The Community Reentry Unit of the Minnesota Department of Corrections produced this booklet. The aim is to address the numerous requests made by caseworkers, corrections agents, community workers and transition support people for a detailed directory of housing options available to offenders reentering the community and the public at large.
This directory is not a complete listing of the varied array of housing options currently available across Minnesota. We recognize that housing resources shift and change dramatically over time. Therefore, we welcome feedback on housing information that will allow us to expand and improve the directory.
An online edition of this directory is available at www.doc.state.mn.us under the Publications link. This online directory will be updated as new housing information becomes available.

MACV accomplishes its mission by providing services directly or in collaboration with other service agencies. MACV provides a structured assistance program giving the Veteran support to once again become a productive member of the community, confidence to rebuild relationships and knowledge that the community recognizes and cares about them and their services to our country.
Qualifications:
To be eligible for services, clients must be a Veteran with 181 days of active duty service.
Veteran must be a Minnesota resident (30 days).
Veteran must be homeless or at imminent risk of becoming homeless.
Must be motivated to make positive change.
To be eligible for housing assistance, the Veteran should pay no more than 30% of their income toward monthly housing expenses.
Other Information:
Structured Independent Living Houses (SIL) are offered for clients who show a desire to live a sober life after drug and alcohol treatment. The length of stay varies, and is dependent on the circumstances, but the maximum stay is two years. SIL house residents are responsible for the management of the normal duties that arise in almost any other independent housing environment. MACV provides a structured program that gives the homeless veteran the support to again become a productive member of the community, the knowledge that the community cares, and the confidence to build relationships. Life in the SIL houses is not a preparation, it is an experience of life as it really is. MACV has eleven SIL Houses located in St. Cloud, Richfield, Minneapolis, St. Paul, Maplewood, and Duluth. Hearth Connection manages a network of supportive housing providers that includes mobile supportive service providers that use predominantly private market scattered-site housing and site-based supportive housing communities. There are over 1,000 people in programs today, including over 300 single adults, 200 families with children and 35 unaccompanied youth.
Hearth Connection does not deliver services or housing directly, it is an intermediary, administrative entity.

Tasks Unlimited provides supported employment, housing and recovery services for people with mental illness so that they achieve a full life with the rights and responsibilities of adults in our society.
Programs and Services include:
Supported Employment Program offers mental health services, paid on-the-job training and basic living skills development. Participants can take part in the supported employment program with or without supported housing. All participants must qualify for and complete one of Tasks Unlimited paid training programs upon admission. Successful training graduates average nearly 11 years of steady employment with the organization.
Housing lodges are stable, supportive homes in clean, desirable neighborhoods throughout the Twin Cities for people with mental illness. Tasks Unlimited lodges do not have 24-hour supervision. Members manage their own lives and provide support to each other in daily activities. Lodge members decide their own rules, pay rent, clean, shop for groceries, go to work and participate in community activities just as one would do in any living situation with roommates.
Mental Health Services provides medication management, illness management and recovery groups, chemical health program, wellness program, and recreation program.
Qualifications:
Be at least 18 years of age.
Meet State of MN requirements for serious and persistent mental illness.
Express a willingness to actively participate in the program.
Be physically prepared to participate in activities.
Not pose a risk of serious harm to self or others.
Have support and funding from the State of Minnesota Vocational Rehabilitation Services.

A veteran is defined as anyone who served 181 consecutive days active duty with an honorable discharge.
Must have entered service in Minnesota or currently live in Minnesota.
Spouses of eligible veterans must be over 55 years of age and reside in the state.

Skilled nursing care is available at every facility (except Hastings).
Private and semi-private rooms, library, coffee shop, barber/beautician, and family lounges available.

The Reconnect program is a program and drop-in center providing information, referrals, and specialized one-on-one planning tailored to the specific challenges that come with having a criminal background. The Reconnect program is a relationship-based service that supports, respects, and empowers the whole person.
Qualifications:
Anyone with a felony, gross misdemeanor, or misdemeanor on their record, whether just out of a federal, state, or county correctional facility or years after release.
Program Services include:
Housing- Offers case-by-case information and referrals to housing opportunities that meet individual needs and are open to those with a criminal background.
Employment- Resources are available for use to access multiple online databases to help searching for jobs in fields that are often open to people with felonies. Daily job openings are posted along with one-on-one recommendations and assistance with finding employment.
Transportation– Subsidized bus passes are offered at a reduced price.
Clothing– Referrals to clothing resources are provided, as well as hygiene products.
Voicemail– Free voicemail number is available for those without a phone, only available for up to six months.
Computers and Phones– Available to use for job searching, house searching, setting up appointments, checking in with probation officers, etc. Setting up an e-mail account, faxing, copying, and printing documents are available upon request.

