Ascension

Before & After School Care

“Ascension is called to be a family of faith so reflecting the life

of Christ, that we draw all people to Him.”

Parent / Provider Contract

This contract is between ______________________________________________, parent(s) of _______________________________________________, and Church of the Ascension, owner of Ascension Before & After School Care, also known as the Provider. This contract is effective as of _____________________.

By signing the contract both parties agree to the following terms:

Hours of Operation

Ascension Before & After School Care follows the Carroll County School Calendar and is open all days that Carroll County schools are in session for children. The operating hours are:

Monday – Friday 6:30 am – 9:00 am Monday - Friday 4:15 pm – 6:00 pm

On those days that Carroll County schools have a two hour and forty-five minute early dismissal, our operating hours will be:

Monday - Friday 6:30 am – 9:00 am Monday - Friday 1:15 pm – 6:00 pm

In the event that schools open late because of weather, Ascension Before & After School Care will open late also. Example – If school opens 1 hour late the child care will open at 7:30 am.

In the event that school closes early because of weather, Ascension Before & After School Care will open to pick-up the children from the bus stop. Parents will be required to pick up the children within 1 hour of the school closing. Example – If school closes and the children are dismissed at 1:00 pm, parents are required to pick-up their child(ren) by 2:00 pm.

 Ascension Before & After School Care will be closed during the week of the Mistletoe Mart in

November. For the 2016/2017 school year, we will be closed during the week of November 7-11, 2016. (School is closed on November 7 and 8, 2016).

Absences

If you plan to keep your child home due to illness, doctor’s appointment or any other reason, you must notify us no later than 7:00 am. If you will pick your child up from school early, and he or she will not attend the after school portion of the program, you must notify us no later than 9:00 a.m. that morning.

Change of Clothing

Parents are responsible for maintaining a spare set of clothing in their child’s cubbies. Children experience growth spirts, so please check every few months to make sure the spare set matches your child’s current size. The spare set should include pants, a shirt and underwear.

Discipline

Our discipline policy is as follows:

1. Disruptive Behavior and Failure to Follow Instructions

If a child in our care is not following instructions and/or is disrupting the activity of other children, the child will be placed in time out. This will count as a first infraction. If the behavior continues, a conference will be arranged with the parent(s).

2. Hitting and Other Physical Behavior

Hitting, biting, kicking, throwing objects or other physically aggressive behavior directed at another child or child care staff member will not be tolerated. An investigation will be conducted to determine the nature of the behavior.

a. First offense – The child will be placed in time out. The parents of all the children that were involved will receive written notification.

b. Second offense – The child will be placed in time out and the parent will be called to pick up the child within one hour. The parents of all children involved will receive written notice. c. Third offense – The child will be placed in time out, the parent will be called to pick up the child within one hour, and the child will be suspended from the program for a period of up to one week. The parents of all the children that were involved will receive written notification.

d. Fourth offense - The child will be placed in time out, the parent will be called to pick up the child within one hour, and the child contract will be terminated. The parents of all the children that were involved will receive written notification.

3. Sexually Offensive Behavior/Abuse

The showing of private body part, touching another child’s private area, inappropriate sexual talk, or other sexually offensive behavior will not be tolerated. An investigation will be conducted to determine the nature of the behavior.

a. First offense – The child will be removed from the setting and the parent will be called to pick up the child within one hour. The parents of the child(ren) that were involved in the incident will receive written notice.

b. Second offense - The child will be removed from the setting, the parent will be called to pick up the child within one hour, and appropriate disciplinary actions will be determined as warranted for the offense.

c. Third offense - The child will be removed from the setting, the parent will be called to pick up the child within one hour, and the child care contract will be terminated.

4. Possessing a Weapon

As per Carroll County Schools’ “Zero Tolerance Policy”, possession of any weapon, look alike, play or otherwise, will not be tolerated. If the child is in possession of a gun, BB gun, switch blade, hunting knife, or other item that could pose the threat of harm to anyone, the parents will be notified and appropriate action will be taken. Ascension Before & After School Care will take all necessary precautions to protect the children in our care.

a. First offense – The child will be removed from the setting, if appropriate, the parent will be called to pick up the child within one hour, and the child will be suspended from the program for one week.

b. Second offense or if the police have to be called - The child will be removed from the setting, if appropriate, the parent will be called to pick up the child within one hour, and the child care contract will be terminated.

Parents should check their child(ren)’s backpacks daily. Even a butter knife and a squirt gun are considered weapons under Carroll County Schools’ “Zero Tolerance Policy”.

