The Place for Wellness Guidelines & General

Information

Hours of Operation

The Place for Wellness is open Monday through Thursday 5:30 am–7:00 pm and Friday 5:30 am–6:00 pm. The Fitness Center is closed weekends and nationally recognized holidays.

Membership

Fitness center membership is open to all AT&T tower tenants and a limited number of Oracle & International Centre employees. Membership dues are outlined in The Place for Wellness brochure. Fees are subject to change.

Oracle/International Centre Membership

Membership to Oracle & International Centre employees is subject to cancellation at any time with a 30 day notice.

Membership Fees

• Initiation Fee: $50

• Membership Dues (automatic deduction):

o Regular Membership Fee: $30 o Oracle Membership Fee: $45

o Bike Membership Fee: $20 (no initiation fee)

• Daily: $5, Weekly: $15

Membership Cancellation

Members wishing to cancel must give notice by the 20th of the month by submitting a cancellation form to The Place for Wellness staff at 901 Marquette Avenue, suite 250, Minneapolis, MN to avoid future charges.

Cancelled members wishing to rejoin within a twelve-month period of cancellation will be charged a re-enrollment fee of

$100.

Sign In

Members will receive an access card after completing an orientation and are required to sign in at each visit by scanning their access card below the tablet screen inside The Place for Wellness front door.

Health Insurance Reimbursement

Eligible Medica, HealthPartners, PreferredOne, and Blue Cross Blue Shield of Minnesota plan holders can receive a

$20 reimbursement on membership dues. See The Place for Wellness staff for more information or to get signed up for this reimbursement program.

Dress Code

Appropriate attire includes shorts, sweat pants, t-shirts, tank tops, etc. No plastic or rubber garments designed to retain heat and increase sweating are allowed due to increased health risk. Proper athletic shoes with non- marking outsoles are required in the fitness area and group exercise studio, except when recommended by the

instructors, as in yoga classes.

Amenities

Amenities in the locker rooms are supplied for your convenience and should not be removed from the fitness center. For the convenience of all members, please help maintain a clean environment in the locker rooms.

Towels

Towels are available as part of your membership and are to be utilized and remain in the fitness center. Used towels should be left in the provided towel hampers on the fitness floor and in each locker room.

Lockers

Day lockers are available for use while members are exercising in the facility at no charge. The Place for Wellness is not responsible for member belongings while in the facility or items left in the facility. A limited number of lockers are available for rent at a fee of $10 per month.

Check with The Place for Wellness staff for current availability or to be added to the locker rental waiting list.

Equipment Use

Equipment may not be reserved ahead of time. Members are responsible for wiping down equipment after use with the disinfectant and paper towels provided.

Lost and Found

All valuables should be locked in a locker while members are exercising in the fitness center. Please report any lost or found items immediately to staff. The Fitness Center is not responsible for lost or stolen articles. Articles not claimed after 30 days will be donated to a local charity.

Injuries

Any member who sustains an injury while exercising in the fitness center, no matter how minor, must notify a staff member immediately. In addition, members who experience an injury or change in health status outside of facility (i.e., muscle strain, joint problems, pregnancy, heart problems) should inform the staff upon returning to the facility.

Emergency Response System

The staff is trained in CPR and First Aid techniques and will respond to emergencies promptly. An automated external defibrillator (AED) is available at the security desk in case of a cardiac emergency. In case of an emergency, dial 911.

ENROLLMENT PROCEDURE Step 1: Program Registration

Complete a Member Registration form, Informed Consent, Release of Liability and a PAR-Q form. If you have indicated

“yes” to any of the questions on the PAR-Q form (or have regular blood pressure measurements above 140/90), a Medical Consultation form must be completed by your personal physician and returned to the fitness center before enrollment can be completed.

Step 2: Member Orientation & Fitness Assessment The Place for Wellness staff will contact you to schedule your member orientation. You will be required to complete a Quick Fit Check during your member orientation.

Additionally, you may choose to complete a Full Fitness Assessment.

Quick Fit Check:

This required appointment provides quick entry into The Place for Wellness. The Fit Check provides minimal assessment of your current fitness level by measuring your resting heart rate and blood pressure, height, and weight.

Full Fitness Assessment:

This is an optional, free, comprehensive assessment of your current level of fitness.

Our fitness center staff will assess your fitness level measuring your resting heart rate, blood pressure, body composition, flexibility, cardiorespiratory conditioning, and muscular strength and endurance. This information can provide the professional staff with baseline measurements and assist them in developing an exercise program tailored to meet your individual health and fitness goals based on your current level of fitness.

Step 3: Program Orientation

Please contact The Place for Wellness staff if you wish to schedule a second appointment to review your individual exercise program and/or complete a facility and equipment orientation.
