INTRODUCTION AND GENERAL DESCRIPTION
Indian Springs Lake Estates is a residential subdivision located on Hwy 190 in Polk County, Texas approximately 11 miles east of the city of Livingston, Texas in Polk County. Wild Indian Lake Dam is a small sized, earthen dam. The property owners, who are represented by the Indian Springs Lake Estates Property Owners Association, own the dam, lake and other common areas of the subdivision, which are used
exclusively by the subdivision residents for recreational purposes.
Wild Indian Lake is a reservoir located on a tributary of the west fork of Double Branch creek. The dam is a small sized, earthen dam with a height of 17 feet. The crest is 300 ft. long by 20 ft. wide. The dam has a normal capacity of 71 ac. ft. and a maximum capacity of 186 ac. ft. The dam is rated as a High Hazard Dam by TCEQ. The normal water level in Wild Indian Lake is 303 ft. msl. Wild Indian Lake drains into Lake Mark, an adjacent lake also located in the subdivision.
The dam was built in 1963 by the Wiggins Land Development Company. It was first inspected in 1973 by the Texas Dam Safety Program within the Texas Water Rights Commission, a predecessor agency to the TCEQ. In 1983, the dam was modified to include an 80 ft. wide, open channel emergency spillway constructed along the right end of the dam and the dam crest was raised 1.3 feet, which was later inspected by the Dam Safety Program. This modification made the dam hydraulically adequate and passed 100% of the probably maximum flood (PMF). It is presumed these modifications were
maintenance on the dam and recommendations were made to remove small trees from the dam and install protective material along the upstream slope to prevent further erosion that was evident at that time. Additional inspections in 1999, 2007 and 2009 have revealed similar conditions as stated above including damage to the service spillway from torrential rains in 1999.
EAP PURPOSE AND IDENTIFICATION
The main purpose of this plan is to provide a systematic means to identify emergency responses to prevent a dam failure as well as prevent or reduce loss of life and property damage should a dam failure occur and have in place a timely procedure for emergency action in the event of probable or eminent danger resulting from overtopping, failure or unscheduled breach of the dam.
For identification purposes, Lake Mark is located south/southeast of Wild Indian Lake and is fed in part by a stream at the toe of and coming from Wild Indian Lake Dam. Aside from possible force on the Lake Mark Dam from water inundation that could result in damage to that dam, there are two other areas that might be affected by the release of water, caused by over-topping or dam failure of Wild Indian Lake Dam. One area
includes four residences located south/southwest below the dam. Another area that might be affected are three to four residences located south/southeast of Wild Indian Lake Dam. Currently, we show all these residences mentioned above to be occupied and could potentially be affected by a catastrophic release of water from the dam.
The success of this Emergency Action Plan depends on the following factors:
1. Responsibilities and Monitoring and documentation of weather conditions
affecting the dam that will help to determine if and when a possible or probably overtopping or failure may occur.
2. Notification of proper authorities who are designated to provide assistance
should failure or overtopping occur.
3. Updating this EAP on a regular basis to insure the information contained herein
RESPONSIBILITIES AND MONITORING:
Monitoring of the lake and dam for the purposes of this Emergency Action Plan is the responsibility of the Board of Directors of the Indian Springs Lake Estates Property Owners Association (ISLEPOA) and/or any members of a designated Dam Maintenance Committee in conjunction with the Indian Springs Volunteer Fire Department. Members of a committee shall be appointed on an as needed basis by the Board of Directors of the Indian Springs Lake Estates Property Owners Association.
An Emergency Action Plan Coordinator has been identified below and will be responsible for overseeing and implementing any procedures or responsibilities as stated in the EAP.
Below is a list of contacts in the order of responsibility to any action or need on Wild Indian Lake Dam. It should be noted that each of the persons listed below has the authority and responsibility to take necessary steps to put this plan into effect in the event of an emergency.
Responsibilities include, but are not limited to, observing the dam during extreme flooding events, during holidays, on weekends, and during normal conditions; implementing each of the required various phases of the EAP; be in charge of
emergency response, communication and coordination channels; setting up location of the incident command center or emergency operating center; and, coordinating lines of succession and assumptions of responsibility necessary to ensure uninterrupted emergency-response actions under any condition.

Once the ISVFD, Polk County Office of Emergency Management, and Polk County Sheriff have been notified, they will take the appropriate action to notify all other agencies, etc. as outlined in the Polk County Emergency Operations Plan.
The use of explicit, standardized language in case notification is required has proved beneficial to complete understanding by all parties. The messages that should be used for notification in these instances are:

THIS IS A DAM EMERGENCY MESSAGE. THE OWNERS OF THE WILD INDIAN LAKE DAM, (TEXAS INVENTORY NO. TX04915 LOCATED IN INDIAN SPRINGS LAKE ESTATES, 11 MILES EAST OF LIVINGSTON, TEXAS HAVE DECLARED A POSSIBLE DAM FAILURE WARNING CONDITION. IF PRESENT STORM CONDITIONS CONTINUE, THERE IS A POSSIBILITY OF DAM FAILURE. WE REQUEST THAT YOU INITIATE EMERGENCY MANAGEMENT PROCEDURES CONTAINED IN THE EMERGENCY NOTIFICATION SECTION OF THE EMERGENCY ACTION PLAN. IF THE WILD INDIAN LAKE DAM FAILS, FLOODING ALONG THE AREA SURROUNDING SOUTH OF THE DAM WILL OCCUR. WE REQUEST THAT YOU IMMEDIATELY INITIATE EMERGENCY MANAGEMENT PROCEDURES CONTAINED IN THE EMERGENCY NOTIFICATION SECTION OF THE EMERGENCY ACTION PLAN. WHEN THE WILD INDIAN LAKE DAM FAILS, A FLOOD WAVE WILL MOVE SOUTH ALONG THE SURROUNDING AREAS OF THE DAM AND INTO LAKE MARK. As part of the NOTIFICATION process, a flowchart which identifies distress conditions at the dam and provides a process for notifying all affected political jurisdictions and appropriate state and federal agencies of such conditions and possible consequences;

TESTING AND UPDATING:
It is the responsibility of the ISLEPOA Board of Directors to ensure that this Emergency Action Plan is current at all times. The Plan is to be reviewed and tested every five years with a tabletop exercise conducted under the direction of the owner or emergency
planner and revised as necessary. The EAP will be reviewed annually for contacts and numbers. Additionally, the NOTIFICATION FLOWCHART should be updated annually and provided to all contacts listed in the EAP.
TRAINING
At an annual meeting for review, or as otherwise deemed necessary by the EAP
Coordinator, a training meeting shall be conducted to provide instructions on how to use the EAP; identify problems and their severity; implement the notification procedures and the communication equipment; and, to provide knowledge of what resources are
available; the importance of their roles during emergencies; and the importance of updating downstream information.
LOCATION
The Wild Indian Lake Dam Emergency Action Plan (EAP) shall be available for viewing by the dam owner, local emergency-management personnel, and other local officials during business hours or as needed in an emergency at any time in the office of Indian Springs Lake Estates Property Owners Association, 266 Pichetto Trail, Livingston, Texas 77351
APPROVAL
Attached to this EAP is a form signed by the members of the Board of Directors of the Indian Springs Lake Estates Property Owners Association and Polk County Emergency Management Coordinator stating they have reviewed the EAP and concur with
notification procedures.
