Hudson Valley Building Blocks

– THINC and its vendor, MedAllies, with a HEAL 1 grant from the NYS DOH, have supported the

implementation of more than 1000 EHRs in the last five years, a significant factor in area’s high rate of EHR adoption.

– THINC’s program has

disproportionately supported small practices, primary care and FQHCs

– 350 primary care providers achieved NCQA Level 3 PCMH recognition in THINC’s medical home project, run in

collaboration with Taconic IPA.

– A second set of 300 primary care providers are participating in CMS’s Comprehensive Primary Care (CPC) Initiative

– THINC is convener for the Comprehensive Primary Care (CPC) Initiative

Medical home recognition of 350 PCPs. Another 300 PCPs in Medicare CPC project
