Profile: Crisis Intervention provides support to persons facing crisis situations or mental health emergencies. The agency emphasizes and supports a person’s potential for recovery. Recovery defined as a personal journey rather than a set outcome, and one that may

involve developing hope, a secure base and sense of self, supportive relationships, social inclusions, coping skills and meaning.

Field Placement Responsibilities: My role at Lehigh County Crisis Intervention is to provide support and complete assessments over the phone or via face to face. Assessments include the collection of current symptomologies, presenting problem, treatment and treatment compliance, along with visual observations of personal hygiene, behaviors, interpersonal relationships, environment, etc. Protective factors are also explored during the assessment process. Based on the assessment, recommendations are made in regard to treatment options.

Future Goals: After graduation I plan to obtain my LSW. I would like to work in the healthcare field with the aging population. My goal is to help older adults achieve and maintain their independence in the community.

Adolescent Transitions

Profile: Adolescent Transitions is a partial outpatient program

for adolescents age 12-18 with mild mental health diagnoses. The

typical two week stay consists of individual, group, and family

therapy with the focus of reintegration into the average school

setting.

Field Placement Responsibilities: I will be conducting

individual, group, and family therapy along with conducting

bio-psycho-social assessments on entering client systems

.

Future Goals: After receiving an LSCW and PhD in research

methodologies, I will complete program evaluative work among

social service agencies. I would also like to become a professor

eventually.

Children's Home of Reading

Profile: The Children’s Home of Reading is a sanctuary for children whom have suffered abuse and have mental health issues associated with those traumas. The Children’s Home of Reading has been open since 1884 serving the needs of children and responding to families in crisis in the Berks county area and surrounding communities. Their goal is to treat children and prepare them for success in life.

Field Placement Responsibilities: I intern at one of their community-based programs called the Acute Partial Hospitalization Program or APHP for short. The APHP program provides a therapeutic

environment for children who are unable to function in schools and community environments. My responsibilities include observing, facilitating and cofacilitating groups, assist in helping the children participate in groups and class activities, observe and assist in the intake process, observe meetings and assist in supervising children.

Future Goals: I hope to complete my master’s degree and become a licensed social worker and open a practice. Then, I would like to obtain my clinical license and go back to school to obtain my doctorate degree.

Alexis Conkle

New Vitae

Profile: New Vitae uses holistic care and strength-base perspectives to work with individuals diagnosed with mental disorders, brain trauma, and drug or alcohol addiction. The services provided include residential housing for multiple levels of care ad behavioral health, including

individual, family, and group sessions.

Field Placement Responsibilities: My field placement responsibilities include facilitating a drug and alcohol group session once a week and document the group in case notes. I will also provide individual therapy to a small case load of clients, while continuing to observe other

therapist and social workers individual sessions. For this case load I will act as a team participant for their recovery plan. The responsibilities of this include maintaining treatment plans, conducting monthly team meetings, writing professional case notes and documentation, and continue communication with the residence staff and family of the client. I will also provide assistance to group facilitators in groups focused on topics such as: cooking, dancing, ceramics, professionalism, LGBTQ+, and a group for clients in partial hospitalization.

Future Goals: I plan to develop my skill in group facilitation and

individual sessions. I also expect to learn more theories and models and implement them into the groups I facilitate and co-facilitate.

Profile: CONCERN is a private non-profit human service agency that offers foster care, adoption, behavioral health, residential services and community-based programs to children and their families. CONCERN has multiple locations throughout Pennsylvania. CONCERN's mission statement is, "CONCERN brings hope, offers opportunity and inspires change."

Field Placement Responsibilities: I will be writing child profiles and doing child preparation work. I will be writing child profiles for

children in foster care placements. This involves gathering information on each child and compiling a record of their history including facts about their biological families. This requires interviewing current and previous resource parents, biological parents, caseworkers, and other personnel that have been involved in the child's life. Child profiles help recruit resource families for children in foster care placement. The goal of child preparation work is to help a child remember their past, where they came from, and where they are going. Child Preparation workers construct a Lifebook with the child that displays pictures, letters, achievements and various other memorabilia from the child's life.

Future Goals: My future goals are to graduate with my MSW and continue doing work with children in a school social work setting. I would like to work for a few years and travel overseas.

