We appreciate your interest in our Certified Nurse Assistant Training Program. The necessary forms to apply for entrance into this program are included in this packet. Please follow the procedures be-low to simplify the application process.
Ukiah Adult School offers Certified Nurse Assistant Training Program classes Monday - Thursday Theory: 8:00am - 4:30 pm at Ukiah Adult School, Room 6 and Clinical: 6:00 am - 2:30 pm at Ukiah HealthCare. This schedule may vary.
ADMISSION REQUIREMENTS

• Have a high school diploma or documentation of a passing score on the GED Test • Physical Exam by a Physician/Nurse Practitioner (within 1 year prior to class start date) • Possess and maintain a current Child/Adult CPR card

INFORMATION FOR PROSPECTIVE STUDENTS
Ukiah Adult School, within Ukiah Unified School District, offers the Certified Nurse Assistant Training Program as a public service. Completion of the program takes 5 weeks. Upon successful completion of the course, graduates receive a certificate of completion and are eligible to take the state licen-sure exam.
Certified Nurse Assistants are employed by extended-care facilities, hospitals, home health, and by individuals who want extra help when they are in the health care facility or in their own home. Program Attendance: The program incorporates theory and nursing practice in Mendocino county. Students attend classes and receive instruction and experience in long term health care facilities al-ways under the instruction of an instructor for 32 hours a week, Monday through Thursday. The pro-gram schedule is 8 hours a day, 4 days a week.
Sixty (60) hours of training on basic skills, patient safety rights, the social and psychological prob-lems of patients, resident abuse prevention, recognition and proper reporting procedures. In addition to the sixty (60 ) classroom hours of training required, the precertification training program shall con-sist of at least one hundred (100) hours of supervised and on-the-job training clinical practice. At least two (2) hours of the sixty hours of supervised clinical training shall address the special needs of persons with developmental and mental disorders, including mental retardation, Alzheimer’s dis-ease, cerebral palsy, epilepsy, dementia, Parkinson’s disease and mental illness.
In order to apply for the state licensure examination, each student must satisfactorily complete the required number of hours in the program, maintaining at least a 70% average in each unit of instruc-tion, and demonstrating satisfactory clinical performance. Additionally, interpersonal relationships are very important and must be satisfactory. All students must adhere to the ethical principles and dress code of the school and the participating health care facilities.
The Faculty: The nursing faculty are all registered nurses with California teaching credentials who ARE PENDING approval by California Department of Public Health.
Additional Costs: The student is also responsible for white shoes, white uniform pants or skirt (if ap-propriate), uniform top (Please contact Uniform Solutions at 463-1470 or visit them at 175 Seminary Avenue in Ukiah. Pewter colored tops are required), a watch with a second hand, and a stetho-scope. All of which must be worn the first day of clinical practice. In addition to the aforementioned uniform and instruments, all students must possess a current CPR card.
State Licensure Fees: The certificate issued by Ukiah Adult School upon successful completion of the course allows you to take the state competency examination. With proof of passing the state ex-amination, the graduate may work as a Certified Nurse Assistant Training Program under the state and federal law guidelines for up to one year while awaiting receipt of state certificate, which could take up to four months.

