Here in Bisley, Surrey, we provide residential placements and specialist support in a homely setting for people with learning disabilities, including behaviour that challenges.

We create tailored support programmes designed to meet each person’s goals, helping them to become more independent and involved in the local community, while developing their daily living skills.

Every individual, along with any significant others in their circle of support, is encouraged to take part in developing their support plan.

Getting around

Our spacious, detached home is set in a quiet residential area along a private driveway leading off Guildford Road in the large village of Bisley.

There are excellent public transport links close by, including a bus stop at the end of the driveway. Brookwood railway station is a mile and a half away. Individuals also have access to two specially adapted vehicles, for even greater convenience.

On our doorstep

Nearby Knaphill has lots of shops catering for day-to-day needs while the bigger stores, cafes and leisure facilities of Camberley, Woking and Guildford are easily accessible via public transport.

We’re also close to various local colleges and libraries that offer a wide range of educational opportunities. Local businesses are a good source of work experience or employment.

Spacious, safe and homely

330 Guildford Road has its own large and enclosed gardens which provide a quiet space for individuals who prefer a bit of peace. Our large lawn is a great spot for ball games, and our generous patio area offers an inviting space for barbeques or sitting in the sun.

The house itself has a large open plan kitchen. It’s the real heart of the home, and weekly menu planning ensures that everyone can look forward to their favourite meals. On the ground floor there’s a big sitting room with a large TV and DVD player.

Individuals are encouraged to personalise their bedrooms with their own belongings and to their own preferences.

The house has five bedrooms over two floors.

Each is bright and airy, has lots of storage, and we can easily organise for special equipment if necessary, including profiling beds, specialist chairs and hoists.

Person-centred support

We understand how important it is to treat each of our residents as an individual and to get to know, understand and respect their experiences, interests and dreams. We take time getting to know each person and their family. By doing so we can create a support plan that’s uniquely and sensitively tailored to them and supports them to lead the life they want.

Focused on independence

Our approach is based on a nationally recognised care and support planning model that emphasises the importance of promoting independence. Everything we do is grounded in relevant and up to date research and reinforced by quality training programmes delivered to all our people.

Promoting positive risk taking

Our home manager and team always conduct a comprehensive risk assessment before supporting an individual with any new activity, investigating the pros and cons and talking to the resident and their circle of support to understand what an acceptable risk might be.

The residents love their regular visits to the cinema and bowling alley, often followed by a meal out.

Our support workers are empowered to encourage them to live their lives to the full, providing the necessary support every step of the way.

Enjoying leisure time

There’s lots to do in the local area, and our team supports each individual to get the most from their leisure time as part of their ongoing support programme.

Individuals often benefit from a regular activity programme, and we’re more than happy to adapt to individual or group needs and wishes.

Every activity as part of an individual’s dynamic weekly programme is chosen with them to meet their own personal goals.

Here is an example of one individual’s chosen programme for a typical week – but nothing is forced or set in stone and every programme is as flexible as it is unique.

Accessing

work and education

Supporting individuals to get involved in work and study opportunities is central to our work at 330 Guildford Road. Our person-centred approach enables us to identify and develop opportunities, with paid work being the ultimate goal.

Over the years we’ve developed strong

relationships with local employers and colleges.

As a result, the links are in place to enable the people we support to attend life skills centres, work place and day centres, as well as local colleges and education centres, so they can develop the vital skills they’ll need to move into

paid employment.

We can support with

accessing and attending education and work placements, escorting and accompanying to college, gaining the skills to find paid employment and

building the confidence for independent travel.

Keeping healthy

Each individual is registered with a GP and supported to access regular health checks at local surgeries. Pictorial health image banks enable individuals to be involved in making decisions about their health.

Keeping active is a big part of everyday life at Guildford Road. The Sussex Road Recreation Park is close by and cycling, swimming and horse riding are all easily accessible in the local community.

We also offer access to a range of specialist services within the home and local area including chiropody, music therapy, optical support and aromatherapy.

Anna attends a local cycling class each week.

By exercising as part of a group and with the support of her support workers, her confidence has increased dramatically and she now feels an important part of her local community.

Developing daily skills

Promoting responsibility

We empower every individual we support to take pride in their home here at 330 Guildford Road.

Support to build up and maintain the skills required for daily living is always on hand. Where appropriate and with the correct level of support, individuals are encouraged to help around the house with domestic tasks, cooking, cleaning and spending time in the garden, such as watering the plants.

A specialist team

Highly skilled, well trained and caring people are our greatest asset. They’re vital to providing and maintaining our excellent service. Our people are trained in specialist techniques such as de-escalation and managing violence and aggression and receive regular training updates to ensure any interventions always follow the least restrictive principles.

This means that we’re able to support individuals with a range of neurodevelopmental, mental health and physical conditions, including:

Comprehensive transition planning

Change makes everyone anxious so that’s why we involve each resident and their circle of support in all decisions, before, during and after their stay with us.

Trying us out

We want people to feel comfortable about living at 330 Guildford Road, so we offer prospective residents the chance to come along and try our facilities before they commit to joining us. This might be for a few hours to begin with and even a weekend stay if they want to fully experience what we have to offer.

Getting ready to move in

Before an individual joins us, we’ll conduct a comprehensive assessment with them and their circle of support (if appropriate). This helps us to identify the individual’s capabilities and develop any necessary behaviour support strategies.

Case managers and support workers from previous placements also take part in these assessments if it’s appropriate.

This assessment helps us to create a

development programme for the new resident, and this is regularly evaluated to check it’s still relevant and effective. Twelve weeks after each individual joins us we produce a full report about their development and discuss it with the them and their support circle.

We’re very careful when we introduce new people into the house. We want everyone to get along, and good friendships to form, so it’s important that we take all personalities into account.

The best outcomes come from involving everyone in their own care and support.

Over time Living Ambitions has established an effective communication strategy that ensures that listening always translates into positive action for the people using our services.

Our Listen to Me groups, living together meetings and other facilitated forums are part of this strategy.

Anyone we support – including residents of Guildford Road – can nominate themselves to join a group, and help to:

Contribute to the service planning process Move strategies forward in their home and area

Feed information into and out of the strategic groups and co-create action plans

Funding

our service

Our current fee structure is based on a full assessment of an individual’s needs in consultation with their care manager. We aim to reduce these fees over time as an individual progresses, gaining a greater degree of independence or needing a reduced level of support.

Our ideal outcome is to help an individual into supported living. Residency is therefore often sold around shorter stays (typically up to 18 months) with person-centred support plans based around three key phases.

Phase 1

Individual assessment, creating a personalised programme of support and meaningful routine.

Phase 2

Embedding the routine and developing the individual’s programme of support to work towards identified outcomes.

Phase 3

Fine tuning the daily living skills and confidence required to move towards supported or even independent living.
