of Operation Enduring Freedom, Operation Iraqi Freedom, or Operation Noble Eagle or other military campaigns are eligible for this waiver. Military personnel being relocated as a result of normal military operation are not eligible.

This waiver is not available to personnel whose telephone service was disconnected for nonpayment, or disconnected prior to military service.
