COVID-19 COMMUNITY BASED RESPONSE AND SUPPORT NETWORK Release Date: 02/16/2021

Due to the ongoing COVID-19 pandemic the County of Sacramento, Department of Health Services, Division of Public Health is seeking a qualified organization to:

1) Coordinate a broad network of agencies, including multi-cultural community based organizations (CBOs), to implement a culturally responsive system of support services for vulnerable Sacramento County residents impacted by COVID-19.

2) Oversee and manage distribution of wrap around support services for Sacramento County residents who have been negatively affected by the COVID-19 outbreak, especially communities

disproportionately impacted including but not limited to Hispanic, Black, Native Hawaiian/Pacific Islander, Native Americans and older adults (65+). Services may include financial, food or meal assistance; mental health support; support services for pregnant women or mothers of infant children; and providing personal protective equipment.

3) Coordinate and implement a Business Navigator Project to provide information to businesses in Sacramento County regarding compliance with COVID-19 restrictions; and guidance on

reopening, and preventing and managing cases among employees.

4) Support operation of culturally and linguistically appropriate COVID-19 community based testing in diverse communities of Sacramento County. Engage CBOs to provide community support for vaccination clinics.

5) Facilitate and support culturally and linguistically appropriate, and accessible information and community engagement to promote COVID-19 prevention, testing and/or vaccination. Develop and provide accessible outreach and educational materials and methods.

Due to the uncertain nature of the COVID-19 pandemic and shifting needs and focus of the County, the services described above may shift and/or become unnecessary. The service needs described are based on information currently available to the County.

The approximate available budget will be $3,000,000 for the anticipated term of July 1, 2021 through June 30, 2022. In the event that additional funds are made available to the County for these types of services, the available budget amount could increase. Funding is contingent on receipt of Federal COVID-19 Response and/or Recovery funds.

To be selected, the organization MUST meet ALL of the following minimum requirements:

1) Have at least 3 years of experience convening and coordinating agencies that work with disadvantaged communities to address health issues in Sacramento County.

2) Have at least 3 years of experience implementing and coordinating complex, multi-agency projects including having contractual relationships with those agencies; and managing multi-million dollar contracts.

3) Have experience with COVID-19 response activities including working with community based agencies to provide culturally sensitive support services for vulnerable residents impacted by COVID-19, and supporting community-based testing.

4) Possess at least 5 years of experience conducting or facilitating efforts to address health issues in Sacramento County.

5) Possess the capacity and system(s) to track, monitor, evaluate and report progress towards meeting multiple contract deliverables.

6) Be identified as a 501(c) 3 organization or other non-profit in good standing.

7) Single agency responses only. No collaborations, partnerships, multi-agency, or fiscal sponsorship responses will be accepted.

Submission Contents:

1) Interested organizations must respond using the form at the end of this document by the due date (see below). The form must include your organization’s information – legal name, address, phone, e-mail, contact name and must be signed by a person with contract signing authority for the organization.

2) Organizations must provide a brief Statement of Qualifications (SOQ) not to exceed one typed page, describing how they are qualified to provide the services described in this request for LOI/SOQ. The SOQ must be on your agency’s letterhead and include LOI/SOQ No. DPH/055. The successful applicant shall be required to sign a Sacramento County contract template, and must agree to all terms and conditions of the contract with Sacramento County.

This will be the only advertisement of this request for LOI/SOQ. The County of Sacramento reserves the right to cancel this public notice, to reject any or all LOI/SOQ’s received, request additional information/documentation from applicants, and to determine which LOI/SOQ shall be accepted based on submissions received and qualifications, including experience of respondents. Should more than one response be received, the most qualified respondent(s) will be selected. The County reserves the right to make a contract award to one or more bidders for all or part of the services. Issuance of this LOI/SOQ in no way constitutes a commitment by the County to award a contract.

Responses are due and must be emailed to Karen Olson at [email protected] by 5:00 PM (PST), February 26, 2021. The following will not be accepted: hard copies of responses that are delivered to County locations, emailed responses to any other email address, or faxed responses. Late submissions will not be accepted and will be automatically disqualified from further consideration. Any respondent claiming that the County did not follow the processes outlined in this document may protest the planned award by emailing the details of such claim to [email protected] by March 18, 2021 with the subject line ‘Protest, LOI/SOQ No. DPH/055. The details of the claim, stating how the County did not follow the processes outlined in this document, will be investigated and the Director of the Department of Health Services will make a finding on the claim.

COVID-19 COMMUNITY BASED RESPONSE AND SUPPORT NETWORK Scope of Work (SOW)

1) Develop and coordinate a network of agencies including multi-culturally community based organizations (CBOs) to implement a culturally responsive system of support services for vulnerable Sacramento County residents impacted by COVID-19.

2) Contract with agencies to provide culturally sensitive support services for vulnerable residents impacted by COVID-19, especially communities disproportionately impacted by COVID-19 including but not limited to Hispanic, Black, Native Hawaiian/Pacific Islander, Native American and older adults (65+). Services may include financial, food or meal assistance; mental health support services; support services for pregnant women or mothers of infant children; and providing personal protective equipment. CBOs shall employ case managers (Care Resource Coordinators) that will communicate and coordinate with Sacramento County case investigators and contact tracers; and will provide community outreach and COVID-19 prevention education.

a. Oversee implementation of an economic aid model to distribute financial assistance to vulnerable Sacramento County residents who experience financial hardship due to the COVID-19 public health emergency.

b. Work with community partners to distribute food to vulnerable residents in Sacramento County; oversee food and meal distribution to individuals and families quarantined or isolated due to COVID-19.

c. In partnership with community-based organizations, provide case management and services to pregnant and infant-parenting mothers, with an emphasis on at-risk communities of color, to prevent and slow the spread of COVID-19. Wraparound services may include financial, food or meal assistance; mental health support services; assistance with accessing healthcare such as transportation and/or child care; and wellness resources. 3) Coordinate and implement a Business Navigator Project to provide information to businesses in

Sacramento County regarding compliance with COVID-19 restrictions; and guidance on reopening, and preventing and managing cases among employees. Hire Business Navigators (BNs) directly or through community-based organizations, and ensure BNs are adequately and appropriately trained and informed on current COVID-19 guidance, orders and/or restrictions. Convene a Business Navigator Project Steering Committee to identify targeted sites in conjunction with the County and community-based organizations.

4) Contract with community-based organizations to provide culturally appropriate, ADA accessible and inclusive COVID-19 community based testing in diverse communities of Sacramento County. Coordinate with Sacramento County Public Health and CBOs on operational planning, monitoring, evaluation and feedback. Engage CBOs to provide community support for vaccination clinics.

5) Facilitate and support culturally and linguistically appropriate, and accessible information and community engagement to promote COVID-19 prevention, testing and/or vaccination. Develop and provide outreach and educational materials to CBOs including in multiple languages; accessible for deaf/hard of hearing and visually impaired residents; and for individuals without technologicaland/or email access.

6) Conduct data collection, monitoring and evaluation of program progress and impact. Report progress towards meeting all program deliverables to Sacramento County Public Health. Provide an open communication process for direct feedback, input and check-ins with subcontractors.
