“My winning quote” Contest
2014 Contest
The contest is held and organized by The Personal Insurance Company and The Personal General Insurance Inc. (collectively referred to hereinafter as “The Personal”). It runs from January 1 to December 31, 2014, closing at 11:59 p.m. Eastern Standard Time.
Exclusivity
The contest is open exclusively to employees and members of organizations that have an agreement with The Personal.
Prizes
There is a total of $50,000 (CDN) in prizes to be won:
♦ Grand prize: one (1) 2015 Volkswagen New Beetle car and some accessories selected by the winner with a maximum total value of $30,000 (CDN), including taxes.
♦ Second prize:
◊ one (1) travel voucher valued at $10,000 (CDN) from a travel agency chosen by The Personal;
or
◊ one (1) 2014 Yamaha Kodiak 450 EPS all terrain vehicle (ATV) valued at approximately $10,000 (CDN), including taxes;
or
◊ one (1) cheque of $10,000 (CDN).
♦ Secondary instant prizes: one of the twenty-five (25) 16 GB Apple iPad mini* tablet device with Wi-Fi connectivity and Retina display valued at approximately $419 (CDN) each plus taxes, with a total value of $10,475.
Eligibility and details of the draws for the instant prizes
There is a total of $10,475 (CDN) in secondary instant prizes, namely twenty-five (25) 16 GB Apple iPad mini tablet devices with Wi-Fi connectivity and Retina display valued approximately at $419 (CDN) each.
1. By filling out the online form at thepersonal.com/mywinningquote to submit their current insurance expiry dates or to request a home or auto insurance quote from The Personal, between January 1 and December 31, 2014, entrants have the chance to win one of the twenty-five (25) iPad mini worth approximately $419 (CDN) each. A person cannot win more than one instant prize. One registration per person per email address. No entry form or purchase required.
2. To be declared the winner of an instant prize, an entrant must first see the message Congratulations! You've won an iPad mini!appear onscreen after completing the online form.
3. The Personal will then confirm the entrant’s eligibility. To be declared an instant prize winner, an entrant must, on the date the aforementioned instant message appears onscreen:
◊ be a Canadian resident;
◊ have attained the age of majority in their province or territory of residence.
◊ and be an employee or member of an organization that has an agreement with The Personal and that is participating in the national contest;
4. The Personal will contact the entrant who received the message Congratulations! You've won an iPad mini! by telephone within seven (7) business days to confirm whether he or she meets the eligibility requirements to be declared a winner. He or she will have to correctly answer a timed mathematical skill-testing question asked over the telephone. If the person does not meet the eligibility requirements, he or she will be declared a non-winner and will receive an email explaining the reasons. The instant prize will be awarded to an alternate winner selected randomly who will have completed the online form and meeting the eligibility requirements. 5. The iPad mini will be sent to the winner by mail from four (4) to six (6) weeks after the date he or she is
declared an instant prize winner.
6. The Personal disclaims any and all responsibility in the event of loss or theft of the iPad mini.
7. The winners acknowledge that the only warranty that applies to the prize is the normal manufacturer’s warranty.
8. An instant prize winner remains eligible to win the grand prize and the second prize.
9. The odds of winning depend on the number of people who have completed the online form at thepersonal.com/mywinningquote on the contest closing date.
10. The prizes are not transferable, either to another person or to the successors or heirs of each winner.
11. In the event that the organizers are unable to award a prize as described in the contest rules, they reserve the right to award a similar prize with an equivalent value or, at their sole discretion, to award the value of the prize described in these contest rules in cash.
Eligibility for the grand prize valued at approximately 30 000 $ and for the second prize valued at approximately $10,000
To be eligible for the grand prize and the second prize, an entrant must:
♦ reside in Canada;
♦ have attained the age of majority in their province or territory of residence on the date of the draw; ♦ on the date of the draw, be an employee or member of an organization that has an agreement with The
Personal and that is participating in the national contest; ♦ meet the following eligibility criteria:
◊ have a home, auto1, or business (in Quebec only) insurance policy in force with The Personal on December 31, 2014;
or
◊ have obtained a home or auto insurance quote from The Personal over the telephone at 1-888-476-8737 or online at thepersonal.com between January 1 and December 31, 2014;
or
◊ for Quebec residents only, have obtained a business insurance quote from The Personal over the telephone at 1-888-476-8737 between January 1 and December 31, 2014;
or
◊ have submitted the expiry date of their home, auto or business (in Quebec only) insurance policy by filling out the form available online at thepersonal.com between January 1 and December 31, 2014.
