Introduction

The Citrus County Board of County Commissioners provides a comprehensive compensation package including group insurance benefits. The Employee Benefit Highlights Booklet provides a general summary of these benefit options as a convenient reference. Please refer to the County’s Personnel Policies, applicable Union Contracts and/or Certificates of Coverage for detailed descriptions of all available employee benefit programs and stipulations therein. If you require further explanation or need assistance regarding claims processing, please refer to the customer service phone numbers under each benefit description heading or contact your Constitutional Office using the contact information provided below.

Notices

Children’s Health Insurance Program (CHIP)

This provision provides employees and dependents a special enrollment right in group medical plan coverage, without having to wait for an open enrollment period, for a loss of eligibility or becoming eligible for premium assistance under CHIP or Medicaid if requested within 60 days of gain/loss of eligibility for premium assistance under CHIP or Medicaid.

COBRA Continuation of Medical Coverage Benefits

Under the Consolidated Omnibus Budget Reconciliation Act (COBRA), employees and/or dependents may be able to continue their enrollment in certain health plans such as medical and dental, if such coverage is terminated or changed due to a qualifying event.

Health Insurance Portability & Accountability Act (HIPAA)

HIPAA nondiscrimination provisions state that individuals may not be denied eligibility or continued eligibility to enroll in a group medical plan based on medical factors, or charged more for coverage than similar employees.

Health Insurance Privacy (PHI)

The employer/plan sponsor of a Group Medical Plan must comply with applicable federal and state laws to maintain the privacy of your Protected Health Information (PHI). PHI may not be used or disclosed for purposes other than enrollment, treatment, payment, operations and disclosures required by HIPAA law without a valid authorization.

Medicare D Creditable Coverage

Citrus County’s prescription drug coverage(s) is considered Creditable Coverage under Medicare Part D. If you or your dependents are or will be eligible for Medicare, you may obtain more information by requesting a Medicare D Disclosure of Creditable Coverage Notice.

Woman’s Health and Cancer Rights Act of 1998 (WHCRA)

Citrus County’s medical plan provides medical and surgical benefits for post-mastectomy related services including all stages of reconstruction, surgery to achieve symmetry, prostheses and treatment for complications, including lymphedema.

All notices are posted on the Employee Intranet http://intranet/ under the Human Resources Link, and on the Citrus County Board of County Commissioner website http://www.bocc.citrus.fl.us/ under the Human Resources Department. Paper copies are also available free of charge in the Human Resources Department.

The SBC is only a summary of the plan’s coverage. A copy of the plan document, policy, or

certificate of coverage should be consulted to determine the governing contractual provisions

of the coverage. A copy of the actual group certificate of coverage can be reviewed and

Group Insurance Eligibility

The County’s group insurance plan year is October 1st through September 30th. Employee Eligibility

Benefit eligible employees working a minimum of 30 hours per week are eligible to participate in the County’s insurance plan, coverage will be effective the first of the month following a 30 day waiting period.

Benefit eligible employeesworking a minimum of 20 hours a week will be required to work for 12 consecutive months to become eligible for benefits after the appropriate waiting period.

An Employee who works less than 20 hours a week, will NOT eligible for benefits.

Termination

If you separate employment from the County, insurance will continue through the end of the month in which the separation occurred. COBRA continuation of coverage may be available as applicable by law.

Dependent Eligibility

A dependent is defined as the legal spouse and/or dependent child(ren) of the participant or the spouse. The term “child” includes any of the following:

Disabled Dependents

Coverage for an unmarried dependent child may be continued beyond age 26 if:

1. The dependent is physically or mentally disabled and incapable of self-sustaining employment; AND 2. The dependent is otherwise eligible for coverage under the group medical plan; AND

3. The dependent has been continuously insured; AND 4. Coverage began prior to the age of 19.

Proof of disability will be required upon request. Please contact Human Resources if further clarification is required. Taxable Dependents

IRS guidelines state that an employee may not receive a tax advantage on any portion of premium paid related to non-qualified dependents; therefore, employees covering adult children under their medical insurance plan may continue to have the related coverage premiums payroll deducted on a pre-tax basis through the end of the Calendar Year in which the child reaches age 26. Beginning January 1st of the Calendar Year in which the child reaches age 27 through the end of the Calendar Year in which the child reaches age 30, that portion of medical insurance premium that is attributable to covering the overage dependent (the “OAD value”) will be deducted on a post-tax basis. The remaining employee contribution, (i.e., the amount over and above the OAD value) will continue to be deducted on a pre-tax basis. Please refer to the medical premium pages. You may contact Human Resources for further details if you are covering an adult child who will turn 27 any time during the upcoming Calendar Year or for more information.