Family Promise of Carbon County

Profile:

Family Promise of Carbon County is a non-profit

organization that provide temporary shelter, meals, and

fellowship to homeless families in Carbon County and the

surrounding community to help the families achieve and sustain

independence. FPCC provides services to help and empower

families by providing weekly intensive case management,

individual and family therapy, nutrition, finance and parenting

classes.

Field Placement Responsibilities:

My role as an intern allows me

to gain experiences in the micro, mezzo and macro level. I will be

working directly with the Director of the agency to assist with

grant writing, planning and organizing events, and fundraisings. I

will provide direct case management, individual and family

counseling. I will assist with both the finance and parenting

classes and the volunteer training.

Future Goals:

After graduating with my MSW, I would like to

continue working in the health care setting and work toward

earning my LCSW. My goal is to work in Hospice and Gerontology.

Profile: Montgomery County Office of Children and Youth is a public child

welfare government agency responsible for accepting and investigating reports of child maltreatment and is organized into specific divisions. The purpose of the agency is to preserve and strengthen children and families within the practice setting of the prevention of child abuse and neglect.

Field Placement Responsibilities: My field placement responsibilities at

Montgomery County OCY are within the Intake Division. I will be investigating referrals consisting of sexual abuse allegations and

participating in scheduled interviews with law enforcement at the Mission Kids Advocacy Center. I will be responsible for working with families through the entire case process from the time the referral is received through the determination. I will be researching and analyzing the overall process in terms of the coordination and collaboration between the agency, Mission Kids, law enforcement, and families. I will be working with all entities to develop policies and procedures within this specified area in order to establish a definitive process to better support the children and families for which we serve.

Future Goals: My future plans are to graduate with my MSW, take the

licensing exam, retire from OCY, secure a position within a school system utilizing both my MSW and M. Ed degrees, work in private practice at the beach.

Alternatives Partial Hospitalization

Profile:

Located at the Muhlenberg Hospital, this

program serves adults diagnosed with mental health

issues. It aims to assist its clients by helping them

function in everyday life. It is more of a group focused

therapy that uses CBT to teach grounding techniques

and coping skills.

Field Placement Responsibilities:

Ones field placement

responsibilities are to assist with the facilitation of

group sessions, insurance reviews, family sessions, and

discharge planning.

Future Goals:

Ones future goals are to obtain licensure

and possibly even obtain clinical licensure. Ones

greatest goal is to work in a VA medical center.

Allison Hauler

Reading Hospital- Project Connect

Profile:

Project Connect is an outpatient social work program. The

social worker acts as an advocate and links patients to resources.

The social worker identifies obstacles to the patients success and

collaborates with the entire team and outside resources to

support the client. The target population is focused on frequent

fliers of the emergency room and inpatient stays.

Field Placement Responsibilities:

Social work student will

function as a resource coordinator and will help each patient take

on a more active role in their medical, social, and emotional well

being. I will be engaging in the planned change process with

clients through completing assessments and intakes, and

implementing a plan to guide the patient towards his or her goals.

I also attend community events held by other programs to

broaden the spectrum of resources that Project Connect provides

to patients.

Future Goals:

Prior to graduating, I plan on taking the LSW exam.

I would like to have a job working in either the NICU or pediatrics

in a hospital setting. My dream job is to receive a job working at

CHOP.

Communities in Schools is a non- profit agency with

a focus on drop out prevention. The community school

model is used allowing site workers to be located in the

school to provide students and their families, with extra

supports. This takes pressure off the student allowing more

focus to be put on school.

Field Placement Responsibilities:

My responsibilities include

case managing about 10 students, running a lunch bunch

group where students can work on social skills, and

participating in school organizations. I will also be helping to

plan a mental health symposium for parents of students in

the school.

Future Goals:

I plan to graduate from Kutztown University in

May with my masters in social work. After graduation I plan

to use my knowledge to help the younger population from

infancy to high school.

Profile:

The SafeGuards program is designed to treat

sexualized children and adolescents who cannot remain

with or return to their legal family.

Field Placement Responsibilities:

My field placement

responsibilities include co-facilitating SafeGuards

group, Independent living, adult SafeGuards group

sessions with therapists and psychologists. Accurately

and competently composing group and individual notes.

Future Goals:

To obtain my LCSW and obtain a position

in an agency working as a trauma-informed clinician

with adolescents. Long-term, I hope to open up my own

private practice targeting the youth population.

