UMMARYMy passion to inspire and support individual behavior change through nutrition education and counseling is deeply rooted in my past and present experiences with my family, friends, and patients. At eight years old my nephew was diagnosed with Type 2 diabetes. This premature diagnosis was a direct result of prolonged poor nutrition and physical inactivity. Throughout college, I witness friends (particularly African- American) struggle with weight management. For several years after college, I worked with research and community projects with African-American populations and I witnessed firsthand the environmental and individual barriers that deterred African-American adults and children from making healthy lifestyle choices.
During my dietetic internship experiences at Cabarrus Health Alliance (Kannapolis, NC), Novant Hospital (Huntersville, NC), and WakeMed Hospital (Raleigh, NC), I noticed that the vast majority of the patients I encountered struggling with weight and obesity-related chronic disease self
management were African-American. After brief discussions with these patients, I realized most of the people I encountered had some basic knowledge of healthy lifestyle behaviors. They knew that eating more fruits and vegetables, choosing leaner meats, and exercising at least 30 minutes a day would help them achieve a healthy weight. However, they struggled to understand how healthy lifestyle behaviors impact overall health and chronic disease management.
Rethink Wellness Nutrition and Fitness Consultants, LLC was born during the middle of my dietetic internship at WakeMed. During this internship experiences, I frequently heard comments from African-American patients and community members who stated that they would like to make healthy lifestyle changes but they were reluctant to talk with a dietitian. Their reluctance stemmed from their beliefs that dietitians would make them give up foods they love, encourage them to eat foods they hate, and suggest physical activities that are not conducive to their usual routine. There was a general concern expressed that diet and physical activity recommendations were often not reflective of an understanding of African-American culture and lifestyle. Hearing these concerns inspired me to address these misconceptions by creating healthy recipes that incorporate familiar foods, using motivation interviewing to help individuals acknowledge and overcome barriers of physical inactivity, and providing evidence-based nutrition information through relevant messages that address common struggles that African-Americans face.
Business Structure

Private individual and small-group personal training packages will be offered for clients interested in improving their health status by incorporating physical activity into their routine. Patrick Mutukwa is currently training individuals and small groups. New clients will be obtained through word of mouth and online advertising through the Rethink Wellness website and Blog, as well as Facebook. Community events (speaking engagements and classes) are the final revenue stream of our business plan. Speaking engagements serve a dual purpose. First, they provide direct revenue because this service will be available to organizations seeking nutrition experts for wellness events. Secondly, speaking engagements will serve as a marketing tool for nutrition counseling andpersonal training services, as well as, group healthy cooking and fitness classes. While our speaking engagements and classes cater to large groups, our strategies for obtaining speaking engagements include seeking endorsements and referrals from individual leaders of corporate, community, and faith-based organizations.

Expertise
Kamaria Mason is a nutrition expert with over five years of public health and community nutrition experience. Kamaria is passionate about helping individuals and families achieve their health and nutrition goals through practical and creative nutrition programs. Her approach to individual and family nutrition counseling uses motivational interviewing to support weight management and mitigate adverse health outcomes associated with obesity-related chronic disease. Prior to
returning to school to earn a Masters of Public Health degree and Registered Dietitian credential at the University of North Carolina (UNC) in Chapel Hill, Kamaria earned a Bachelor of Arts in
Women’s Studies at the University of Michigan. She continued her pursuit of knowledge of human health and wellness by focusing on biomedical sciences earning a Master’s of Science from Barry University in Florida. Kamaria has experience working with child, adolescent, and adult populations implementing community nutrition programs focused on promoting healthy eating behaviors, establishing healthy eating environments, and supporting opportunities for physical activity. Kamaria’s work with corporate and community wellness campaigns, includes Lunch and Learn presentations, nutrition counseling for weight and chronic disease management, as well as, healthy eating and cooking classes.

passion for chemistry and understanding how pharmaceutical treatments work in congruence with diet and exercise to improve outcomes of obesity-related conditions inspired him to pursue a doctoral degree in pharmacy from the University of North Carolina in Chapel Hill. Patrick has a unique talent for creating custom exercise routines that embraces the current physical barriers of his clients and unlocks their untapped potential, allowing them reach their fitness goals. His transformative programs are centrally focused on increasing core strength to achieve total body results. Patrick’s incorporation of cardiovascular exercise, as well as, breathing and stretching techniques make his exercise programs dynamic and unforgettable.M

