I. Students requesting Pre-AP/AP/Dual Enrollment/TAG Courses: Please list the courses in the space below.
Pre-AP/AP/DE/TAG Pre-AP/AP/DE/TAG
Pre-AP/AP/DE/TAG Pre-AP/AP/DE/TAG
Pre-AP/AP/DE/TAG Pre-AP/AP/DE/TAG
I understand that when I enroll in Pre-AP/AP/DE/TAG courses, I am committing myself to a more rigorous curriculum that requires additional time and effort. A recommended prerequisite of entering Pre-AP, AP, DE, or TAG level courses in KISD is grade of 85 or better in the previous course work. I also understand that if I fail all or part of this course or receive an NG, I will not be allowed to retake the class at the Pre-AP, AP, Dual Enrollment, or TAG level. If I need this credit I will be required to retake it as a regular class. I understand that if I earn a failing grade in a Pre-AP, AP, DE, or TAG class at the semester grading period, I may be placed in a lower level course for the following semester.
Students enrolled in an AP course will be required to take the Advanced Placement Exam.
II. Students requesting athletics or other special course may:
• provide their own transportation for these activities. • spend extra time after school.
• wear and be responsible for uniforms. • be required to attend performances.
• meet UIL eligibility requirements. • follow school rules at all times.
• wear ID badges to school sponsored events. • attend summer practices.
• have required physicals for athletics before working out.
Coach/Teacher Signature Course
I understand the requirements for extracurricular activities. I agree with the expectations and will assume responsibility for providing transportation when necessary.
III. Parents and/or Guardians:
I understand that my student is enrolled for a year’s worth of courses. Schedule changes will
be made only when absolutely necessary. Students may request a course change only if an error
in preregistration course requests has been made. My signature confirms agreement to
selections of student’s final requests for 2015-2016 school year (assuming no failures), Pre-
AP/AP/DE/TAG course requests, and curricular expectations. The courses that I have
approved listed on this form do meet the goals toward graduation that I have for my
student.
