Ability to Thrive offers two programs:

Wellness and Fitness

Activity and Learning Center

Our mission is to promote health and wellness for our members through education, exercise and preventative measures to ensure a

healthi-er lifestyle. This is accomplished in a safe harmonious, positive, empowhealthi-ering and fun environment. We believe that instead of phealthi-erceiving

fitness as a “work out”, we have designed our program to take a friendlier and nurturing approach by activities in a playful way resulting in

pleasure and laughter. The motto of Ability to Thrive is

“Work, Play, Live Fully and Enjoy Life!”

We believe that both a healthy body and mind are essential to healthy living. We incorporate an educational component that includes

clas-ses on nutrition, yoga, stress reduction, positive thinking, meditation, proper dental care and developing positive lifestyles. We provide

both in-house instructions as well as inviting guest speakers from time to time.

The program is held at World Gym located at 9481 Princeton Glendale Road, Fairfield Bowling Lanes located at 5181 Dixie Hwy and the

YMCA located at 6645 Morris Road.

The mission of Caregivers for Independence is to provide optimal services for the individuals we

support with respect to their individual needs, goals and personalities. We choose to focus on

individuals “abilities” while respecting their “disability” and encourage as well as promote

independence for the best quality of life possible. Caregivers for Independence is dedicated to

provid-ing quality home care and day habilitation while givprovid-ing support to the individual to reach their own

personal goals for independence. We believe in the golden rule, “We treat others the way we would

like to be treated.” We understand the needs of the people we support and their families. We take

every consideration to make the individuals and involved support team feel like they are part of our

family at Caregivers for Independence. We strive to enhance the lives of individuals, increasing the

level of their independence and inclusion in community activities through residential and

Offer members the opportunity to socialize, enjoy peer support, and receive health and social services in a fun,

stimulat-ing and supportive, safe environment that promotes better physical and mental health and celebrates independence.

Recreational Activities

Daily exercise, Games, Crafts, True Movie Theater Experience, Gardening Station, Men’s Workshop, Casino slot

Ma-chines, Wii Exercise Station, Billiards, and more. Educational Programs, Computer Training and assistance Guest

speakers and Entertainers. Day trips and outings as arranged by personnel of The Charleston Club.

Nutritious Meals

Diets are tailored to individual dietary needs and overseen by a Registered Dietician. Next Day Menu

Selec-tions ,Nutritious breakfasts, hot lunches, afternoon snacks and evening meals as required.

We provide after-school care in the Lakota and Mason area school districts for children with developmental disabilities,

providing respite for their families. The Charleston Club staff provide assistance with homework, recreation activities

and transportation. Support is provided for young adults transitioning into adult services after graduating from high

school.

- Each home consists of one to three individuals living together learning skills

- This program provides supports to people so they may live in the communities

Our mission is to assist developmentally disabled

indi-viduals gain the experience and tools necessary for as

in-dependent and self-directed a lifestyle as possible, and to

join hands in support with their families, through

dedi-cated care, professional support and a multitude of

ser-vices.

Community Options employees are subject to

back-ground checks and drug screening prior to completing

orientation. In addition, staff are certified in CPR, First

Aid, Crisis Prevention Intervention, are trained to

pro-vide delegated nursing tasks if needed, and are able to

provide transportation when appropriate.

Mission Statement

Our core mission is to support individuals with developmental disabilities to successfully live, work and learn in the community and to attain the highest possible level of independence.

Company Philosophy

We believe in a person centered system of care, where we value each individual as a person, created in the image and likeness of God. We, in collaboration with the coun-ty board of DD, and individuals’ families, are committed to supporting individuals to meet their personal goals. We also believe that all human beings have the right to be treated with dignity and respect regardless of their disabilities, race, religion or family background.

We make it a point to ensure that staff in key roles has a wealth of experience working with people with developmental disabilities, and apply true ethical values in their professional lives.

Our current C.E.O has eight years of experience working as a direct care staff, and six of these years were spent in managerial positions. He was also awarded the Butler County 2012 Independent Provider of the Year.

Our current quality assurance personnel has a wealth of experience as a direct support staff and currently teaches a Sunday school class at Fairfield Church of Christ, for people with developmental disabilities.

Our current financial manager has a wealth of experience working for the past eight years as cashier at a renowned department store. She is honest, hardworking and dedicated.

Universal Transportation Services (UTS) is a locally owned LLC in Fairfield Ohio. With over seventy years in the transportation industry, UTS has been

operating non-medical emergency transportation services for a variety of customers including boards of development disability, school systems,

el-derly services, and governmental agencies throughout Ohio for many years. It is the goal of UTS to be the first choice in public and paratransit

needs. We have experience in all transportation system types, including adults and students with special needs, on-demand service for NET, PRS and

UTS has specialized in transportation of special needs children and adults throughout our history. Today, we remain committed to serving the

acquires a wide range of knowledge and skills to service children and adults challenged by physical and mental disabilities.

The American Disabilities Act required all transit authorities to provide paratransit services for people unable to use regular public transit. But UTS

knows that transit is much more than just a specialized transport. For our passengers, transit means accessibility and independence. Our trained

per-sonnel provide quality service with compassion and sensitivity, respecting the dignity of our riders.

While we work hard on customer satisfaction, our primary goal is safety. We are committed to an exemplary level of safety throughout our

organiza-tion. Our attention to detail is second to none! Our entire fleet is inspected daily. We feel the best policy is an intensive preventive maintenance

ef-fort to avoid problems before they develop decreasing the likelihood of break downs and accidents. We want to assure our customers of a safe and
