APPLICATION FOR REGISTRATION AND CATEGORISATION OF ELECTRICAL CONTRACTORS FOR THE YEAR 2009/10 GENERAL NOTE:

INFORMATION SUPPLIED IN THIS APPLICATION MUST, WHERE NECESSARY, BE ACCOMPANIED BY SUPPORTING DOCUMENTS OTHERWISE NO POINTS WILL BE ALLOCATED

1. All contractors should submit their application forms to Procurement Office on or before 6th March 2009 at 12:00 noon.

2. It is important that the contractor answers each and every question fully, with supporting document where required, as the responses shall be carefully examined. 3. The applicants must note that they may be called for an interview on some or all

answers in the application form; and some of the responses may be physically verified.

4. The applicants must note that no individual, partnership or company shall be allowed to register more than one electrical contracting company with Lesotho Electricity Company; otherwise such applications will be disqualified.

5. The application form will not be considered if the Traders License is not attached. 6. Tools like comealong and ratchet are pre-requisite to be in category B, B+, A and

A+.

Attach certified copies of tax clearance certificates including VAT and Income Tax. 5.0 FINANCIAL PERFORMANCE

a) Submit the audited accounts of the business for at least the past 2-years.

b) If audited accounts are not available, please provide a letter from the bank stating to which level they can provide you with a credit.
