APPLICATION FOR HARDSHIP EXEMPTION
Applicant herein applies for a Hardship Exemption in order to continue to operate as a
member-supported non-profit mutual-benefit collective. The non-profit purpose of this corporation is to
promote, support, and facilitate the cultivation, procurement, refinement, distribution,
dispensation and informed and proper use of medical marijuana by and for its patient/
caregiver-members in accordance with the spirit and statutory provisions of California's Compassionate
Use Act (Proposition 215), the Medical Marijuana Program Act of2003 (Senate Bill420), the
California Health and Safety Code Sections 11362.5,
et seq.
Applicant respectfully reserves the right to amend and augment this hardship exemption
application at any time prior to the hearing hereon and thereafter as may be provided by law.