Social Workers and Advocates of Kutztown is an

organization based out of Kutztown University with the mission of

enhancing student awareness of social justice and assisting the

Kutztown community through a commitment to social justice.

Field Placement Responsibilities:

As an intern for SWAK I am

responsible for coordinating and facilitating opportunities for

social work students to discuss issues/concerns related to the

field, and to identify micro, mezzo, and macro practice

opportunities within the Kutztown community that address such

issues. In addition, I will be assisting with the facilitation of

various professional social work conferences throughout the

academic year. I am also tasked with the responsibility of

representing SWAK by attending on-campus and community

organizing events. By attending such events and volunteer

opportunists, I hope to effectively advocate for unmet community

needs identified by Kutztown students as well as the broader

Kutztown community.

Future Goals:

Currently, I am most interested in researching,

writing, and working with clients in the disability services field,

specifically individuals with autism spectrum disorder.

in the military and their families. Veteran's Justice Outreach

Program works with veterans who have criminal charges. The

program works to help Veterans to enter society as a functional

member by helping the veteran meet their individual needs.

Field Placement Responsibilities:

My field placement

responsibilities focus on the veteran population, specifically those

who have been convicted of a crime. I attend treatment team

meetings and collaborate with other disciplines involved with the

veteran's care. I advocate for the veteran's to receive the services and

consequences that will best benefit the individual. Consequences are

supported by research informed practice and practice informed

research. I get to experience different environments in which the

program works their practice.

Future Goals:

My future goals are to attain my LCSW. I plan on

Profile:

Merakey is a provider of mental health services that

strives to create individualized care for clients and their

families.

Field Placement Responsibilities:

As an intern, I am

responsible for assisting with administrative duties of the

outpatient therapy program, as well as observed and

conducting intakes for new patients, collaborating with a

psychiatrist for tele-psychiatry services, and observing and

eventually providing therapy sessions.

Future Goals:

In the future, I hope to obtain my LCSW and

continue to practice in an outpatient setting.

Profile:

The focus of this agency is providing psychiatric

hospitalization for patients in need of acute and extended

acute care. This agency serves adults and patients from

Adams, Lancaster, Lebanon, and York counties.

Field Placement Responsibilities:

My role is this setting

consists of conducting bio-psycho-socials and incorporating

my perspective during daily treatment team meetings.

Encouraging family involvement and providing family

therapy when necessary, are also services provided.

Ensuring each patient has a safe discharge plan by making

appointments, talking with supports, and working with

patients to create a safety plan. I've had the opportunity to

conduct statistics regarding follow-up, utilization of

software, and more.

Future Goals:

Once earning my MSW, I hope to work with

individuals with mental illnesses. I'll be applying for

licensure and would be fortunate to find a position with a

clinical supervisor.

Profile:

Haven House has provided mental health services to

the Lehigh County and Northampton County residents. Their

mission is to improve and elevate the independence of

individuals having mental illness while supporting recovery

and enhancing their quality of life.

Field Placement Responsibilities:

My current responsibilities

are; conduct assessments of new clients, facilitate individual

therapy sessions and complete treatment plans. I primarily

work on the micro level unless I need to facilitate a role and

work on the mezzo level to meet the needs of my clients.

Future Goals:

My future goals after graduating with my MSW

is to work for the Veterans Affairs and help veterans who are

suffering rom PTSD or are having difficulty adjusting back

into civilian life.

Profile:

A school is the host setting for social workers in that field

of practice. A school social worker offers mental health and

support services to students and their families. Networking with

other professionals and referral services on behalf of the students

is required in order to produce beneficial practice outcomes on

micro, mezzo and macro levels of practice.

Field Placement Responsibilities:

My responsibilities as the school

social worker's intern at Northern Lebanon are hardly limited at

all. My field instructor insists that I jump in to every task the

same way I would if I were the actual school social worker. As a

collaborative netowrk, we work on all homeless cases within the

district and practice in respect to state and federal law

requirements. I have been able to work one-on-one with students

seeking guidance, have helped mediate student descripencies, and

intervened in several crisis situations for individuals and families.

Future Goals:

I enjoy my field placement in a school district and I

foresee myself pursuing my first MSW position in the school

setting. However, I am open to trying another field of practice for

my next field placement.

I will be working at a micro

learning more about the services provided through

independence waivers and Office of Long Term Living

Services. I will also be working at a Macro level and learning

evaluations and service delivery. This semester I will focus

on the macro level of the agency and how macro level work

influences the individuals and families receiving services.