• A natural child • A foster child • A child for whom legal guardianship has been awarded to the participant or the participant’s spouse

• A stepchild • A newborn (up to age 18 months) of a covered dependent (Florida) • A legally adopted child

Eligibility requirements for eligible Over-Age Dependents were eliminated for group insurance. Dependents may be covered by the medical plan through the end of the calendar year in which the child turns age 30. For dental, coverage may be continued through the end of the calendar year the child reaches age 25.

Qualifying Events and IRS Code Section 125

IRS Code Section 125

Premiums for medical, dental insurance and/or certain supplemental policies are deducted through a Cafeteria Plan established under Section 125 of the Internal Revenue Code (IRC) and are pre-tax to the extent permitted. Under Section 125, changes to your pre-tax benefits can be made ONLY during the Open Enrollment period unless you or your qualified dependents experience a qualifying event and the request to make a change is made within 30 days of the qualifying event.

Under certain circumstances, you may be allowed to make changes to your benefit elections during the plan year, if the event affects your own, your spouse’s, or your dependent’s coverage eligibility. An “eligible” qualifying event is determined by the Internal Revenue Service (IRS) Code, Section 125. Any requested changes must be consistent with and on account of the qualifying event.

Examples of Qualifying Events • You get married or divorced • Birth of a child

• You gain legal custody or adopt a child • Your spouse and/or other dependent(s) die(s)

• You, your spouse, or dependent(s) terminate or start employment

• An increase or decrease in your work hours causes eligibility or ineligibility • A covered dependent no longer meets eligibility criteria for coverage • A child gains or loses coverage with an ex-spouse

• Change of coverage under an employer’s plan • Gain or loss of Medicare coverage

• Losing eligibility for coverage under a State Medicaid or CHIP (including Florida Kid Care) program (60 day notification period) • Becoming eligible for State premium assistance under Medicaid or CHIP (60 day notification period)

IMPORTANT

If you experience a qualifying event, contact your benefits representative from the Board or your Constitutional Office to make the appropriate changes to your coverage. Your request must take place within 30 days of the qualifying event. Beyond 30 days, requests will be denied and the employee may be responsible both legally and financially for any claim and/or expense incurred as a result of a dependent who continues to be enrolled but no longer meets the County’s eligibility requirements. If approved, changes to add coverage will take place the latter of the date of the qualifying event or the date of the written request for change in coverage is received by Human Resources, except for newborns and marriage, which are effective on the date of the qualifying event. Changes to cancel coverage will take place on the first of the month following the qualifying event (ie. divorce, change to POA time status), except cancellation of coverage following a death which terminates on the subsequent day. You will be required to provide valid documentation supporting the qualifying event such as a marriage certificate, birth certificate, divorce decree, or death certificate.

To search for a participating provider, contact Customer Service or visit www.floridablue.com. Click the “Find a Doctor” tab and under “Select a Plan,” select “BlueOptions” as your plan type from the drop down and then “Continue.” Complete the additional Search Criteria and click “Search now.”

The County provides medical insurance through Florida Blue to benefit eligible employees. The costs per pay period for coverage are listed in the premium tables below. For information about your medical plan please refer to the Summary of Benefits and Coverage (SBC) provided.

Tobacco Use Surcharge

The County will be implementing a “tobacco free” incentive for all employees covered under our medical plan. If an employee is a non-tobacco user, the County will waive a $50.00 per month charge to the insurance premium currently paid. Current tobacco users that have not completed a tobacco cessation course, however, will be responsible for the $50.00 per month charge to the insurance premium currently paid. We will launch the “tobacco free” program on October 1, 2015 and will allow a six month period of time for any current tobacco user to complete tobacco counseling or a tobacco cessation program. Free resources are available through the Health Department or our Employee Wellness Center. The County will assist you in locating a program to fit your needs as necessary. Please also note that on October 1, 2015 Chantix (stop smoking prescription) will be a covered medication under our Florida Blue plan. While the incentive for the Tobacco Free Program will initially be based on completion of an affidavit, random employee nicotine testing will begin six months after implementation of the program to ensure compliance.

