FUNDING AND SUSTAINABILITY

This grant will not support full funding for program implementation. There is an expectation that sites will provide some level of community match to fund a portion of the work. Research shows that local investment is essential to successful, sustainable implementation of IY in a community.

1. Please list the funding sources available, or that you will seek, to help support your implementation of IY in 2016-2017. Please note if the funding is pending or already secured. Please also note any additional funding, which is considered a community match, that is allocated to general program implementation, money earmarked for a specific piece of the implementation (i.e. materials), in-kind support of the program, etc.

2. Does your organization have experience finding collaborative funding and resources to sustain programs? Are you currently using, or do you plan to use, any collaborative resources in support of IY?

3. What is your long-term vision for implementation of this program in your community?