Future Goals:

In the future, I would like to work in a hospital

setting assisting families with resources and support.

My role at Promise

Neighborhoods of the Lehigh Valley is a Community

Organizer. I guide the client system through the planned

change process by engaging with community members,

fostering cooperation through rapport building, utilizing

services through interdisciplinary collaboration, utilizing the

role and skill of advocacy, and applying relevant theories to

better the implementation of the social work practice.

Future Goals:

My future goals are to graduate as a LSW,

MSW in 2019 and to earn my Ph.D.

Youth Advocate Program

Profile: The Youth Advocate Program is a nonprofit agency that offers

a variety of different services. The Community Treatment Center

program, is an alternative to placement for juvenile boys ages 13-18.

The program focuses on supporting youth post-adjudication to aide in

compliance and other needs. The program also provides reintegration

support for boys transitioning out of placement.

Field Placement Responsibilities: As a MSW level intern, I am

responsible for running both individual and group sessions. The group

sessions are focused on anger management, social skills, and drug and

alcohol. I conduct assessments with individual clients such as ACE,

Casey Life Skills, CANS, etc. I am responsible for monitoring my

clients' progress throughout their time in the program and for

providing them the support and guidance that they need to be

successfully reintegrated into society.

Future Goals: My future goals are to work in both child welfare and

geriatrics throughout my career as a social worker.

Field Placement Responsibilities: My role as an MSW student Intern at Haven House will include collaborating with the team of trained

professionals. of diverse backgrounds that include MSW’s, LCSW’s, MA’s, psychiatrist, nurses and administrative team. As a professional team member of Haven House, I will share in the responsibilities of adhering to the agency’s policy and procedures demonstrated

throughout my field work. My task will include professionally engaging with a diversified clientele with mental health challenges, while

demonstrating ethical, cultural sensitivity and professional behaviors. I will be conducting thorough assessments of clients needs on a Micro and Mezzo level and co-creating interventions and goals from a family-in-environment and strength based perspective.

Future Goals: My future goal included becoming a LCSW with a private Family Practice, focused on bridging the family together by providing therapy for families impacted by incarceration and or mental health.

Also, Service Access Management provides housing services to the

homeless population, however, these recipients need to be

diagnosed with a mental health diagnosis to qualify for the

assistance. The client works closely with a case manager to create

and set time frames to meet goals.

Field Placement Responsibilities:

My field responsibilities will

include, assisting Spanish speaking families complete the housing

application. I will also be responsible for educating them on time

lines and expectations of the housing program. On many days I

will be responsible for attending Treatment Court, for clients

involved with Service Access Management and providing

information to the judge regarding the client. The Judge will make

decision on whether the client is making enough progress to

continue in Treatment Court.

Future Goals:

Finish my MSW and obtain my LSW. Eventually I

would like to work at a hospital.

Valley Against Sex Trafficking (VAST)

Profile:

The focus of the agency is to reduce and eliminate

commercial sex exploitation, particularly in the Lehigh Vally.

This is done through community awareness campaigns,

victim outreach, survivor support, and campaigning for

policy change on the local, state, and federal levels.

Field Placement Responsibilities:

My responsibilities are

primarily research and development of protocols that can be

used to promote community awareness and information to

provide to volunteers. I am also involved in local policy

campaigning as well as the Speaker's Bureau in which I

deliver informational presentations to interested groups in

the area.

Future Goals:

My future goals include assisting schools to

develop transitional programs for students with autism to

transition from high school to college, then to independent

living.

My Field Responsibilities

include the following doing the initial assessments,

documenting progress notes, making referrals to other

agencies, going to rounds, setting up services for patients,

and making transportation arrangements.

Future Goals:

Once I graduate with my MSW, I plan on

obtaining my license. I would also like to get a job working at

a hospital.

Profile: The Community Action Committee of the Lehigh Valley is a non-profit organization, focused on improving the quality of life for Lehigh Valley residents by building a community in which all people have access to economic opportunity, the ability to pursue that opportunity, and a voice in the decisions that affect their lives.

