The results of this study will need to be extended and examined further by incorporating more material for analysis through multi-centre collaboration or the establishment of a specialized National Gastric Cancer Registry.
Our findings constitute prima facie evidence that a majority of gastric carcinomas in our population is most probably associated with H. pylori-induced chronic antral gastritis. Nevertheless, we found a significant number of cases falling among young age groups that would ordinarily not be reasonably linked pathogenetically with this pathway. Recent evidence suggests that gastric carcinoma is perhaps not a homogenous disease in that a peculiar range of molecular genetic anomalies might underlie such cases, especially in the young. Thus, it would certainly be of interest to subject materials from our cases to further investigation that would elucidate the molecular genetic profile of our gastric carcinoma from the very young and the archetypal older ones. Comparison between these two groups should yield interesting findings in understanding gastric carcinogenesis in our population.
There are obviously two epidemiological context of gastric carcinoma: The low incidence type that is associated with the developing countries and the high incidence type that is linked with the developed countries. Our study has further shown that the pathological features of these epidemiological types may be fairly distinct. It would be interesting to examine whether this disease similarly has a geographical molecular epidemiology.
