Please follow the guidelines below for compliance with the admission requirements and process to the upper division of the UCO Nursing Program

ACCEPTED APPLICATIONS

1. All applications must be COMPLETE and SUBMITTED ON TIME to be accepted and considered.

REJECTED APPLICATIONS:

1. LATE and INCOMPLETE applications will not be considered. 2. It is your responsibility to ensure that your application is complete.

3. You are welcome to come by the nursing office (Closed for Lunch from 1:00 – 2:00 pm) and check your file to see what has been turned in or call the department (974-5000).

4. The Nursing office is located in the Coyner Health Sciences Building, Room 121.

DEGREE CHECK:

1. All applicants must enclose a current copy (within the last 6 months) of a DEGREE

CHECK by Ashley Weingart, nursing advisor. TEAS TEST:

1. All applicants must complete the Test of Essential Academic Skills (TEAS) – VERSION V

prior to application deadline.

2. DO NOT PROCRASTINATE in meeting this requirement.

3. You may register online at http://broncho2.uco.edu/testing services/ 4. You may obtain testing dates from the above website.

5. Scores will be sent to the Nursing Department by the testing company. 6. Tests results must be scored and in the Nursing Department office by

a. SPRING SEMESTER September 11, 2015 b. FALL SEMESTER January 29, 2016

7. It is strongly suggested that you study for this exam. You may obtain study booklets and complete practice exams online at www.atitesting.com .

TOEFL or ENGLISH EQIVALENT EXAM

1. All students for whom English is a second language MUST take an English equivalent exam.

2. For the TOEFL:

a. The minimum score for admission consideration is i. 560 on the written examination

ii. 83 on the internet examination

OSBI CHECK (OKLAHOMA STATE BUREAU OF INVESTIGATION CRIMINAL BACKGROUND CHECK)

1. All applicants must complete an OSBI Criminal Background Check including Sex Offender and Violent Offender registries. SEE ATTACHED CRIMINAL HISTORY

INFORMATION REQUEST.

2. On the OSBI application make certain you include:

a. All names by which you’ve been known (I.e. maiden name, previous married names, aliases, etc.)

b. Your SSN.

3. The process takes about 15 minutes. This level of OSBI check does not require fingerprints.

4. OSBI Cost: a. $19.00:

b. Payable in cash, cashier’s check, money order or credit card. c. NO PERSONAL CHECKS are accepted.

5. DIRECTIONS TO OSBI CENTER (close to 63rd and Broadway Ext.)

6. NOTE: The ORIGINAL OSBI background check MUST be submitted with your nursing application. THE FORM MAY NOT BE ALTERED IN ANY MANNER.

PLEASE NOTE:

Applicants must retain a copy of all materials submitted to the Nursing Department. No material/documents will be duplicated from student files for student use.

PERFORMANCE STANDARDS FOR ADMISSION AND PROGRESSION IN THE DEPARTMENT OF NURSING:

Candidates for admission to the Department of Nursing must have the abilities listed on the attached sheet.

OKLAHOMA BOARD OF NURSING STATEMENT

“The University of Central Oklahoma is approved by the Oklahoma Board of Nursing. Graduates of this state-approved program are eligible to apply to write the National Council Licensure Examination (NCLEX) for

registered nurses. Applicants for Oklahoma licensure must meet all state and federal requirements to hold an Oklahoma license to practice nursing. In addition to completing a state-approved nursing education program that meets educational requirements and successfully passing the licensure examination, requirements include submission of an application for

licensure, a criminal history records search, and evidence of citizenship or qualified alien status. Applicants for practical nurse licensure must also hold a high school diploma or a graduate equivalency degree (G.E.D.) [59 O.S. §567.5 & 567.6]. To be granted a license, an applicant must have the legal right to be in the United States (United States Code Chapter 8, Section 1621). In addition, Oklahoma law only allows a license to be issued to U.S. citizens, U.S. nationals, and legal permanent resident aliens. Other qualified aliens may be issued a temporary license that is valid until the expiration of their visa status, or if there is no expiration date, for one year. Applicants who are qualified aliens must present, in person, valid documentary evidence of:

1. A valid, unexpired immigrant or nonimmigrant visa status for admission into the United States;

2. A pending or approved application for asylum in the United States; 3. Admission into the United States in refugee status;

4. A pending or approved application for temporary protected status in the United States;

5. Approved deferred action status; or

6. A pending application for adjustment of status to legal permanent resident status or conditional resident status.

The Board has the right to deny a license to an individual with a history of criminal background, disciplinary action on any professional or

occupational license or certification, or judicial declaration of mental incompetence [59 O.S. §567.8]. These cases are considered on an individual basis at the time application for licensure is made, with the exception of felony charges. An individual with a felony conviction cannot apply for licensure for at least five years after completion of all sentencing terms, including probation and suspended sentences, unless a

presidential or gubernatorial pardon is received [59 O.S. §567.5 & 567.6].
