INTRODUCTION
The CRM Form to Web Internet Lead Capture is a Web application that communicates between Microsoft Dynamics CRM and a hosted asp.net web site in order to allow your customers to contact your organization and submit that information directly in CRM.
The solution is highly customizable, and enables you to design your form right from CRM.
The application can resides either on your local network, or is available as a hosted Windows Azure solution.

RUNNING THE REGISTRATION WIZARD
After the installation has been completed, the first step is to run the Registration. The registration is accessible on either our windows azure server or if you purchased the local copy, will be accessible in on your loval web site in port 9764.

When the application is run for the very first time various configuration settings have to be set in order to be able to connect to between CRM and IIS Web site. The table below contains a list of all the information that will be required, as displayed in the subsections below.
Make sure that you have all the information available before running the application, as the configuration/ registration process will not be able to complete if information is missing, and the application will fail.

The designated prefix of your
organization. This will be used to access the forms that you create.
The license key is required in order to register your CRM Organization within Windows Azure or your own corporate environment. All of the other forms settings will not be enabled until the license key is entered. Enter the license key in the on the top of the form above as shown in the following screenshots:
If you license key is validated, a green arrow will appear next to the license number text, and you will be able to continue and enter the other required information.
COMPANY AND PERSONAL INFORMATION
The company and personal information contain the details of your organization and the contact information of the primary person that is responsible for the product. This is the contact information that we will use to communicate with you regarding new releases and updates to the application as well as technical support incidents.
All fields on this form are required with the exception of the Mobile Phone number.
CRM SETTINGS
The CRM Server Service Url and login credential are required in order to communicate between the web application and your CRM Server and query your list and contact entities.
The following screenshots show the required information if connecting to CRM On-Premise or CRM Online. For CRM On-Premise, the Url is comprised of the Server Name, Port, Organization name and the Organization WCF Service file: XRMServices/2011/Organization.svc, as shown in the image below.
The domain login has to be a valid login in the format of DOMAIN\USER, and the Password and Confirm Password fields must match.
The Web Service Url will be your CRM Online Organization Url, and the Username will be your Windows Live Id.
FORMS URL
The Forms Url specifies the location or the url that your users will use to access the application. The prefix is an 8 character alphanumeric string, that will be used to access the forms the your create.
The screenshot below shows an example of what is required to be entered in the Forms Url Prefix field:
Once the forms Url has been entered, you are ready to save your settings. Note that the Url is unique, and will be verified upon saving of the application. If another user has already selected this Url, you will be required to re-enter the Url.
For the Url entered above, the forms that you create will be accessible via the following Url: http://apps.briteglobal.com/webform/abgflkdf/1/MyFirstForm
