ccidents happen, and when they do, the plans of a lifetime can be changed forever.

Unfortunately, there is no way to guarantee that you or your family will never be involved in some form of accident. However, thanks to this Optional Accidental Death and Dismemberment Insurance Plan, financial compensation may be available if you or a member of your family dies or is injured as the result of an accident.

How It Works

Optional Accidental Death and Dismemberment Insurance is available in two forms: a Member Plan and a Family plan. It offers coverage in units of $50,000 up to the maximum principal sum of

$300,000. The protection you receive is in force 24 hours a day, whether you're at work, home or on vacation. What's more, because the Memorial University Alumni Association has endorsed this plan, you can make your payments through no-charge monthly automatic bank deduction.

What could be easier than that?

Who’s Eligible for Coverage?

If you're under the age of 65, you and your spouse are eligible for coverage. Your unmarried, dependent children are also eligible for coverage if you choose the Family Plan. (We consider dependent children to be unmarried children of the member or the member's spouse who are less than 21 years, or up to 25 years if they are attending an accredited educational institution on a full-time basis.)

What Does It Cover?

Accidents resulting in death, loss of limbs or use of limbs, sight, speech or hearing are covered anytime, while at work, home, or on vacation. However, please remember that all coverage terminates at age 70.

Coverage Limitations

Your Optional Accidental Death and Dismemberment Insurance does not provide coverage for death or injury resulting from:

• any intentionally self-inflicted injury

• suicide while sane or insane

• viral or bacterial infections

What Are The Benefits?

If any of the following losses occur within 365 days after the date of the accident, benefits will be paid as follows:

Both hands, both feet or sight of both eyes 100%

One hand and one foot 100%

One hand and sight of one eye 100%

One foot and sight of one eye 100%

Speech and hearing in both ears 100%

One arm or one leg 75%

One hand, one foot or sight of one eye or speech or hearing in both ears 50%

Thumb and index finger or at least four fingers of same hand 25%

All toes of one foot 12.5%

FOR LOSS OF USE OF:

Both legs or both arms or both hands 100%

One leg or one arm 75%

One hand 50%

Paraplegia 200%

Quadriplegia 200%

Hemiplegia 200%

Family Plan Benefits

If you select Family Plan Coverage, the amount of principal sum your dependents receive is determined by applying the following percentages to the amount of principal sum for which you are insured.

• your spouse and there are no children eligible for benefits 60%

• your spouse and there are children eligible for benefits 50%

• your child(ren) and there is no spouse eligible for benefits 15%

• your child(ren) and there is a spouse eligible for benefits 10%

What Does It Cost?

This insurance is available in units of $50,000 up to a maximum of $300,000.

Alumni Association Benefits

For its assistance in administering this plan, your Alumni Association will receive a percentage of the total premium paid.

The above description is intended to highlight the features of the Optional Accidental Death and Dismemberment Insurance plan. The actual plan provisions are detailed in the group contract issued to your association by The Great West Life Assurance Company. The group contract shall be the governing document.