NALYSISTrends in the Marketplace
The economic burden of obesity and related chronic disease is steadily rising. Inadequate physical activity and poor dietary behaviors are the primary culprits of rising rates of obesity and related chronic diseases.14 Healthcare organizations are turning to prevention and disease management as the means of lowering staggering healthcare costs. As a result, hospitals and private physician practices in North Carolina are looking to increase access to health care providers trained to manage weight and obesity related chronic disease. Outpatient nutrition services are becoming increasingly popular because registered dietitians are trained to assess weight and counsel individuals on setting healthy lifestyle goals that will result in improved disease self-management.
Community Profile
In 2014, the obesity rate of North Carolinian adults was 29.7 percent, ranking North Carolina 24th highest obesity rate among 51 states. 13 According to the 2013 Wake County Community Needs and Health Assessment, nearly 2 out of 3 adults of Wake County adults were either overweight or obese in 2010.14 High mortality rates from heart disease was another significant problem in Wake County. While the mortality rates from heart disease have dramatically decreased since 2004, heart disease was the second leading cause of death for residents in Wake County in 2011. 14 Additionally, in 2011 the mortality rate from heart disease was 32 percent higher among African-Americans in Wake County compared to non-Hispanic Whites. 14 Diabetes was the seventh leading cause of death for residents in Wake County in 2011. 14 The mortality rate from diabetes was higher among Americans in Wake County compared to other races. The diabetes mortality rate among African-American males was 3 times higher than white males and the rate of mortality was even more significant for African-American women at 3.5 times higher than white females in Wake County in 2011. 14 However, in 2015 Wake County was ranked #2 as the second healthiest county of 100 counties in North Carolina 2 indicating that residents of Wake County are engaging in some healthy behaviors and are potentially pursuing opportunities for weight and chronic disease
management.11

more likely to eat five fruits and vegetables compared to households earning with an income less than $50,000. 14 According to the 2013 Wake County Community Health Assessment, over 82 percent of Wake County adults reported participating in physical activities in the past month. 14 The top three resident reported locations for exercise include neighborhoods, public parks/trails, and at home. 14 Similarly, to the statistics for fruit and vegetable consumption, residents in Wake County with a household income over $50,000 and some college experience are more likely to participate in exercise. 14 In 2012, 47 percent of the city’s residents earned a bachelor’s degree or higher and the median household income in Raleigh, NC was $69, 937, greater than the state average suggesting a greater potential for spending on wellness. 8,11Target Customers
In 2014, 44 percent of African-Americans had household income greater than $50,000. 6 The
number of African-Americans earning college degrees continues to rise and the 2013 Nielson report revealed that younger college-educated African-American professionals are migrating south from large northern and eastern urban cities. 9 African-American buying power also continues to rise and forecasted to be $1.3 trillion by 2017. 9 Marketers are currently focusing their efforts on twelve major metropolitan areas in the United States with a high-concentration of African-Americans that represent a significant share of the market population. 9 Raleigh, North Carolina ranks #10 of the twelve metropolitan areas in 2013. 9
Health is a topic of interest among American consumers. In 2014, 92 percent of African-Americans in 2014 believed that living a healthy lifestyle was important. 6 When asked about their top health goals, 54 percent of African-American consumers said losing weight was their top priority, 36 percent indicated maintaining weight was their main goal, and 28 percent said that overcoming an existing disease or condition was their primary health goal. 6 African-American consumers also have an interest in purchasing health food items. In 2014, number of African-American adults 18-54 with a household income greater than $50,000 shopping at health food stores (Whole Foods, Trader Joe’s or Fresh Market) increased by 40 percent. 6 Also in 2014, 55 percent of African-Americans 18-54 years old with a household income of $50,000 or greater indicated that they would purchase or support a product by a person of color or a minority-owned business. 6 Since African-Americans earning, $50,000 or greater are already purchasing health food items and are interested in supporting products sold from minority-owned businesses, this
population may potentially purchase health services, such as nutrition counseling for weight and chronic disease management and personal training, from a minority-owned and operated business.

constraints on patient visits. Such constraints limit their ability to implement behavior modification techniques, assess barriers, and establish SMART goals with patients. Registered Dietitians are the leaders in delivering preventative services in both clinical and community settings. 12Dietitians are
trained to assess weight status and conduct motivational interviews that promote healthy lifestyle behaviors. In addition, dietitians provide strategies for self-monitoring eating behaviors, reducing fat intake, increasing physical activity, making healthy restaurant choices, and overcomingemotional eating. 15
Corporate and community organizations are establishing wellness policies regarding healthy eating and increased opportunities for physical activity. Many facilities have healthy cafeteria and vending options. Other organizations are supporting their employees and members by hosting health fairs, Lunch n Learn presentations, and Weight loss challenges. Leaders of corporate and community organizations are continually looking for nutrition and fitness experts to provide everyday solutions for healthy eating and physical activity through interactive workshops such as grocery store tours, cooking classes, and group fitness classes.
Assessment of the Competition
There are several private practice dietitians in Raleigh and the greater Raleigh area. The vast majority of these individual and group practices offer individual and group nutrition counseling. Many private nutrition practices also offer services and programs for Worksite Wellness
campaigns. Healthy Diets, Inc. is a large group practice that offers metabolic testing in addition to nutrition counseling and speaking engagements for corporate clients. Custom Fit Nutrition and Wellness, LLC. is small group practice that offers on-site nutrition counseling to corporations in Raleigh. Two other Raleigh based companies, Nutrition Partners and Triangle Nutrition Therapy offers services associated with nutrition and fitness. Nutrition Partners, partners with CORE Fitness gym and offers packaged deals that include visits with a personal trainer. Triangle Nutrition
Therapy has a Registered Dietitian on staff that is also a personal trainer. They provide one-on-one in-person and web-based personal training packages. In addition to private practice groups, there are a plethora of gyms and wellness centers in Raleigh offering nutrition counseling and personal training. However, these facilities require individuals to pay membership fees.