No entry form or purchase required. One chance of winning the grand prize or the second prize per insurance product.
The Personal, its employees, representatives or agents or those of any other entity that is part of Desjardins General Insurance Group Inc. or one of its subsidiaries, as well as anyone domiciled with said employees, representatives or agents, are not eligible to participate in the contest.

Details of the grand prize and the second prize draws
1. The winners of the grand prize and the second prize shall be chosen by two (2) electronic draws on January 16, 2015 at the head office of The Personal, which is located at 6300 boulevard de la Rive-Sud, Lévis, Quebec, G6V 6P9, at 4 p.m. Eastern Standard Time from among all eligible entries.
2. The chances of winning are based on the number of expiry dates submitted, quotes requested and customers in the database of The Personal on the contest closing date.
3. To claim his or her prize, each winner must meet the eligibility conditions described herein. In addition, he or she must:
a) be reached by telephone in the ten (10) business days following the draw;
b) correctly answer a timed mathematical skill-testing question asked over the telephone; c) accept the prize as awarded;
d) consent to the use of his or her name, place of residence, photograph and voice in any contest-related advertising, without compensation;
e) sign the prize confirmation form and a declaration and release of liability form if requested to do so by The Personal and return them within ten (10) days of receipt.
Failing this, the organizers will select another winner. This subsequent draw will proceed in the same manner as described herein, with the exception of the date, which cannot be the same.
4. A contest entrant is defined as someone whose name appears on an eligible insurance policy, insurance quote request form or renewal date reminder form. If two people jointly hold an eligible insurance policy or jointly requested a quote, it is the person who is an employee or member of an organization that has an agreement with The Personal who must answer a mathematical skill-testing question, and the prize shall be awarded in that person’s name only. If both people are employees or members of an organization that has an agreement with The Personal, either may answer the mathematical skill-testing question, and the prize shall be awarded in both of their names.
5. If the insurance policy, insurance quote or renewal reminder is issued in the name of a corporate body, the mathematical skill-testing question will be submitted to the person whose name appears on the insurance policy, quote request or renewal reminder as the representative of that corporate body, or to any other designated representative thereof, and the prize will be awarded in the name of the corporate body indicated on the insurance policy, quote request or renewal reminder.
6. Within ten (10) business days following the draw, each winner will receive a letter confirming that he or she is a winner as well as a Declaration and Release of Liability Form by registered mail. Each winner will also receive a prize confirmation form to indicate his or her choice to either get the Volkswagen New Beetle car or a cheque for $30,000 (CDN) in the case of the grand prize, and, in the case of the second prize, a travel voucher, an all-terrain vehicle or a cheque for $ 10 000 (CDN) in returning the completed forms, and, within the period specified in the letter. Once the choice will be confirmed in writing to the contest organizer, no changes will be accepted.
7. Within ten (10) days of receiving the signed prize confirmation form and the Declaration and Release of Liability Form from the winners, the contest organizers will contact each winner to inform them of the procedure to claim his or her prize.
8. If a winner chooses to receive his or her prize in the form of a cheque, it will be sent within ten (10) days of receipt of the duly signed forms.
9. The prizes are not transferable, either to another person or to the successors or heirs of each winner.
10. In the event that the organizers are unable to award a prize as described in the contest rules, they reserve the right to award a similar prize with an equivalent value or, at their sole discretion, to award the value of the prize described in these contest rules in cash.
Conditions: the vehicles
1. The total price of the vehicles must include sales tax, transport and preparation charges, tire tax and, if applicable: excise tax on air conditioners, additional exterior paint costs, accessory installation charges, any extended protection program for the offered vehicle and all other applicable fees. If the total price of the vehicles is less than $30,000 for the 2015 Volkswagen New Beetle and less than $10,000 for the 2014 Yamaha Kodiac 450 EPS all terrain vehicle (ATV) , no cash reimbursement will be made for the difference. If the total price of the vehicles exceeds the said amounts, the winners will have to pay the difference in full.
