Adding Personnel
MHow do I get a physician’s name to display in the Attending Practitioner drop-down list?
You must add him/her in the Personnel tab with a
job description.
You must also add the physician’s NPI or UPIN.
Valid CROWNWeb format reminder:
o Personnel NPI = 10 digits in length.
o Personnel UPIN = six characters in length. (Example: A12345 or ABC123)
Admitting Transient Patients
MDoes CROWNWeb automatically change a transient patient’s status to a regular transfer in if he/she stays at our facility for >30 days?
No. Once the transient window expires, the
transient facility must edit the Admit/Discharge record and change the “Transient Status” from Yes to No.
Access the CMS-2746 Form
MHow do I enter a CMS-2746 form for a patient via CROWNWeb?
From the Home Page, click the Patients tab.
You must then click the Edit Patient link to enter
the “Date of Death” and “Cause of Death.”
CROWNWeb will display an “Add 2746” link.
Administered vs. Prescribed

Date Access Type Changed
MWhat should I enter in the "Date Access Type

Enter the actual date the patient’s current access
type changed.
CROWNWeb may display a warning message if the
date is prior to the patient’s admit date.
Patients Missing from List
MWhat should I do when a patient is missing from my list on the clinical data screen?
Check to see if you have selected the correct
treatment type for the patient you are trying to locate.
Patients will only display for the collection type
selected for that clinical month.
No Lab Data
MWhat should we enter if labs were not drawn for a patient during the clinical month?
You are not required to enter lab values that you do
not obtain in a clinical month.
CROWNWeb's Clinical section was designed to
comply with CMS' data submission requirements and allow users to report lab data that was