Through motivational interviewing with a Registered Dietitian, clients will reveal barriers to adopting healthy eating behaviors and with guidance; they will establish realistic health goals. Private in-person sessions are facilitated on-site at physician offices, corporations, community and faith-based organizations. Also, sessions will be conducted at the client’s home or other preferred location, as well as, via teleconference and videoconference.
Initial Consultation (60-minutes):
Includes a medical and nutrition history review, health behavior assessment, discussion of food preferences and intolerances, nutrition education, and a personalized nutrition care plan. A custom meal plan, handouts, and other materials will be provided based on the client’s individual needs and goals.
$120 for a 60-minute session/$130 for a 90-minute session
Follow-up Consultations (45 minutes/30 minutes):
Includes a discussion of progress, reassessment of nutrition goals, continued guidance and
education for nutrition and health behavior goals, as well as, an updated custom nutrition care plan. An updated meal plan and other materials will be provided based on the client’s individual needs and goals.

Adopting healthy eating behaviors and creating a healthy eating environment can be strongly influenced by family members. The family wellness package comprises of nutrition counseling sessions that consider family dynamics, as well as, each family members individual nutritional needs. The package includes a 90-minute initial nutrition and health behavior assessment with nutrition education, a family meal plan, a grocery store tour, and two 45-minute follow-up sessions. $380 for the package

TRATEGY Nutrition CounselingPricing for nutrition-related services is determined using data from the 2013 Compensation and Benefits Survey of the Dietetics Profession (charts located in Appendix A). 10 An average of the hourly rates at the 50th percentile of three sections (hourly rate by education and years in the field; hourly rate by work setting: private practice; hourly rate by employment location) is used to calculate the hourly rate of $26.14. The total price includes billable hours for nutrition counseling, as well as, administrative costs such as preparation, communications, and charting. Details located in Appendix B.
Personal Training
Pricing for personal training is based on Patrick’s current rate of $50 per hour private personal training and $35 per hour for small-group training plus travel fees. Discounted rates are available for the 6-Week (buy 5, get 1 free) and 12-Week Private (buy 10, get 2 free) Fitness packages. Details located in Appendix C.
Speaking Engagements and Classes
Pricing for speaking engagements is based on the experience of the speaker, educational
background, and competitors pricing. 16 Both Patrick and I have speaking experience. Patrick’s has experience speaking at large conferences and services at faith-based organizations. I have

a part of a pilot program hosted at a faith-based organization to increase exposure of Rethink Wellness. Pricing for speaking engagements and classes does not include travel.S

TRATEGYOutreach to Health Care Professionals, Corporations, and Community Organizations
A large portion of our clientele for nutrition services will come from health care provider referrals. Our goal is to work contractually with at least 10 physicians (internists, pediatricians,
endocrinologists, and gynecologists) and dentists in to provide onsite nutrition counseling (weekly or monthly) for their patients and employees. 3 In order to achieve this goal, we will send letters of introduction. The letters of introduction will discuss how our nutrition services add value to their practices 3 by improving health outcomes of their patients struggling obesity-related chronic diseases. Also we will schedule office visits with receptionists and have short in-person meeting with physicians and dentists. In addition to visiting physician offices, we will also schedule in-person meetings with local corporate, community, and faith-based organizations. At our corporate visits, we will discuss how our services can help with chronic disease management, which
potentially increases employee productivity and reduces absenteeism. During our community and faith-based organization meetings, we will discuss how our interactive workshops and classes provide wellness opportunities for their members.
Printed Materials
In addition to letters of introduction, we will leave a small Rethink Wellness gift bag for the health providers that we met with in-person. The gift bag will include a hand-written thank you card, a pound of coffee, a referral pad, as well as, business cards and brochures to give their patients.
Online Marketing
The Rethink Wellness Website and Blog will be a critical component of your online marketing strategy. The mobile friendly website will include our contact information, short biographies (Patrick and myself), information about our services, Blog posts (recipes and fitness tips), and videos (cooking and exercise demonstrations). The website will also include like social medial links (Facebook and You Tube).We will also use the Academy of Nutrition and Dietetics (AND) national referral service that is free to AND members.