2. The contract will be issued in the name of the winner, and The Personal will settle payment.
3. The winner is responsible for all fees not included in the description above, including but not limited to: auto insurance, registration, driver’s licence and all other fees associated with claiming, registering and using the prize vehicle. The winner is responsible for regular maintenance and repair fees that are not covered by the manufacturer’s warranty and travel expenses to the selected dealership to claim the vehicle.
4. If the vehicle chosen by a winner is not available for any reason, including but not limited to: production delays caused by work interruptions, a shortage of parts, bad weather or other unexpected events, unavailability of the model year, or any other reason, the winner will have to choose another vehicle of equal or less value, subject to the approval of The Personal.
Conditions: claiming the vehicles (New Beetle car or ATV)
1. When taking possession of the vehicle, the winner must present photo identification to claim the prize, must have a valid driver’s licence and must have proof of appropriate automobile insurance.
2. Warranty: The winner acknowledges that the only warranty applicable to this prize is the normal manufacturer’s warranty and that services related to vehicle delivery are the sole responsibility of the manufacturer and designated dealership. The Release Form will include a declaration to this effect.
3. Responsibility of suppliers: The winner acknowledges that upon receipt of the letter confirming the prize and explaining how to claim it, the delivery of services related to this prize become the complete and exclusive responsibility of the prize supplier. The Release Form will include a declaration to this effect.
4. Grand prize and second prize substitution: The Volkswagen New Beetle car and/or the 2014 Yamaha Kodiak 450 EPS all terrain vehicle (ATV) selected by the winner cannot be exchanged, ceded or refunded and must be accepted as awarded, with no possibility of substitution.
Conditions : travel voucher
1. Upon receipt of the travel voucher redeemable at the travel agency chosen by The Personal, the winner agrees that the obligations related to the travel voucher (including the trip) becomes the sole and exclusive responsibility of the travel agency and The Personal shall not be held liable for any harm the participant may suffer as a result of accepting or using said prize. The Personal assumes no liability of any nature whatsoever in the event that the company could not meet its obligations (eg, bankruptcy of the company).
2. It is each traveler’s responsibility to have appropriate travel insurance at his/her own expense. Under no circumstances will The Personal be held responsible for medical costs or other costs normally covered by such insurance.
3. Arrangements and travel-related reservations must be made by the winner with the travel agency chosen by The Personal and are subject to availability.
4. If the prize is not used in whole, no compensation will be due to the winner or his/her guest or guests.
General conditions
5. 2. By submitting an insurance policy expiry date, an entrant agrees to allow a representative of The Personal to
contact him or her a few days prior to the renewal date of said insurance policy for the purpose of providing a quote.
3. The Personal reserves the right to modify the contest conditions or terminate the contest at any time, without notice. In Quebec only, the approval of the Régie des alcools, des courses et des jeux du Québec is required. 4. The Personal reserves the right to disqualify any entrant or cancel any entry that does not comply with these
rules.
5. All decisions made by The Personal are final and binding, subject, in Quebec only, to any decision of Régie des alcools, des courses et des jeux on any matter under its jurisdiction.
6. For Quebec residents only, disputes regarding the organization or conduct of a promotional contest may be submitted to Régie des alcools, des courses et des jeux du Québec for a ruling. Disputes regarding the
awarding of a prize may be submitted to Régie des alcools, des courses et des jeux, but only for the purpose of helping the parties reach a settlement.
7. The Personal cannot be held liable for lost, misdirected, or late entries, or for any failure, for any reason whatsoever, that may affect the availability or operation of the website or any telecommunications systems and networks for the duration of this contest. Moreover, The Personal cannot be held liable for any damage to entrants’ computers or to components of computer systems used to enter this contest.
8. By entering this contest, participants release The Personal of all liability for any damage that could result from said entry or from the acceptance or use of the prize if a person is declared the winner.
9. Each winner must consent to the use of his name, place of residence, photograph, voice and any statement about the prize in any contest-related advertising, without compensation.
10. Volkswagen Group Canada Inc., Yamaha Motor Canada Ltd. and Apple Inc. are not associated with this contest organized by The Personal, and do not sponsor it in any way.
11. The Personal shall not be held liable for any lost or stolen prize. 12. The contest is subject to all applicable laws.