Emergency Contact Information

The State of Maryland Regulations require that all children must have a completed Emergency Card on file with the child care, and that it is updated annual before the beginning of each school year. This must include at least one person, who is not the parent/guardian, who is authorized to pick up the child in an emergency.

Illness

If your child has any of the following illnesses or conditions, the child may not attend Before & After School Care.

 A temperature above 100 degrees  Vomiting (2 or more times in 24 hours)  Diarrhea (3 or more water stools in 24 hours)  A severe rash or nits

 Eye Infection (indicated by pink/red eyes, itching and a feeling of something in their eye)  Sore throat (hard to swallow and glands feel swollen)

 Any Communicable-Disease (chicken pox, virus, etc.)

We understand that illnesses can come on suddenly. If your child develops any of these symptoms while in our care, you will be notified and must pick up your child within one hour of notification. Depending on the illness, you may be required to obtain a doctor’s note before your child returns to the program.

Immunizations and Annual Physical Exam

The Immunization Certificate and the Health Inventory forms must be completed by the parent(s)/guardian and a physician and returned to Ascension Before & After School Care prior to your child’s first day of enrollment. The forms are included in the Parent Packet. Additional forms can be obtained from the program Director or the Church office.

Meals

Ascension Before & After School Care will provide a snack before and after school. We are not responsible for providing the children in our care with breakfast or lunch. If your child has any food allergies, allergies

to other materials (ex. latex) or special dietary needs, please let us know. Weekly menus are posted on

Medication

In order for us to administer any medication, the Medication Administration Authorization form must be completed by the parent(s)/guardian and a physician and returned to Ascension Before & After School Care. Any medication must be in the original bottle or container. Prescription medication must include the original prescription label and instructions.

Ascension Before & After School Care will not administer any medications, creams, or sunscreen without written consent from the child’s parents. Parents must hand all medications, instruction and consent forms directly to the Director. Please do not leave medication on the Director’s desk. The form is included in the Parent Packet. Additional forms can be obtained from the program Director or the Church office.

Tuition

Ascension Before & After School Care’s mission is to provide affordable child care to lower-middle income families. The families that fall within the financial guidelines, outlined below, are given “first priority.” After August 15 of each year, spaces will be filled on a first come first serve basis. To be considered for a “first priority” opening, parent(s) /guardians must provide the two most recent pay stubs for each working adult in the household.

 There is an enrollment deposit of $36 per child that will be applied to the last week of care.  Tuition is due on the morning of the first day of the school week for each upcoming week.

(Example: If Carroll County Schools are open Monday – Friday that week, a full week’s tuition is due on Monday. If school is closed Monday and Tuesday, three days’ tuition is due on Wednesday.) Failure to pay the tuition on time may result in termination.

 Tuition may be paid in cash or by personal check. A $36 fee will be assessed for returned checks. In the event that two or more checks are returned during the school year, you will be required to make all future payments in cash.

 You are responsible for your child(ren)’s tuition even if the child(ren) will be on vacation or away for other reasons. If the child(ren) will be away for a week or less, the tuition for the week the children were away is due upon the return of the child(ren). If the child(ren) will be gone for more than one week, the tuition must be paid in advance in order to hold your child(ren)’s slot.

 A late fee of $5.00 will be assessed, per child, for every 15 minutes you are late picking up your child in the evening. Excessive lateness, defined as more than two times per month or five times per school year, may result in termination of the contract.

Termination of Care

The parent(s)/guardian and provider will make every effort to make the program work for each child, and to communicate and work together if there are concerns. Reasons for termination of care include, but are not limited to:

 Availability of resources to meet the child’s needs and the parent’s willingness to access them (ex. access to Individual Education Plan)

 Ability of provider/program to meet the child’s needs  The impact of the child’s needs/behavior on the program

 Willingness of the parent(s) to work with the provider and the ability of the provider to work with the parents.

 Custody issues/conflicts that are detrimental to the child’s participation in the program  Failure of parent(s) to fulfill their contractual agreement

7  Late or nonpayment of fees

 Repetitive late pick-up

For termination of the contract, two weeks’ notice must be provided by the parent(s)/guardian or the provider, unless otherwise stated in the contract. Termination of the contract, without the appropriate notice, may result in the loss of your enrollment deposit.

 If the provider chooses not to enforce any portion of the contract, it does not give up the

provider’s right to enforce any other portion of the contract.

 The contract can be revised at any time by the provider if necessary.
