Metrocare’s pilot project (providing mental health case management services to persons being discharged from institutions), emancipated minors up to age 24 (especially those with children), those who have Notices to Vacate (within two weeks)/utility disconnect notices (within one month), those with documented financial emergencies (e.g., loss of income and registered with TWC, medical emergency, unexpected bill) who anticipate being able to pay for housing next month.

Other documentation may be required as determined by the agency. Assistance through this program is needs based: Cleints may be required to pay a portion of the rent or utilities. Case managed clients must actively participate in their stabilization plan through class participation and other trainings. These clients re-certify every 3 months and may or may not experience a graduated/declining subsidy.
