High School Concurrent Enrollment Information Packet – Spring 2015

Welcome to Contra Costa College!

We are happy that you have chosen Contra Costa College to help you achieve your future goals. Please use

the attached documents to help guide you through the admissions/registration process.

Steps to Registration for Concurrently Enrolled Students

Step 1 - Apply online

Admissions and Records Office (SSC-115).

Step 2 – Clear your prerequisites

to the Office of Admissions and Records (SSC-115) prior to enrollment. Please allow 2-3

business days for processing. A full list of course descriptions with prerequisites are available in

Step 3 – Register for classes in person

Bring your completed

Concurrent Enrollment Form

and a photo ID to the Admissions and Records

Step 4 – Get your free photo ID

After registering for your class/es, please bring your class schedule (InSite Portal) and a photo ID

(State or Government Issued ID or Valid High School ID) to the Welcome Center in SSC-111 to

obtain your free Contra Costa College photo ID.

Other Information

Future Dream Act Students:

If coded as a non-resident student please complete the California Residency Reclassification Request and submit an official high school transcript. Non-resident tuition fees will be removed. See the Admissions ‘FORMS’ page.

After the course is over:

If requested by your high school, please submit your official transcript request online only after final grades are posted. The first two standard copies ordered online are free. Instructions are enclosed.

Final high school transcript:

Upon graduation from high school, please have your official transcript sent to the Admissions and Records Office at CCC, which should include your graduation date. This will allow us to update your records to reflect your new status as a high school graduate.

Dear Student:

Apply online to expedite the processing of your application. Once submitted, your

application will be completed within 2 hours. The online application can be accessed at

www.contracosta.edu

Select ‘APPLY NOW’, Select ‘New Students’. Start with “Step 1”.

If you have not received an email with your student ID# within 2 hours of applying, contact

the district help desk by email at

your student ID#.

If you have previously attended Contra Costa College, you will need to submit a new

application. Instructions for completing the online application are included in this packet.

Due to privacy laws, CCCApply will not accept online applications for persons under the age

of 13. Paper applications can be obtained at the Admissions and Records Office, SSC-115.

IMPORTANT

It is recommended but not required to provide your Social Security Number when you apply. If you provide your SSN on this application you may find it easier to access your student records when you need them if you forget or misplace your CCC student ID number. If you do not provide your SSN or do not have one, you will always need to use your CCC student ID number to access your student records. This information is emailed to you when you apply online. You will need to come in person to the Admissions & Records office or Enrollment Lab and bring a photo ID to obtain your stu-dent ID number and InSite WebAdvisor User Name if you misplace this information. All OpenCCC institutions are re-quired by law to keep your SSN private and secure and not to reveal it without your permission.

We will only be testing students who are enrolling at CCC. We will not test students who are planning on attending another school and we will not send test scores to other schools.

If you have previously taken an assessment test at CCC and would like to use your scores at another school, you will need to print your Test Summary from InSite/WebAdvisor to send to the school.

Concurrent Enrollment (Special Admit) Expectations and Responsibilities

Special Admit Enrollment

 Title 2, Chapter 5, Article 1, Section 48800 of the California Education Code allows the governing board of any school district to determine which students would benefit from advanced scholastic or vocational work. The intent of this section of the California Education Code is to provide educational enrichment opportunities for a limited number of eligible pupils, rather than reduce current course requirements of elementary and secondary schools.

 Contra Costa College interprets “advanced scholastic” coursework as those courses designated as degree applicable (DA) in the CCC Catalog.

 Students who are 14 years of age or older or those who have completed the eighth grade or higher are eligible for concurrent enrollment as “special admit” students at CCC.

 Contra Costa College reserves the right to exclude or limit enrollment of special admit students into impacted programs and other programs or courses based on health, safety, instructional methodology, faculty constraints, or legal

requirements.

 Students enrolling in PE activity courses are subject to the 10% limit for each section.

 New and returning high school students must submit a college application (online) along with a Concurrent Enrollment (Special Admit) Form during in-personregistration. Continuing high school students need not reapply.

 Concurrent students may enroll in person during open registration. Please submit your application online prior to your enrolment date. Bring your signed concurrent form, and proof of prerequisite completion if enrolling in a course that has a prerequisite. Concurrent (Special Admit) students registering for CCC courses at LMC or DVC before these dates will be dropped from the CCC classes.

Student Responsibilities

 You may register for approved classes only. Registering for non-approved classes will result in a loss of Concurrent (Special Admit) enrollment privileges.

 All coursework taken at CCC become a permanent part of your college academic history. Grades received or excessive drops have implications for future status involving financial assistance, athletic eligibility, academic standing, etc.  Clear prerequisites and arrange for assessment testing, if necessary.