Refunds:
Ukiah Adult School has established a fair and equitable refund policy for the refund of tuition, fees, or other charges in the event a class is cancelled or if a student decides not to enroll or does not complete the period of enrollment for which the student has been charged. This policy uniformly ap-plies to all nursing students.
If Ukiah Adult School cancels a class before it begins, 100% of all tuition and fees collected in ad-vance of the start date of the program will be refunded within 45 days.
Students who have not visited the school facility will have an opportunity to withdraw without penalty within the first three days of class.
If a student familiar with the facility withdraws from the program before the first day of class, 100% of any tuition or fees collected will be refunded, less the nonrefundable application fee of $250.
If a student withdraws or is terminated from the program after class begins, tuition and fees will be refunded on a pro-rated basis determined by the number of days of theory and clinical instruction remaining in the semester of withdrawal or termination. The cost of materials and services such as, but not limited to, books, liability insurance, and name tags will not be refunded.
Refunds will be made within 45 days of the last day of attendance by the student or within 45 days from the date the student is terminated or withdrawal has been determined by the school.
Prior Convictions:
If you have ever been convicted by any court of a crime other than a minor traffic violation, you must submit your fingerprints (via Livescan) and an application to the Department of Health Services for Certified Nursing Assistant, prior to enrollment in this course. Once you have obtained a criminal background clearance from them, you may enroll in the CNA Training Program. The state may re-quire the following information, which must be sent to the Department of Health Services, Licensing and Certification Program AID and Technician Certification section P. O. Box 997416 Sacramento CA 95899-7416 (916) 327-2445 to obtain clearance:
• Statement of facts, i.e., date(s) and nature of incident(s) • Disposition of the case (provide court papers)
• Current status
• Letters from the probation officer (if applicable), letters of recommendations (if applicable)
Ukiah Adult School has the right to deny a student enrollment based on the prior conviction penal section from the Department of Health Services, which constitutes automatic Denial/Revocation of certification (see “Conviction of Penal Code Section). If you have been cleared by the Department of Health Services for any conviction, bring your stamped clearance to Ukiah Adult School. Please contact the Director of Nursing regarding your questions and concerns about prior convictions.
Certified Nurse Assistant
Job Description:
Certified Nursing Assistants (CNA) were previously referred to as nursing aides (females) or order-lies (males). CNA’s provide basic patient care under the direct supervision of the nurses responsible for providing direct patient care. CNA’s may spend more time with patients than anyone else on the health care team. For this reason, they can easily form lasting connections with patients, especially when they work in extended-care facilities where the same patients are cared for each day.
CNA training is intended for those who seek entry-level employment in the health care field. A CNA program may also be prerequisite for students who need direct patient care experience as an admis-sion requirement for a higher level health care program or for those who wish to test their interest in health care as a career. Individuals might also consider CNA training if they are interested in work-ing in health care to support their education.
CNA’s are employed by extended-care facilities, hospitals, home health, and by individuals who want extra help when they are in the health care facility or in their own home. When working in a pri-vate home there are often additional duties, such as cooking, shopping, or light cleaning.
Duties and Responsibilities:
• Take vital signs ( temperature, blood pressure, pulse and respiratory rate).
• Provide direct patient care such as assisting with bathing, eating, dressing, and walking
patients.
• Turn and reposition bedridden patients. • Change bed linens
• Record amount of oral intake and measure urinary output • Collect specimens for test.
• Supply and empty bed pans. • Interact with patients and family
• Transport patients and equipment as needed.
• Keep a record of care given. “If you don’t document it, it didn’t happen.”
• Monitor patients and report any variances to the nurse for further assessment. • Follow infectious disease precautions to prevent the spread of organisms.
• Respect the person’s rights, provide privacy, and maintain patient confidentiality. • Respond properly to emergencies and know all facility safety procedures.
Personal Qualities:
CNA’s spend much of their time with patients who are ill or injured, sometimes doing unpleasant work such as changing bedpans. They must be compassionate and truly interested in helping oth-ers. Other important qualities are patience, honesty, good health, and the willingness and ability to follow directions.
Certified Nurse Assistant (CNA)

• Pass a Physical Exam by a Physician/Nurse Practitioner (within 1 year prior to class
start date)
• Obtain TB Screening within 6 months prior to class start date. (1 year for X-ray’s) • Upon completion of the Ukiah Adult School Certified Nurse Assistant Training Program,
successfully pass the state licensure exam. Career Ladders:
CNA is a prerequisite for many RN or LVN programs. Because of the increasing importance of the CNA to the health care system, some employers are offering opportunities for CNA’s to learn new skills and move up to positions with more responsibilities.
A CNA may not perform any nursing functions that must be performed by a licensed health care pro-fessional such as:
• Perform invasive procedures
• Pass/administer medications or give injections (This includes the administration of any
medica-tion associated with treatment of eyes, ears, nose, mouth, or genitourinary tract.)
• Perform ostomy care or apply wafer to skin • Insert, irrigate or replace catheters
• Suctioning
• Perform sterile procedures (dressing change, debridement, tracheostomy care) • Administer gastronomy or nasogastric feedings
