Life Insurance

Your MGM MIRAGE benefits package includes life insurance at no cost to you. This company-paid coverage is equal to your annual income ($15,000 minimum), and includes Accidental Death and Dismemberment (AD&D) insurance in the same amount. MGM MIRAGE also provides life insurance for your spouse and

dependent children.

If something were to happen, will the company-paid insurance cover the mortgage, car payment, credit card bills and other expenses? MGM MIRAGE offers two types of voluntary life insurance products to help protect you and your family.

Term Life provided by UnumProvident

Term life is basic life insurance. You select the coverage amount, up to $500,000, and the benefit is payable to the beneficiaries you elect. Term life insurance may also be purchased for your spouse and dependent children. You may also purchase AD&D insurance to complement your term life policy. So if the unexpected happens, you and your family are further protected.

Universal Life provided by Genworth

Universal life gives you the flexibility to secure your family’s future, while building an accessible cash account. Unlike term life, the premiums for universal life insurance remain the same, even as you grow older.

Universal life premiums can be higher than term life premiums, because a portion is put into an account that

Disability Insurance

There is a lot depending on your paycheck — your home, car, food and much more. How would you pay for everyday expenses if you were unable to work for an extended period of time?

Disability insurance is income replacement. You can have peace of mind knowing your income is protected when you cannot work due to an injury, illness or pregnancy.

Short-Term Disability provided by UnumProvident

Short-term disability insurance can replace your income for up to 26 weeks. Choose your level of benefit and the elimination period that would best meet your needs:

Benefit Amount: Choose to replace 40% of your income,

up to $500 per week or 60% of your income, up to $1,000 per week.

Elimination Period: The waiting period before you

receive benefits. Choose between 14 days and 30 days.

Long-Term Disability provided by UnumProvident

Long-term disability insurance can replace your income when you are unable to work for an extended period of time. Choose your level of benefit and the duration of the benefit that would best meet your needs:

Benefit Amount: Choose to replace 50% or 60% of your

income, up to a maximum benefit of $10,000 per month.

Benefit Duration: Choose the length of time the benefit

Long-Term Care Insurance

When you need help with daily activities due to injury, chronic illness or advancing age, look to long-term care (LTC) insurance. LTC helps pay the bills that health insurance, disability insurance and Medicare do not, such as home health care, and care at adult day care centers, assisted living facilities, nursing homes or hospices.

Based on Aetna’s claims history, the top five (5) medical conditions requiring LTC are:

1. Alzheimer’s disease

2. Cancer

3. Stroke

4. Parkinson’s disease, and other neurological conditions

5. Arthritis

Long-Term Care provided by Aetna

Long-term care insurance provides daily cash benefits. Choose the level of benefit that suits you, whether it’s $100, $150 or $200 per day. You may also select the length of time the benefit is payable, three (3) or five (5) years. Once you enroll for long-term care insurance, your premiums remain the same, even as you get older.

Personal Insurance

Quality health insurance means security. Personal insurance plans increase your security by paying you cash to help you cover your deductibles, co-payments, coinsurance and other out-of-pocket expenses that your health insurance may not. MGM MIRAGE offers you the choice of three personal insurance plans:

Personal Recovery Plus provided by Aflac

Science has increased survival rates for many serious medical conditions, such as heart attack and stroke. The Personal Recovery Plus plan benefit is paid upon diagnosis. The plan includes a first occurrence benefit, a reoccurrence benefit, hospital confinement benefits, continuing care benefits and much more.

Personal Accident Indemnity Plan provided by Aflac

Accidents are the leading cause of injury-related

emergency room visits. The Personal Accident Indemnity Plan pays for emergency treatment, hospital confinement and much more, if an accident occurs on or off the job.

Personal Cancer Indemnity Plan provided by Aflac

According to the American Cancer Society, men in the United States have slightly less than a 1-in-2 risk of developing cancer during their lifetime; for women, the risk is a little greater than 1-in-3. The Personal Cancer Indemnity Plan benefit includes coverage for hospital confinement, nursing services, radiation and
