Purpose: To provide dentures at an affordable cost to Orem, Provo, Draper, Heber City, Riverton, Sandy and many other communities in the surrounding area.

Hours: Monday through Friday starting at 8 a.m.

in the surrounding area.

Services: Complete dentures, partial dentures, repairs, relines, adjustments, immediate extraction service, emergency extraction service, and x-rays.

Fees: Dependent on service rendered. Economy package or lowest fee is $795 for full denture and $550 for partial or single denture. Complete fees are listed on website.

Purpose: CHIP provides health insurance for low-income children of working families who do not qualify for Medicaid or have any other health insurance coverage.

Hours: Depends on individual provider.

Population: Children between the ages of 0 through 19 whose families income isles than equal to 200% of the Federal Poverty Level yet is too large to qualify for Medicaid. Children must be U.S. citizens, legal residents. Or have been born in the United States.

Services: Dental services include: comprehensive oral exams including radiographs; preventive care including cleanings, topical fluoride treatments and sealants; amalgam restorations; pulpotomies; extractions; stainless steel crowns and space maintainers.

Fee: Co-payments and fees vary

Application: Applications for children are always open. To receive more information and/or

Purpose: The College Dental Clinic provides general dentistry for any patient at a discounted price. Works with The Utah College of Dental Hygiene.

Hours: Monday through Thursday 8 a.m. to 5 p.m., Friday 8 a.m. to 2 p.m.

Population: Anyone 3 years or older

Services: General dentistry- fillings extractions, crowns, root canal treatment, sealants, bleach trays, and x-rays. No dentures. Spanish speakers available as needed.

Fees: Dental Exam including x-rays is $20. After that, any treatment that needs to be done is approximately 50% of the average dental office price. Most insurance are accepted, such as private, Medicaid and CHIP.

Purpose: Community Health Connects mission is to improve access to quality health and dental care for low-income uninsured adults and children. They accomplish this goal through the coordination of a Volunteer Provider Network, providing health education and by maximizing existing community resources.

Service: Eligibility screening located at above address. Other care provided varies according to the available health care providers.

Hours: Depends upon where services are received.

Community Health Connect hours: Monday and Wednesday 9 a.m. to 6 p.m., Tuesday and Thursday 9 a.m. to 4 p.m.

Population: Low income uninsured men, women, and children that are Utah County

residents. Patients whose income falls at or below 150% of the Federal Poverty Level.

Services: The car coordinators provide connect patients with doctors and dentists willing to provide care on a charity basis. They provide eligibility screening, coordinate appointments between providers and patients and also coordinate ancillary care needs. Patients in the dental program receive preventive services and a full spectrum of restorative care.

Fee: There is a $20 membership fee that needs to be paid every six months. There will also be additional costs related to other hard costs including crowns, bridges, partials and dentures.

Application: To receive dental services through Community Health Connect, Individuals must

Purpose: The Utah Medicaid program pays for medical and dental services for some low-income children, adults and pregnant women.

Hours: Depends upon where services are received.

Population: Only U.S. citizens or legal residents are eligible for the different types of

Medicaid programs. The monthly income standards for these programs vary between approximately 55% and 133% of the Federal Poverty Level. Some eligible categories of adults include: adults with children under 18; women with breast or cervical cancer; pregnant women; adults over the age of 60; adults who are blind or disabled; and legal refugees. Only individuals who have a Traditional Medicaid cared are eligible for dental services. Children with an income less than or equal to 133% of the FPL between the ages of 0 and 6 and less than or equal to the 100% of the FPL between the ages of 7 and 18 are eligible for the Medicaid program CHEC, which has different dental benefits.

Services: Limited dental benefits available for adults. Children on CHEC can have periodic examinations with radiographs and prophylaxis, which include: preventative care including topical fluoride applications and sealants; restorative care including resin and amalgam restorations; root canals; orthodontia in cases of severe malocclusion; and general anesthesia with limitations and/or prior approval.

Population: Provide care to all individuals, with an emphasis on those with Medicaid. Services: General dentistry

Fees: Mountain Ridge Dental accepts all insurances. Depending on an individual’s

insurance coverage, there may be co-pay required.

Purpose: Health professionals providing and collaborating with other partners to assure high quality health care for everyone in the community

Hours: Monday through Friday 8 a.m. to 5 p.m.

Population: MCHC will provide care to any individual.

Services: Comprehensive dental services, primarily cleanings, tooth extractions and restorative fillings. Some root canal procedures are performed. No dentures or orthodontic work is done. Emergencies are worked on as the schedule permits.

Fees: Sliding fee scale. $40 deposit due at time of registration.

Application: Patient registration form is filled out at the clinic, Monday through Friday from

10 a.m. to 4 p.m. If insurance is applicable, present insurance card. If no insurance is available, provide verification of income for the past month. This may include: last month’s check stubs, state unemployment funding information, state

Purpose: The Primary Care Network is state health coverage for low-income adults who do not qualify for Medicaid.

Hours: Depends upon where services are received.

