• Each user has a unique user name.
• Users can have different sets of user rights:
• WHAT THEY CAN SEE: e.g. all incident reports, or only
the ones that they are involved in.

REPORTING FORM
This form is filled in by HSE/Ops personnel when the incident record in
the system is created.
Tabs along the top are filled in by authorised personnel as the incident
progresses through the workflow – i.e. First Aid details, Follow-Up
Actions, Management Comments, Root Causes, Attachments.
Once created, the
incident is assigned
to the next person in
the workflow.

When the next person in the workflow, in this case the First Aider, opens the incident report, some fields are greyed out and cannot be edited.
White fields remain editable.
The First Aider adds some information for the next person in the workflow.
The First Aider then selects the next person in the workflow and clicks ‘Save’.
As before, the next person receives and email notification, and when they log in, they see the incident information on their dashboard.
The incident can be assigned from
person to person until the incident
form has been fully completed.
Personnel in the Incident Reporting workflow can create related Actions at any time.
These actions are linked to the Incident, but follow their own workflow to completion – i.e. can be assigned to different people and closed out.
• Sub-Actions MUST be completed before the Incident Report can be closed.

All Incidents are shown on the Home Screen.
• It shows all Incidents and Actions that the user is involved with (or ALL Incidents and Actions for Management or HSE users).
• Incidents/Actions are presented in tabular format. You can customise what fields from the Incident Report you want to show in this table.
• You can export the data to Excel.
• Filters at the top make it easy to find specific Incident Reports or Actions. • Double-click on an item in the table to open up the full Incident Report.
• You can switch between Incident Reports and Corrective Actions by using the tabs at the bottom of the screen.
• You will notice additional tabs, e.g. for ‘Management of Change’. This is because you can create many different types of data entry forms for many different purposes – not just Incident Management. Other HSE uses might be ‘Audit Plans’, ‘Shared Learnings’, ‘Improvement Opportunities’

Authorised Users can create Corrective Actions via buttons at the bottom of the
Incident Report Screen:
• Sub Actions MUST be completed before the Incident Report can be closed. • Linked Actions can remain open even when the Incident Report is closed.
