Please confirm in incidences where a water service replacement is within a

sodded Blvd. at street line however the existing water box is found in the existing

sidewalk, will item no 68 apply to this?

Please confirm in incidences where a water service replacement is within a hard

surface (ex. asphalt driveway) and the existing water box is found in the existing

sidewalk, will a quantity of 2 be paid for the two different operations? (ex.

Restoration of asphalt driveway apron and restoration of min 3 sidewalk bays).

Line Item 68 on the Pricing Forms for both service areas has been changed and Line

Item no. 31A found in service area A pricing form has a quantity of 0. Is this

correct?

Line Item 31A on the Pricing Forms for both service areas has been changed. Please

see Section II – Revisions, Items 3 and 4 of this Addendum No. 5

Q3.

Pre-Construction Condition Survey states:

“Measurement will be paid by each unit for construction condition surveys.

Payment shall be made upon receipt of each construction condition survey report

and shall be pro-rated based on the following:

- 70% at the completion of pre-construction report and

- 30% prorated against the pre-construction report, per

post-construction report at the completion of the post-post-construction project, if required.

Based on this description, this implies that when a quantity of 1(one) is paid for

this item, 70% of the item will be paid for pre-condition survey and the remaining

30% of the item will be paid for condition survey, however only if a

post-condition survey is required.

In addition, it appears that the quantity in the pricing form for this item is double

than what is required.

A3:

Site Specific Special Provisions SP23 has been revised.

Revisions, Item 2 of this Addendum No. 5.

Special Provision

January 2019

Pre-construction Condition Survey

The Work performed under this RFP item shall be the completion of a site pre-construction

condition survey of all properties adjacent to the proposed Work. The sites requiring condition

surveys shall be provide to the Contractor at the start of each program year.

The pre-construction condition survey shall be completed by an independent consulting engineer

with documented experience, as a third party, in interior and exterior condition surveys of

structures near construction operations.

The pre-construction condition survey shall be completed at least one week prior to the start of

each street location of the Work, and shall be conducted on all businesses and residences

immediately located within, adjacent to, or within 30 metres of the Work area and for selected

business or residences located outside of the Work area.

A Construction Notice on City of Toronto letterhead shall be prepared by the Contract

Administrator advising of the pre-construction condition survey. These notices shall be hand

delivered by the Contractor to all affected owners and occupants.

The pre-construction condition survey consists of the following activities:

1.

Survey data shall be recorded in a written and by photographic means, as deemed

necessary for proper record;

2.

The type and date of construction shall be recorded;

3.

Differential settlements and visible cracks in walls, floors and ceilings shall be identified

and described room by room. All other structural or cosmetic damages shall be identified

and recorded;

4.

All driveways, walks and stairs shall be inspected and any existing damage shall be

recorded; and

5.

Completion of a report indicating properties inspected, refusals of entry and an evaluation

of potential hazards.

Following delivery of survey notices and preliminary attempts to contact property owners, the

Contractor shall provide the Contract Administrator with an intermediate report detailing attempts

to contact property owners and responses.

Upon the completion of the survey, the Contractor shall provide the Contract Administrator with a

log of all dates the properties were visited, which properties were surveyed, and which properties

refused survey.

Upon the completion of the surveys and reports, the Contractor shall provide one hard copy and

one digital copy in PDF format copies of the condition survey report to the Contract Administrator.

When requested from a resident, the City not the independent consulting engineer is to provide a

copy of the condition survey and photos of the surveyed property to the property owner.

Post-construction Condition Survey

The same independent consulting engineer shall complete the pre-construction condition and the

post-construction condition surveys.

The post-construction condition survey shall be completed on all residences and businesses that

have reported issues as a result of the Work. The post-construction condition survey shall inspect

any damage that may have occurred as a result of the Work.

In the event of any claims for damages because of the construction, the independent consulting

engineer shall supply post-construction condition surveys and accompanying reports.

The Contractor shall be responsible for dealing with and settling any claims that may result. The

Contract Administrator shall be kept informed of all claims received and the status of the claims.

The Contractor shall be responsible for all damage due to Construction. The Contractor shall

indemnify the Owner against any claims by abutting property owners for damages sustained due

to any construction activities.

Measurement for Payment

Measurement shall be paid by each unit for construction condition surveys. Payment shall be

made upon receipt of each construction condition survey report.

Where existing curb stops are located within the existing sidewalk the Contractor shall remove

the existing curb stops and abandon the existing water service where directed by the Contract

Administrator.

The locations on the Contract Drawings are approximate and may be adjusted in the field, as

directed by the Contract Administrator.

Sidewalk removal and replacement due to utility and maintenance repairs is covered under the

Roadworks utility cut and maintenance items.

Removal of the existing curb stops within existing sidewalks where directed by the Contract

Administrator including:

 removal of existing curb stop box and rod to a minimum of 0.3 m below finished grade;

 removal of existing concrete sidewalk from expansion joint to expansion joint (three (3) bay

minimum) and disposal offsite.

 permanent restoration of concrete sidewalk as per TS-3.70. A minimum of three (3) bays

from expansion joint to expansion joint of sidewalk are to be removed and replaced;

 permanent restoration of sodded areas and asphalt driveways as per TS 3.70.07.16 and TS

310 adjacent to the replaced sidewalk that have been disturbed by the sidewalk

construction.

 in the event that a driveway apron, the area between the back of curb and edge of sidewalk,

is impacted, this entire area shall be restored.

Measurement of Payment

Payment shall be made per each occurrence of existing concrete sidewalk to be removed and

replaced as a result of an existing curb stop being removed within an existing concrete sidewalk

at the price Quoted by the Contractor. All other temporary and permanent restoration activities