Services: U.S. citizens and legal residents ages 19 to 64 whose income is below 150% of the Federal Poverty Level yet is too high to qualify for Medicaid.

Services: Preventative dental benefits including examinations with radiographs and prophylaxis every six months; restorative care including resin and amalgam restorations; and emergency examinations for pain or tooth infection.

Fees: An annual enrollment fee of up to $50 and co-pays for doctors’ visits not to exceed $1000 per year.

Application: Applications are only accepted during enrollment sessions. PCN is currently only

Purpose: Primary Children’s Medical Center serves five states in the Intermountain West-

Utah, Idaho, Wyoming, Nevada and Montana. The 232-bed facility is equipped and staffed to treat children with complex illness and injury.

Hours: Monday through Thursdays from 9:00 a.m. to 4:30 p.m. and Fridays from 9 a.m. to noon.

Population: The Primary Children’s Medical Center Dental Program serves children ages 0 up

to 14 years old that have special needs, disabilities or medical problems, such as heart problems, cancer, bone marrow transplants, autism delayed development, and spina bifida. Children with dental needs requiring surgery are a priority they serve children who have private insurance, Medicaid or who are without

insurance or public benefits.

Services: Comprehensive dental care.

Fees: Low-income children without Medicaid can apply for Primary Services, which will cover their dental services.

Purpose: Salt Lake Donated Dental Services provide preventative and restorative dental care for those who are low-income or homeless.

Hour: Monday, Tuesday, Wednesday — 9 a.m. to 12:30 p.m. and 1:30 p.m. to 4 p.m., Thursday and Friday — 9 a.m. to noon.

Population: SLDDS provides dental service to the homeless and those falling below the “very

low income” level as defined by the Federal Standards. SLDDS will treat those with Medicaid, PCN or CHIP as long as their income qualifies. Those seeking services should call Salt Lake Donated Dental Services to find out if they meet the income requirements.

Services: Salt Lake Donated Dental Services provides comprehensive dental services, including preventive and restorative procedures and extractions. Because the clinic is staffed by volunteer dentist, the dental services offered can change daily.

Fees: Salt Lake Donated Dental Services request a $10 donation but it is not a requirement for services. Patient must pay lab costs for prosthetic procedures (dentures, partials flippers)

Application: Salt Lake Donated Dental Services accepts patients on a walk-in basis. Children

Utah College of Dental Hygiene

Purpose: The Utah College of Dental Hygiene offers a Bachelor of Science in Dental Hygiene upon completions of a two year program. A significant part of the students training involved the clinical treatment of patients in the UCDH Clinic. Students are supervised by licensed dental hygiene instructors and a licensed dentist.

Hours: These change yearly for the fall 2011/Spring 2012. Tuesday, Wednesday, Thursday patients will be seen at 8:00 a.m. and 1:00 p.m. From January 2012 to the end of April no patients will be seen. Summer clinic hours TBA.

Population: Anyone age 3 and over is eligible

Services: Preventive services such as dental examinations, x-rays, polishing, fluoride treatments, dental sealants and deep scaling. Local anesthesia is available. Spanish speakers available as needed.

Fees: There is a one-time $10 sterilization fee. Duplicate x-rays are $50. Chlorhexidine prescription rinse is $10 and Clinpro fluoride paste is $10.

Utah Valley University Dental Hygiene Program

Purpose: The Utah Valley University Department of Dental Hygiene offers an Associates of Applied Science in Dental Hygiene degree upon completions of a two-year program. A significant part of the students training involves clinical treatment of patients in the UVU dental hygiene students’ clinic. Students are supervised by licensed dental hygiene instructors and a licensed dentist.

Hours: Spring Semester:

Monday — 3 p.m. to 6 p.m.

Tuesday, Wednesday, Thursday — 8:30 to 11:30 a.m. & 12:30 to 3:30 p.m.

Fall Semester:

Tuesday, Thursday 12:30 to 3:30 p.m.

Wednesday 8:30 to 11:30 a.m. & 12:30 to 3:30 p.m.

Population: Individuals age 3 and older, if the parent has concerns about a younger child they can be brought in to be seen as well.

Services: Preventive services such as cleanings, x-rays, polishing and fluoride. Sealants are provided as needed.

Fees: For students and staff of UVU it is a $13. For adults not a part of UVU it is $25 (depending on income it could be less). For children ages 4-10 there is a $10 fee and for adolescence ages 11-16 there is a $15 fee. For a Panoramic X-Ray it costs about $18 - $20. For sealants it is $5 a tooth or $15 a quadrant of teeth.

Purpose: To provide immediate relief for individuals who are unable to receive treatment elsewhere.

Hours: Currently there is only one dentist. He works 4 days out of the week alternating between Monday thru Thursday and Wednesday thru Saturday. Office hours are 8 a.m. to 7 p.m.

Population: Serviced are available to general population. Services: General dentistry.

Fees: Dependent on services provided. Accepts Medicaid, PCN and CHIP as well as other insurances.
