2016-2017 GRANT APPLICATION - READING & LEARNING CENTER / COMPUTER LAB

The Dallas Mavericks Foundation (The Mavs Foundation) is dedicated to building a stronger community by empowering youth, women and families who need us most through financial support, advocacy and service.

In order to receive funding from the Mavs Foundation, organizations must:

 Qualify as a 501(c)3 tax exempt public charity, with 501(c)3 status that has been active for at least two years.

 Operate programs within 150 miles of American Airlines Center in the Dallas/Fort Worth Metroplex

 Provide programming that benefits children, women or families in North Texas

 Use the funding within the designated grant year (Oct. 2016 – Sep. 2017) GUIDELINES/RESTRICTIONS:

Grants will not be made to:

 Individuals

 Churches

 Public, Private or Charter Schools

 National organizations that do not have locally, financially independent chapters Grants will not be made for:

 Salaries

 Medical research

 Travel

 Political campaigns or fundraising events, including the purchase of tables, tickets or advertisements The Mavs Foundation does not commit funds for multi-year grants, endowment campaigns, administrative costs, advertising or fundraising drives, intermediary funding agencies and/or research. If an organization is awarded a grant for two consecutive years, that organization will be ineligible for an award the following year.

TO APPLY FOR A GRANT: Application Form and Completed Application REQUIRED for each Request Type 1. Complete the ONLINE GRANT APPLICATION FORM with executive summary.

2. Submit the COMPLETED APPLICATION (must be in this order): a. Completed application (no more than 6 pages)

b. Attach copy of IRS final determination letter - 501(c)(3) only.

c. Include your wish-list for a new Reading & Learning Center/Computer Lab, along with the actual or tentative project budget.

d. Include the proposed project timeline.

e. Include a roster of your officers and board members.

f. Please send 10 complete sets of your proposal. Incomplete packets and/or handwritten applications will automatically be disqualified. Requests for grants cannot be processed via fax, e-mail or telephone. g. Staple the entire application together (do not use paper or binder clips, folders, etc.).

Please do not include any other attachments. Additional pages will not be considered.

APPLICATION DEADLINE: All applications must be received in our office by 5 p.m. June 30, 2016.
