This Addendum forms a part of the Contract Documents and modifies the original bidding documents as issued on January 26, 2022, as noted below. Acknowledge receipt of this Addendum in the space provided on the Bid Form. Failure to do so may subject Bidder to disqualification.
The following changes or clarifications shall be made part of the Bid Documents and shall be taken into consideration when submitting bids.
GENERAL:
1. Bid Form Pricing Sheet (Rate Sheet)
a. Updated A112 Science – Diversified Cabinet to include the modification requirements, see attached Bid Form Pricing Sheet.
b. Updated A132 – Principals Office to A131 – Principals Office, see attached Bid Form Pricing Sheet.
c. Updated CLASSRMS – National Public Seating quantity from 1 to 4 for Part #6700 stool with back, see attached Bid Form Pricing Sheet
d. Updated CLASSRMS – Spectrum Furniture quantity from 1 to 4 for Part # SLIM Lectern adjustable height SLIM Lectern, see attached Bid Form Pricing Sheet.
e. Updated MULTI PURPOSE RM – National Public Seating quantity from 1 to 2 for Part #Dolly, Dolly, hanging, see attached Bid Form Pricing Sheet.

a. Updated A107 – History Classroom from tables to Koi tables, see attached Ridgeview HS Proposed Furniture Plan, Delta 1, dated 02/14/2022.

23. Indiana Canvas Series Wire Management Channel is an approved equal to National undersurface wire manager as long as Indiana Warranty Match for Lifetime is utilized.
24. Indiana Canvas Series Acrylic Modesty Panel is an approved equal to National Resin Modesty Panel as long as Indiana Warranty Match for Lifetime is utilized.
25. Indiana Canvas Series Desk Top, Table Top, and End Panels is an approved equal to National Epic Series Desk System as long as Indiana Warranty Match for Lifetime is utilized and HPL is used for all laminates.
26. Indiana Canvas Series Metal X Base is an approved equal to National footings X base as long as Indiana Warranty Match for Lifetime is Utilized.
QUESTIONS FROM BIDDERS:
27. Can you please advise, do we need to bid on all items to be considered?
a. Yes, you need to bid on all items to be considered a responsive bid.
28. Is the “rate sheet” the same as the Bid Form pricing sheet?
b. Yes, the rate sheet and Bid Form pricing sheet are the same thing.
29. Is the lowest responsible bidder by line item or in total?
c. The lowest responsible bidder is by total for all items.
30. Once a substitution request has been submitted, what is the turnaround time for approval or rejection?
d. All substitution requests will be reviewed and if approved included in this Addendum.
31. Please confirm which switch on the electric height adjustable bases (Workrite Model No. 70-FDLX54- 72-F30) are to be provided. The standard, programmable, or Bluetooth switch.
e. Please provide standard switch on adjustable base.
32. The drawings of the building show the History Classroom with tables. Please confirm if this room is to receive tables, or the Koi (or comparable) student desks.
f. Please provide Koi desks (or approved substitution), see attached revised Ridgeview HS Proposed Furniture Plan, delta 1, dated 02/14/2022.
33. How early can product start being delivered to the building?
g. Anticipated construction completion is the end of May 2022. Lowest responsible bidder to coordinate with UBC (contractor) and Studio W on start of furniture installation.
34. What is the MOD in the part number that signifies a unit is altered for A112 – Science Diversified 1 4101K-RS-MOD Demo Cabinet?

35. Regarding the Sico tables in the Multi-Purpose Room, do you want the optional accent guard that goes under each seat?
i. Please provide optional accent guard under each seat.
36. The hanging chair dolly from NPS only holds 84 chairs. You are requesting 120 chairs. Would you like a second dolly? Please confirm.
j. Yes, provide two dollies.