 You are responsible for reviewing and abiding by all academic and student life policies in the Schedule of Classes and the College Catalog.

 You are required to meet all academic deadlines.

 Books and materials must be purchased for all classes. They are not loaned or given out free of charge.

 Concurrent (Special Admit) students are exempt from enrollment fees if enrolled in less than 12 units. However concurrently enrolled students must pay the student representation fee ($1-mandatory) and the Student Activity Fee ($5 - optional). These fees are due each semester. Please see the catalog for more information.

 CCC is an adult academic learning environment and you are expected to behave accordingly.

 By signing the space provided on the front of this form, you agree that you are eligible to enroll as a Concurrent (Special Admit) student and will abide by all CCC policies and procedures. Additionally, you agree that you have reviewed these expectations with your parent or guardian.

 Home schooled students must provide verification of an Affidavit for Home Schooling, filed with the Department of Education, at the time of registration.

Parent or Guardian Information

 Special Admit students are treated like every other college student. All college coursework is governed by the Family Educational Rights and Privacy Act (FERPA) which allows release of academic information, including grades, to the student only – regardless of age. Academic information is not released to parents or third parties without written consent of the student. Upon consent by the student, information is only released to the authorized party in person or in writing. (NOTE: The student’s signature on the front of this form may, or may not, authorize release of academic information. If authorized, it may be release only to the parent/guardian who signed the form.)

 CCC Admissions and Records Office does not keep daily attendance records for students. Parents should not contact college offices or instructors with the expectation of accessing their student’s attendance or other information.  Review your student’s Concurrent (Special Admit) Enrollment Form with him or her. Please be aware that all

coursework at CCC becomes a permanent record on the student’s academic history; grades received or excessive drops have implications for future student status at the college, including financial assistance, athletic eligibility, etc.

 By entering a college environment, your son or daughter will be exposed to a diverse population in education programs designed for an adult learner that may involve sensitive topics that might be considered controversial or offensive in the secondary setting. CCC will not change course content or curriculum to accommodate Concurrent (Special Admit) students.

PREREQUISITE VERFICATION/CHALLENGE

INSTRUCTIONS and FAQs

GENERAL INSTRUCTIONS AND INFORMATION: 1. Complete the front side of this form.

2. Attach supporting documentation. Documentation may take the form of:  official or unofficial transcript

 report card with final grades

 letter from instructor/school on official letterhead stating the course was completed and grade. These documents must include your name and the name of the school. You must attach a copy of your documentation to this form even if you have already submitted official transcripts. All coursework being used to satisfy a prerequisite must be completed with a “C” grade or better.

3. Submit the completed form to the Admissions and Records Office, SSC 115.

Note: Incomplete forms will not be processed. Please ensure all supporting documentation is attached. All follow-up correspondence will be emailed to your InSite email address.

HOW DO I CLEAR A PREREQUISITE REQUIREMENT? Complete the personal information at the top of the reverse side of this form, Part A, and sign it. Submit this form and documentation to the Admissions and Records Office (SSC-115). If you completed the prerequisite coursework at CCC, DVC or LMC prior to summer 1999 you must also submit this form. It is not necessary to attach a transcript.

WHAT IF I TOOK A SIMILAR COURSE OR HAVE GAINED THE PREREQUISITE KNOWLEDGE IN ANOTHER WAY? Complete the personal information at the top of the reverse side of this form, Part B, and sign it. Attach documentation to support the way in which you have satisfied the prerequisite. Submit this form and

documentation to the Admissions and Records Office (SSC-115). Admissions and Records will tentatively enroll you in the course to hold your seat during the 5 business day review period. You will be notified of the outcome via your InSite portal email address. Prerequisites may be challenged on one or more of the grounds listed below:

a) The prerequisite has not been established in accordance with the district’s process for establishing prerequisites;

b) The prerequisite is either unlawfully discriminatory or is being applied in an unlawfully discriminatory manner;

c) The student has the knowledge or ability to succeed in the course or program despite not meeting the prerequisite; or

d) The student will be subject to undue delay in attaining the goal of his or her educational plan because the prerequisite course has not been made reasonably available.

CAN I USE IN-PROGRESS COURSEWORK? In-progress coursework cannot be used unless you are taking the course at CCC, DVC or LMC.

HOW CAN I GET THIS INFORMATION TO THE ADMISSIONS AND RECORDS OFFICE? You may mail, fax (510-412-0769) or hand-deliver this form to Admissions and Records (SSC-115), with your documentation, before your registration appointment time. Please allow at least 3-4 business days for prerequisite verification processing.

HOW WILL I KNOW IF MY PREREQUISITE IS APPROVED? You will be notified by email in 3-4 business days. If you have challenged the prerequisite you will be notified in no more than 5 business days.
