Dear Community Partners and Families,

Partnership for Families’ (PFF) Project Corazón, is a First 5 LA initiative

focused on reducing maltreatment and abuse of children ages 5 and under.

Project Corazón’s services include a formal partnership with Human Services

Association (HSA) as a subcontracted agency, as well as the Department of

Children and Family Services and other community based organization in the

Service Planning Area 7 (East Los Angeles and Southeast LA communities).

Bienvenidos

has been the lead agency for Project Corazon Partnerships for

Families (PFF) and the SPA-7 Community Partnership.

This SPA 7 Community Resource Directory has been compiled for your

convenience to help you find the appropriate services as well as to inform you

of the community services available to you and your families.

We will provide you any changes in written addendums so that you can

update this resource directory for your own use.

Community Services provides a variety of supportive services to meet the specific needs of our children and families. With intensive support offered to families in crisis, our highly trained staff provides a comprehensive array of in-home and center-based services designed to strengthen families and to help parents provide a safe and nurturing home for their children.

In addition, we provide assistance with the necessities fundamental to daily living. Be it food vouchers, housing referrals, bus tokens or emergency clothing, we meet the critical needs of the children and families we serve. We empower the community with opportunities for education, growth and self-development through parenting classes, school readiness, support groups, health fairs and cultural events.

Family Preservation

Our Family Preservation programs are available for families who have been referred to us by the Department of Children and Family Services and who are at risk of having their children

removed from the home. Our in-home outreach counselors are able to offer parents and children individualized emergency support, assistance, instruction and encouragement.

Group Sessions on: Anger/Conflict Resolution, Domestic Violence, Self-esteem, Health Education and Stress Management.

Parent Education, Parenting Classes, Drug Education and Co-Dependency. Mental Health Therapy and Relapse Prevention

Individual and group therapies are used in a twelve-step program, and women in the program benefit also from regular medical screenings and on-site childcare.

(323) 268-5442 Serves families with children ages 0-5years; referred by DCFS, with current or prior history of Domestic Violence, Mental Health needs and/or Substance abuse, living in East and South East LA. This is a family strengthening and support program which offers the following:

In- home counseling, case management, therapy and support groups,

Parenting classes for adults and pregnant teens as well as Fatherhood parenting classes. Family Fun events, evening workshops in English and Spanish, respite childcare during

workshops.

S.A.F.E PRORGRAM (Salud, Arte, Familia, e Educación)………. (323) 728-7770 Project SAFE is a bilingual intervention for youth ages 12-17 at risk for substance abuse, HIV, and other problem behaviors. The curriculum, which uses the Storytelling for Empowerment model, aims to decrease alcohol, tobacco and other drug use by building youth resiliency, promoting positive role models and strengthening parent-child relations through story telling and learning.

FOSTER CARE AND ADOPTION PROGRAM

Provides short-term emergency and therapeutic foster care to children and siblings who cannot immediately return home to their birth families. The aim of the program is to work with all related parties toward reunification, when that cannot be achieved, we can provide a permanent adoptive arrangement that will nurture and provide stability for children. We specialize in the care of young children, sibling groups and teenage mothers and their babies.

Focusing on providing preventative medical care through responsive education and early

screening techniques, our Health Services division empowers patients with the skills necessary to maintain a healthy lifestyle and manage illness, both minor and chronic. Our Clinics provide safety-net services aimed at improving the health and well-being of patients by delivering a variety of outpatient health services in an environment that respects culture and gender sensitivities.

The services offered include: STD and HIV/AIDS education and testing, family planning, infant care, ob-gyn services, pre-natal treatment, routine physicals, healthy living education, senior services and general medical care.

Partnerships for Families

Partnerships for Families (PFF), or Project Corazón (“Project Heart”), is a child abuse prevention program that provides case management, in-home and onsite counseling, and parenting services to pregnant women and teens with a history of mental health issues, domestic violence, or substance abuse that live in these cities also qualify for the program and participants that are referred to the program by DCFS.

Parenting Classes

Weekly parenting classes in English and Spanish are offered at HSA and at different locations throughout the southeast area. There is a nominal fee for the 12-week courses, and a certificate of completion is awarded to each parent that successfully completes the course.

Family Preservation

A program is designed to protect children by strengthening and preserving families whose children are at risk of abuse, neglect, and exploitation. The Los Angeles County Department of Children and Family Services refers all cases to HSA. The ultimate goal of the Family

Preservation program is to assure the physical, emotional, social, educational, cultural, and spiritual development of children in a safe and nurturing environment.

Domestic Violence Support Services

Current CalWORKS participants or persons eligible for CalWORKS qualify for HSA’s Domestic Violence Assistance program. Clients and their children can benefit greatly from counseling, parenting classes, and emergency assistance, services all designed specifically for domestic violence victims.

Child Abuse Prevention & Intervention

HSA offers families a comprehensive list of programs to prevent and stop child abuse. All services are offered by caring professionals and include counseling, parenting classes, support groups, and other important programs that address child abuse.

Rider Relief Transportation Program……… (562) 806-0250 The Rider Relief Transportation Program (RRTP) provides fare subsidy coupons and tokens to eligible MTA, Long Beach Transit, Montebello bus lines, Norwalk Transit and Foothill Transit bus riders who purchase daily, weekly, or monthly passes. Eligible participants include adults, K-12 and college/vocational students, senior citizens, and the disabled.