Field Placement Responsibilities: As a member of the planning department, my responsibilities consist of submitting request for funding proposals to various local, state, and federal funding sources in an effort to financially support the various subsidiaries and

programs that operate under the Community Action Committee of the Lehigh Valley. I am also engaged in the efforts of a racial and ethnic justice advocacy group “Estamos Listos” or “we are ready” which seeks to ensure quality living conditions for the families affected by Hurricane Maria and forced to evacuate Puerto Rico and relocate to the Lehigh Valley.

Future Goals: After receiving my MSW, I plan to obtain my LCSW. I will also pursue my DSW after the required two years in the field and aspire to hold lead position of a substance abuse and recovery

Field Placement Responsibilities:

My field placement

responsibilities are assist individuals who are going trough a

crisis either face to face or on the phone.Conduct phone

surveys for the department, go out on outreach calls with the

staffs in the department. When the department receives calls

from hospitals, our job is to go to the hospital to meet with

the individual to try to divert him or her from getting

admitted to a inpatient hospital. The department also

conduct wellness checks on individuals.

Future Goals:

My future goals after my graduate degree is to

get my LCSW and work in a mental heath setting.

LVHN Street Medicine Program

Profile: The program’s focus is to begin where the client is; in their camps, in clinics, or simply out on the streets. The purpose of the program is to provide free medical care to a population who would otherwise face barriers including transportation and cost of healthcare. The program’s goal is to bridge its clients to a traditional medical

practice.

Field Placement Responsibilities: It is my responsibility to focus on client’s non-medical needs by linking them to the services that are necessary to meeting their basic needs and to build their capacity. I assume the role of advocate by challenging the structural and social injustices that hinder their ability to thrive. I assist in grant writing and policy creation. I help clients apply for services, schedule appointments, and conduct homeless needs assessments. I participate in client

outreach during street rounds, providing social support, distributing necessities, and assisting in engagement with newly identifies “rough sleepers”. Finally, I collaborate with inter-agency services and external agencies to provide a multi-faceted intervention and supportive network to our clients.

Future Goals: I plan to complete my MSW, become an LSW, and practice at the macro level. I hope to systemically address the issues of social and economic injustice and inequality.

Profile: The My Place Program is coordinated by Disability Service’s Office and works to provide support resources that go beyond

traditional service means and focus on developing students’ academic success, skills for workplace readiness and independent living skills. My Place offers unique and customized individual sessions with students to assist them to gain a connection with the University.

Field Placement Responsibilities: The My Place Program at Kutztown University is a premiere program that works with students to address individual needs to aid in student’s success. Mentors provide support through individualized attention to help student’s in their adjustment to college life, to promote academic success, to connect students to

resources throughout campus and working on social skills to promote belongingness. Mentors understand the importance of their role in planning and preparing all group sessions for recreation, study hall and dinners as well as providing the opportunities for students to attain more connections with other students and resources. They must

advocate, collaborate and broker students to other services on campus.

Future Goals: Further developing the My Place Program to further address the unique needs of the students that successfully increase the graduation rate of the students involved in the program.

Future Goals:

After my Master's degree, I would like to persue my

Social Work license and a certification in Thanatology so I may

my Music Therapy certification.

will be responsible for direct practice with clients by conducting individual psychotherapy using generally accepted techniques and models. Given the agency’s emphasis on promoting wellness for families and communities, I will be required to apply systems theory and a family-in-environment perspective to my work with these individuals. This will, at times, require me to extend treatment to other family members and to advocate on behalf of the client to fill any gaps in services that hinder their well-being. To be successful in this practice, I will be required to participate in staff meetings, trainings, and weekly supervision. I will also be expected to adhere to the ethical standards of the agency and the NASW Code of Ethics.

Future Goals: My primary interests are in clinical social work and advocacy.

I plan to work for an agency when I graduate to pursue these interests and to obtain my LCSW.

My role at United Way will be to

collaborate with the Community School Coordinator at McKinley

Elementary School in Allentown, to engage and plan with families

that have been identified as being chronically absent or at risk of

becoming chronically absent. Additionally, I will be collaborating

with my colleagues at United Way, to engage with business owners

as trusted messenger to provide materials and information to

parents within the Allentown Community about the Talk Read

Sing campaign. Parents will be encouraged to talk, read, and sing

to their children from birth to five years old to help them increase

their reading level.

Future Goals:

When I graduate with my MSW I plan to work in a

nonprofit agency similar to United Way. I would like to stay

involved in community development and work within the mezzo

and macro levels of practice.

Future Goals:

I would like to obtain my license while
