HOT WORK PERMIT

PERMIT APPROVAL

D. Site Superintendent / Project Manager

1. Understands the purpose of the Company’s Safety, Health and Loss Prevention Program.

2. Communicates the Program to all employees on a regular basis.

3. Recommends program changes to the Safety Coordinator on updating safety/health and environmental regulatory standard changes, production procedure changes, etc.

4. Enforces the company’s Safety, Health and Loss Prevention Program through the

company's disciplinary process. This would include positive observations, discipline, etc.

5. Submits accident reports involving personal injury and product/property damage above

$200 to the Safety Coordinator. Documents the causes and submits a written summary report on the accident upon request. Reviews the employee's submitted accident report for clarification or additional comments.

6. Works with employees and the Safety Committee in correcting identified hazards. When a hazard cannot be eliminated immediately or completely, identifies alternate means of minimizing loss until a more permanent repair/fix is established.

7. Responsible to identify accident potentials.

8. Responsible for regular on-site safety training (includes weekly meetings for subcontractors).

9. Periodically reviews work areas for physical hazards, improper work habits, and Standard Operating Procedures that can cause personal injury, property or product damage.

10. Monitors and ensures compliance and effectiveness of New Hire Orientation.

Effectiveness can be measured by observing operations, employee work habits, training records, accidents and equipment abuse.

11. Informs all Subcontractors and Subcontractor employees of job-site safety rules and requirements, emergency and evacuation procedures and enforcement program.

12. Implements Company’s Return-to-Work Program in cooperation with the Claims Coordinator.

13. Identifies accident trends, costs, and common factors with the Safety Coordinator.

Cooperates with the Safety Coordinator when performing an overall analysis on incurred losses.

14. Is responsible for inspection and hazard report review.

a) Reviews the audit reports and findings.

b) Reviews hazards that have been identified by the Safety Coordinator, the OECS, Inc.

Associate, employees and/or subcontractors.

c) Takes corrective action if necessary.

15. Identifies budgetary items needed in the Safety, Health and Loss Prevention Program, including equipment repairs and alterations, and compliance with current safety and health regulations.

16. Ensures that first aid kits/stations are available and well maintained. Ensures through regular training that all employees and subcontractors are informed of procedures for responding to major medical emergencies.

17. Ensures that potable water and shelter is provided for all employees and subcontractors.

18. Recommends annual Safety, Health and Loss Prevention Program objectives to the Safety Coordinator.

19. Places the “OSHA Workplace Poster” in a prominent location for employees to review.

20. Maintains tool box talk documents, training records and reports, etc.

E. Foreman

1. Understands the purpose of the Company’s Safety, Health and Loss Prevention Program and cooperates in the implementation processes.

2. Communicates the Program to all employees on a regular basis.

3. Recommends program changes to the Safety Coordinator on updating safety/health and environmental regulatory standard changes, production procedure changes, etc.

4. Enforces the company’s Safety, Health and Loss Prevention Program through the

company's disciplinary process. This would include positive observations, discipline, etc.

5. Completes and submits accident reports involving personal injury and product/property damage above $200 to the Site Superintendent. Documents the causes and submits a written summary report on the accident upon request. Reviews the employee's submitted accident report for clarification or additional comments.

6. Volunteers to participate in the Company’s Safety Committee. Cooperates with the Foreman, Site Superintendent, Safety Coordinator and others to identify pertinent meeting topics.

7. Works with employees and the Safety Committee in correcting identified hazards. When a hazard cannot be eliminated immediately or completely, identifies alternate means of minimizing loss until a more permanent repair/fix is established.

8. Addresses hazards identified by other workers or management. Responds with corrective actions for hazards identified and reports them to the Site Superintendent.

9. Responsible to identify accident potentials.

10. Continuously reviews work areas for physical hazards, improper work habits, and Standard Operating Procedures that can cause personal injury, property or product damage.

11. Monitors and ensures compliance and effectiveness of New Hire Orientation.

12. Holds and documents tool box talks with his/her workers and provides documentation to Site Superintendent.

F. Employee

1. Understands the Safety, Health and Loss Prevention Program. Cooperates with the implementation of the Company’s Safety, Health and Loss Prevention Program.

2. Volunteers to participate in the Company’s Safety Committee. Cooperates with the Foreman, Site Superintendent, Safety Coordinator and others to identify pertinent meeting topics.

3. Immediately reports all accidents and occupational diseases to the Site Superintendent.

Cooperates in filling out a First Report of Injury and any additional documentation necessary.

4. Cooperates with the Company’s Return-to-Work Program.

5. Accident Investigation

a) For all personal injuries with lost time in excess of one hour, complete an Employee Accident Investigation Report. For all serious accidents, contact the immediate Supervisor.

b) Cooperate on investigation of all property damage in excess of $200.

c) Documents all near misses that may have likely resulted in a lost time, work related injury or product/property damage.

6. Walk through their designated work area and observe for situations/conditions that contribute to a work related injury, product or property damage. Notify the immediate Supervisor if a situation is observed that may cause a loss.

7. Suggests to the immediate Supervisor needs for equipment maintenance repair and required materials.

8. Suggests annual Loss Prevention objectives to the immediate Supervisor or the Safety Coordinator.

9. Follows Standard Operating Procedures. Makes suggestions to the immediate Supervisor for procedural change to lower risk factors causing injury and property loss.

10. Follow the company’s general safety rules.

11. Performs job tasks as trained.

12. Exercises care to prevent injury to other employees and to one's self. When in doubt, contact immediate Supervisor for proper instructions.

13. Give whole-hearted support to the Safety, Health and Loss Prevention Program and attend required employee Skill/Safety trainings as provided by Company.
