Position Purpose

The purpose of the Hume School Focused Youth Service is to ensure that at risk young people (aged 10 to 18 years) are well supported in their communities, that risk factors are addressed or mitigated and that they continue to be engaged and connected to education. This will be achieved by:

 Coordinating the provision of community based services in partnership with schools and community agencies to support schools to address the needs of at risk young people

 Fostering an integrated service delivery model through developing shared protocols and systems

 Enhancing existing partnerships and creating new partnerships and pathways between schools and community organisations to support at risk young people

 To assist all schools to this endeavor including Government, Independent and Catholic schools in their support for at risk young people, and in particular, by working closely with and complementing the Student Support Services.

 Achievement of performance, financial and individual Key Performance Indicators as outlined in Performance Appraisal process

Duties and Responsibilities

Hume School Focus Youth Service:

o Plan, implement, review and evaluate the Hume SFYS work plan and collect data as appropriate.

o Adhere to the SFYS program guidelines.

o Coordinate care to vulnerable young people and their families by fostering an integrated service model between schools and local service providers.

o Chair and coordinate the SFYS Governance and Advisory Group and to ensure active membership by key stakeholders such as SSS Network Executive Chairs and Co-ordinators, Catholic Education and Independent Schools representations, Hume Whittlesea Local Learning and Employment Network (HWLLEN), Hume Council, Mental Health, Child First.

o Identify the psychosocial needs and emerging issues of the community and of individuals and identify at risk/priority groups and work pro-actively with schools and community agencies to address the needs and reduce risk.

o Ensure the Hume SFYS is responsive to local issues, emerging issues and trends by accessing and utilising local and broader data as well as utilising outcomes/measuring tools such as the Student Online Case System (SOCS), the Australian Early Development Index (AEDI), and the Victorian Child and Adolescent Monitoring System (VCAMS).

o Work in partnership with service providers, community agencies and groups and health

professionals within and external to Dianella to develop implement and evaluate other integrated health promotion programs.

o Utilise a range of health promotion interventions and be prepared to work in settings outside the centre to promote social and emotional wellbeing.

o To coordinate the use of flexible funding as per the SFYS work plan and ensure funds are accounted for by working collaboratively with Dianella Accounts Payable/Finance.

Professional Development:

o Participate in individual and group supervision.

o Plan, implement and evaluate a professional development work plan.

o Utilise and contribute to the evidence based research on at risk youth and disseminate this information accordingly in appropriate internal and external forums.

o Contribute towards social work fieldwork education by maintaining links with professional bodies and educational institutions, and through the support of undergraduate student placements.

o Ensure a high quality social work service by updating own professional skills and knowledge through attendance at appropriate courses/workshops/seminars.

General responsibilities o Maintain Duty of Care.

o Maintain confidentiality and client privacy.

o Assist in the planning, development, implementation and evaluation of programs that respond to the needs of the community.

o Perform duties within the scope of practice of the role and the standards and guidelines set out by the funding and accreditation bodies.

o Undertake appropriate activities as directed by management to ensure the quality and safety of services. This includes participating in quality development activities, clinical audits, conduct of client satisfaction surveys, and program evaluation as relevant.

o Supervise volunteers or students as required.

o Participate in the annual performance appraisal process.

o Promote distribute and share knowledge and information within the organisation and relevant parties.

o Develop and maintain working relationships with appropriate local and regional service providers.

o Participate in the development and maintenance of effective team processes.

o Provide a best practice model of service delivery.

o Be professionally accountable.

o Identify work processes that could be improved and present possible strategies for service.

Additional Information

 Travel between sites may be required including outreach work within the Dianella catchment.

 Occasional after hours and weekend work may be required.

 This position description is indicative of the role and may include other duties as directed

 The position is subject to available funding and Dianella financial status

 Specific tasks and functions of this role will be detailed through objectives and key performance indicators

Professional Skills and Qualifications

 Maintain registration as a Social Worker and fulfil the requirements of the Board including maintaining their code of ethics

 Demonstrated commitment to ongoing professional development

 Maintain regular contact with the other clinical staff at Dianella and work together to share expertise and improve client outcomes

 Demonstrate a commitment to reflective practice and continuous quality improvement AUTHORITY

The occupant of this position has authority as per Delegation Manual

Key Selection Criteria

Essential Qualifications, Experience and Registrations:

 Qualified Social Worker eligible for membership of A.A.S.W. or equivalent.

 Minimum of 2 years field experience.

 Experience working as a Social Worker in a community setting including work with CALD, Aged and clients with a disability

 Experiencing in the provision of care coordination of services for young people in a school setting and/or within an integrated service delivery model.

 Experience in developing partnerships with key stakeholders in the provision of collaborative and integrated services.

 Experience in developing, implementing and evaluating evidence based psycho-social programs.

 Well developed verbal and written communication skills.

 Sensitivity to issues related to the provision of services in a community of high disadvantage and cultural diversity.

 Ability to work as a member of a team.

 Demonstrated ability to participate in the development of new and innovative programs.

 Computer literacy.

 Current Victorian drivers licence

 Valid Working with Children’s Check (valid for 5 years from the date of issue)

 Satisfactory Police Check (valid for 3 years from the date of issue)

Desirable Qualities and Experience

 Ability to write clear, concise and informative reports

 Experience or understanding of the relevant funding initiatives

 A relevant community language

 Experience facilitating and supporting students

 Knowledge of community support services

 Experience in working within schools to support vulnerable young people including working with Student Support Services.

 Demonstrated knowledge of relevant frameworks and guiding principles in the provision of support services to support young people in schools identified as at risk.

 Demonstrated knowledge and experience in working in primary health care within a multidisciplinary setting.

 Demonstrated knowledge and experience in working within a social model of health and an understanding of the social determinants of health.

 Demonstrated knowledge and experience in working with women and children who have experienced family violence.
