Attachment B – the Guide to the Fee Data Collection Spreadsheet

Please read the following information before completing Attachment C – The Fee Data Collection Spreadsheet:

Consent

Upon submission of a completed fee data collection spreadsheet, you have consented to the use of the information contained in the Fee Data Collection spreadsheet in the following way:

The trustee acknowledges that

1. The information contained in the Fee Data Collection Spreadsheet (the information) that has been provided voluntarily to the Australian Prudential Regulation Authority (APRA) will be released to the Australian Securities and Investments Commission (ASIC).

2. The information will be made available on ASIC's FIDO website.

General instructions for completing the data collection spreadsheet:

1. Please enter all products on one spreadsheet, do not create a new spreadsheet for each product.

2. If you are using ASIC Class Order [CO 04/1030] In-use notices for employer-sponsored superannuation, you have two options: • Provide details for each subplan as described below; OR

• Provide identical fee information for each subplan. To do this, enter as a minimum Contribution Fee or Management Cost the figure corresponding to lowest cost product based upon the ‘cost of fund’ minimum. Enter the maximum costs corresponding to the highest cost product based upon the ‘cost of fund’ maximum. This data must be based on the examples of annual fees and costs and associated notes in your PDS as set out in Division 5 of Schedule 10 of the Corporations Regulations (as amended by Corporations Amendment Regulations 2005 (No.1)).

General guide to completions of the Fee Data Collection Spreadsheet

The fee information to be provided in this spreadsheet must consist solely of the fee information as disclosed regarding the example of annual fees and costs in the current Product Disclosure Statement (1 July 2005 onwards), and produced to comply with Division 5 of Schedule 10 of the Corporations Regulations 2001 (as amended by Corporations Amendment Regulations 2005 (No.1)).

SPIN Superannuation Product Identification Number. Please enter "No SPIN" if appropriate. Name of Superannuation Product The name of the superannuation product within a specified superannuation fund as

shown in the Product Disclosure Statement (PDS), e.g. XYZ personal plan (as part of XYZ Mastertrust). For closed funds that do not have a PDS, enter the name of the fund. PDS Date Please enter the date of the PDS that the information is sourced from. For closed funds: if

you do not have a PDS, please leave this field blank. The display format is dd-mmm-yyyy, e.g. 25-Jun-2005.

PDS Link If your product is available on the internet, please provide an URL that will allow a consumer access to the PDS. Please do not provide a link to the PDS itself, but to the web page that contains the link to the PDS - this will provide some context to the consumer and will ensure that the PDS link does not become outdated.

Name of Trustee of Fund Please enter the name of the corporate trustee, or, if the trustee is a group of individuals, please enter “group of individual trustees”. You are not required to record the names of trustee directors or individual trustees.

Trustee's ABN Please enter the ABN of the trustee. If the trustee does not have an ABN, please leave this field blank.

APRA Regulated Superannuation Entity Please enter the fund that the superannuation product belongs to.

Super Entity ABN Please enter the ABN for the APRA Regulated Superannuation Entity shown in the preceding column. If the entity does not have an ABN, please leave this field blank. Type of Superannuation Product In the column "Code", please enter one of the numbers 1 to 6 to denote the category of

product as shown below:

1. Public Offer Superannuation Fund (including public offer industry funds) 2. Public Offer Subplan CO-04/1030 (for subplans using ASIC Class Order [CO

04/1030] In-use notices for employer-sponsored superannuation – see general instructions above)

3. Corporate Superannuation Fund

4. Industry Superannuation Fund (not public offer) 5. Eligible Rollover Fund

(The description of the category will be automatically shown in the following column.) The data you provide here must be consistent with the "PDS In Use" notice.

Fee Calculation Base – balance Please enter $50,000 or other amount as permitted under item 215 in Part 2 of Schedule 10 – This should also be the amount as disclosed in the PDS for the superannuation product. You can only enter numbers into this field.

Fee Calculation Base – contributions Under item 217(2) this can only be $5000 – This field is automatically populated when you enter data into the product name field. This should also be the amount as disclosed in the PDS for the superannuation product.

Contribution Fee - minimum Refer to the $ amount as disclosed in the PDS for the superannuation product. You can only enter numbers into this field. If the minimum and maximum figures are the same, enter the same number in both columns.

Contribution Fee - maximum Refer to the $ amount as disclosed in the PDS for the superannuation product. You can only enter numbers into this field. If the minimum and maximum figures are the same, enter the same number in both columns.

Management Costs - minimum Refer to the $ amount as disclosed in the PDS for the superannuation product. You can only enter numbers into this field. If the minimum and maximum figures are the same, enter the same number in both columns.

Cost of Fund - minimum You cannot enter data here - this field will be automatically calculated as the sums from the preceding fields. This figure should reflect what is shown in the PDS.

Establishment Fee This information should be copied from the notes section beneath the disclosure of the example of annual fees and costs in the product disclosure statement (in accordance with item 211 of Div 5 of Schedule 10). This field can contain a maximum of 250 characters; if you need more space, write: "Refer to Product Disclosure Statement."

Termination Fee This information should be copied from the notes section beneath the disclosure of the example of annual fees and costs in the product disclosure statement (in accordance with item 211 of Div 5 of Schedule 10). This field can contain a maximum of 250 characters; if you need more space, write: "Refer to Product Disclosure Statement."

Withdrawal Fee This information should be copied from the notes section beneath the disclosure of the example of annual fees and costs in the product disclosure statement (in accordance with item 211 of Div 5 of Schedule 10). This field can contain a maximum of 250 characters; if you need more space, write: "Refer to Product Disclosure Statement."