The County will continue to offer a self-funded dental plan through Heritage Consultants. The entire premium cost of the Employee Only coverage is paid for by the County.

Plan Benefits

The dental plan is “open access”, meaning it is not limited to a specific network of dental providers. The dental plan allows you to receive services from any dental provider you choose, without having to select

a Primary Dental Provider or obtain a referral to see specialists. The dental plan will reimburse for services based on what is determined to be a “Usual and Reasonable Charge (URC)”. The URC may vary by the type of Dentist or service received. The URC can be defined as the most common charge for a particular dental procedure performed in a specific service area.

Out-of-Network Benefits

The dental plan is not limited to a specific network; therefore there are no out-of-network benefits. You may receive services from any licensed dental provider.

How to Locate a Provider

The dental plan allows you to receive services from any licensed dental provider. Calendar Year Deductible

The dental plan requires a $50 individual and $150 family Calendar Year Deductible (CYD) to be met before most benefits will begin. The deductible is waived for preventive services.

Calendar Year Benefit Maximum

The maximum benefit the dental plan will pay for each covered member is $1,250. Once the Calendar Year Benefit Maximum has been met the member will be responsible for any future charges until the next Calendar Year. All services, including preventive services, accumulate towards your Calendar Year Benefit Maximum.

Coinsurance

The percentage of coinsurance a covered member is responsible for is based upon the plan’s Usual and Reasonable Charge assigned to each particular service. For example, for Regular Services the plan pays 80% coinsurance based on the Usual and Reasonable Charge for that service. Then, the member would be responsible for 20% coinsurance, once the Calendar Year Deductible has been satisfied.

Please Note the Following:

Each covered member may receive up to 2 routine cleanings per Calendar Year under the preventive benefits offered.

Late entrant provisions, waiting periods and age limitations may apply.

Benefit amounts are calculated based on Usual and Reasonable Charges. Please contact Heritage’s customer service for any questions concerning your plan benefits.

Basic Term Life Insurance

The County provides Basic Term Life insurance through Cigna to all benefit eligible employees at no cost to you. The benefit amount for active employees is $20,000. The Basic Term Life insurance benefit will be paid in the event of the insured’s death.

Coverage reduces by 35 percent at age 70 and by 50 percent at age 75. Reductions in coverage would be effective at the beginning of the plan year (October 1) following your 70th or 75th birthday. For example: if you turn 70 on December 12, 2014, your plan would be reduced October 1, 2015.

Accidental Death & Dismemberment Insurance

Also at no cost to you, the County provides Accidental Death & Dismemberment (AD&D) insurance through Cigna to all benefit eligible employees. The AD&D rider pays a benefit in addition to the Basic Term Life benefit when death occurs as a result of an accident. The AD&D benefit amount equals the Basic Term Life benefit amount. Voluntary Employee Life and AD&D Insurance

Employees may purchase additional Life and AD&D insurance on a voluntary basis through Cigna. This coverage may be purchased for yourself, spouse or child(ren) at different benefit levels. The AD&D benefit amount equals the Voluntary Life benefit amount. This additional coverage offers life insurance above and beyond the Basic Term Life and AD&D coverage provided by the County.

• Coverage is available in $10,000 units, starting at $10,000, up to a maximum of six times your salary, not to exceed $500,000.

• $200,000 is guaranteed for new hires; no medical questionnaire required. If you do not enroll when first eligible for Voluntary Life, you must complete a medical questionnaire and are subject to Cigna’s approval. In addition, if you wish to increase your benefit, you will need to complete a medical questionnaire.

Voluntary Spouse Life Insurance

• You may elect coverage for your spouse for a $7,500 Dependent Life benefit amount. All late applications for coverage require medical underwriting approval.

Dependent Child(ren) Life Insurance

• You may elect $2,000 of Dependents Life insurance for your eligible children. Your unmarried child may be covered from birth through age 20, or through the end of the calendar year in which they turn 25 if a full-time student.

• Child(ren) applications are not subject to medical underwriting.

