What are we working on?
•Resource Specialist efforts to provide better
assessments
•Supportive Services / Fire recovery – Unmet needs •Multi Agency Complex Cases
•
Prior to COVID-19, we were exploring ways to
expand the types of caregiver education and
training opportunities we offer throughout the
county. COVID-19 has advanced those efforts. We
will continue to explore a variety of educational
workshop opportunities that for now, we can
offer online by being a part of the Age Well
Education Conference.
•
With the teams and work distribution proposed,
we will have the opportunity to provide direct
Caregiver services and support, resource, and
planning information. Caregivers have expressed
a clear desire to have information for and about
caregivers centralized and easy to access.

Outreach
Outreach / Connecting with the Consumer
Due to the complexity of the cases that our agencies will be collaborating, the AAA will
have available the following outreach options:
•
Material distribution:
The AAA has bilingual postcards available that can be distributed
to key contacts/partners, and member of this group to facilitate information to
consumers about how to connect with services. – We currently have printed material
that can be shared with committee members and other partners.
•
Harm reduction - Blind Outreach:
When the referring agency or professional identifies
that there is no urgent need, but the consumer can benefit of knowing about the AAA
services, a letter will be sent by the AAA. Only the address of the consumer is needed.
-No name required.
•
At risk - Critical Assessment:
No progress regarding consumer’s safety - Safety code
violation: the Agency or professional working the case identifies an urgent need that
requires immediate intervention to assist an older adult, or if the consumer is at risk
and needs support to maintain their independence. An ROI from the consumer is
needed for the AAA to contact consumer via phone, video conference, or when possible
schedule a home visit.