Please Note: The rate for spouse and/or dependent child(ren) coverage is $1.75 per pay period. The rate is the same if you are covering a spouse only, covering eligible dependent children only, or covering a spouse plus eligible dependent children.

The County provides a Long Term Disability (LTD) benefit through Cigna to eligible employees at no cost to you. Eligible employees must be regular, benefit eligible Board and Constitutional Offices’ employees who work at least 30 hours per week. The LTD benefit pays you a percentage of your gross monthly pre-disability earnings if you become disabled due to an illness or non-work related injury. A summary of the plan’s benefits is provided below:

• The LTD benefit pays 60% of your monthly pre-disability earnings, up to a monthly benefit maximum amount of $8,000. • This benefit may be reduced by other income such as Social Security, Workers’ Compensation, retirement benefits, etc. (see

plan summary or contact Cigna Customer Service for more information).

• The benefit begins on the 91st day after the disabling event (known as the elimination period). • The duration of the LTD benefit payable is based on your age at the time the disabling event occurs. • If you return to work on a part-time basis, you may continue to be eligible for partial benefits. • Periodic evaluations will occur at the discretion of Cigna.

• You will receive benefits for the first 36 months if unable to return to your own occupation.

• After 36 months, you must return to any occupation in which you are suitably trained, educated, and capable of performing. • The LTD benefit does not pay a benefit for disabilities resulting from war or act of war, intentionally self-inflicted injury,

Supplemental Insurance

Aflac offers a variety of voluntary supplemental insurance plans that may be purchased separately on a voluntary basis; premiums are paid by payroll deduction but all plans are portable at the same discounted group rate and guaranteed renewable for life. Aflac has provided the following information about its plans but detailed brochures are available from human resources or by contacting your agent. Please contact your agent during open enrollment for an appointment or schedule an appointment on ROSCOE as dates and times are posted.

Accident Insurance

Provides you with cash if you or a covered dependent receive treatment for injuries sustained in a covered accident on or off the job. This policy provides an emergency treatment benefit, specific-sum injuries benefit, initial hospitalization benefit and even an accidental death benefit.

Hospital Advantage

Three new plan options allow you to cover gaps in your health insurance and offer financial protection for any medical reason you are hospitalized. Plans are designed to pay you an initial hospitalization benefit ranging from $500-$1,500 that can be a match to your health insurance deductible with additional options that include benefits for doctor visits, surgery, ambulance, diagnostic, emergency treatment and more.

Cancer Insurance

Three plan options provide you with cash benefits if you or a covered dependent are diagnosed with an internal cancer or skin cancer. This policy gives you an initial diagnosis benefit, hospital confinement benefit, radiation and chemotherapy benefit, surgical/ anesthesia benefit, as well as ambulance, transportation and lodging benefits.

Critical Care Event Protector/Hospital Intensive Care Combination Plan

Aflac pays a first-occurrence benefit as well as hospital confinement and continuing care benefits for: heart attack & coronary artery bypass surgery, stroke, end-stage renal failure, major human organ transplant, major third-degree burns, paralysis and coma. In addition the Plan also provides you with the benefits if you are confined to Intensive Care for Accident or Sickness. Also includes ambulance benefit and major human organ transplant benefit.

Hospital Intensive Care

Aflac provides you with cash benefits if you or a covered dependent are confined to Intensive Care for Accident or Sickness. Also includes ambulance benefit and major human organ transplant benefit.

Whole Life Insurance for Children

Can be purchased by parents or grandparents for children from birth through age 17. Coverage starts at $30K and on the child’s 18th birthday, the face value doubles to $60K. Rates are locked in at the age of enrollment for life and plans build cash value.

Employee Health & Wellness Center

Citrus County’s Board of County Commissioner employees, retirees and dependents are able to use the Employee Health & Wellness Center. Primary care services include acute care; sore throats, ear aches, headaches, cough, sinusitis, strains and sprains. Services also include chronic illness evaluation, treatment and management including, diabetes, high cholesterol and hypertension. The Center also provides many popular medications, health risk assessments, lab work, health coaching & education, and occupational medical services as a no-cost option to the traditional insurance network.

Deferred Compensation is basically a second retirement source for employees. It is tax deferred money deposited into your account. You pay taxes on the money once you withdraw it. The County does not match the amount you deposit or make any deposits into your account on your behalf. It is strictly an employee contributory plan.