Minnesota Place Apartments

Minnesota Place Apartments provide a safe and supportive environment for chronically homeless tenants to stabilize their housing and income, build relationships with others, and access appropriate services.
Minnesota Place Apartments offer:
Clean, safe, and affordable housing for low-income and formerly homeless adults.
Assistance developing community living skills.
Connections to community resources.
Support in reducing personal problems and barriers to self-sufficiency.
Qualifications:
Openings at Minnesota Place are limited. Eligible residents must meet the Department of Housing and Urban Development’s “chronic homeless” definition (An unaccompanied individual with a disabling condition who has been continuously homeless for a year or more or has had at least four episodes of homelessness in the past three years).

Jackson Street Village is a supportive housing community that provides site-based permanent housing for 24 previously homeless families. Services offered at Jackson Street Village are designed to help families maintain their housing and maximize their ability to achieve self-sufficiency and transform their lives.

The mission of Portland Village is to provide families safe, sober, affordable housing in combination with support services that will facilitate family stabilization, an increase in skills and income, an increase in health and wellness, and overall housing stabilization. Portland Village is a 26-unit, sober, supportive, permanent, multi-family housing community in Minneapolis. Portland Village consists of three apartment buildings and a central administrative building that houses a PICA/HeadStart daycare program, staff offices, and a large family activity room. The apartment buildings contain 8-two bedroom units, 12–three bedroom units, and 4-four bedroom units. Each building includes access to laundry facilities, storage area, and community room. A six computer resource lab is available for adults.

The Central Avenue Apartments Supportive Housing Program, located in Minneapolis was designed to provide subsidized housing and support services to 61 homeless individuals.
Central Avenue is a sober building, and the tenants commit to living a sober lifestyle.
Qualifications:
Central Avenue Apartments serves homeless, economically disadvantaged single men and women who experience a disability.
Other Information:
It is expected that participants be productive for at least 25 hours per week including employment, education, volunteering in the community, and health care.
All units have kitchens, full baths, air conditioning, bed, chair and table
Utilities (except telephone and cable) are included in rent
Rent is subsidized. Residents pay 30 % of their income for rent
Supportive services are offered to all residents. Case managers are available to help residents maintain sobriety, find employment, continue their schooling and access other resources.

Seventh Landing is a 12 unit sober, supportive, permanent, singles housing program situated on bus lines, near several community human service agencies, hospitals, schools, and other community resources. Seventh Landing consists of 12 studio apartments, a community meeting room, staff offices, and the non-profit business and job skills training program, Fresh Grounds Café.
Qualifications:
Seventh Landing serves homeless, economically disadvantaged single men and women who experience a disability. Seventh Landing prefers adults who have had a significant (6 months +) experience in out-of-home placement as a youth and who have not lived independently before.

Property Amenities:
Emanuel Apartments is a 101 unit sober, supportive, permanent, singles housing program that is situated on the Hiawatha Light Rail Line, bus lines, and is near several community human service agencies, hospitals, schools, and other community resources. Emanuel consists of 95 studio apartments and 6-one bedroom apartments that include a tenant lounge area, a computer lab, an exercise room, an enclosed courtyard, walking trail, and staff offices. Emanuel Apartments include access to laundry facilities and a community room, a ten computer resource lab and meeting room is available for adults.
Qualifications:
Emanuel housing serves economically disadvantaged single men and women. Emanuel housing includes specialized housing that serves 11 homeless veterans through the VASH program, 54 units of housing that prefers GRH eligible homeless disabled adults, 14 units of housing that serve chronically homeless disabled adults through the shelter and care program, 11 flat rate units, and six project based section eight subsidized units.
Other Information:
Emanuel is a pet friendly building. Adults who wish to own a pet must comply with the Emanuel Apartments lease requirements for pet ownership.

Dillon Apartments is a 32 unit sober, supportive, permanent housing community for single adults. Dillon Apartments are situated on bus lines and easily accesses community resources, clinics, and services. Dillon consists of two apartment buildings. One building houses staff offices, a laundry facility, a community room, and a computer resource room for adults. The units are a mix of one bedroom and efficiency sizes, leased according to availability.

Jackson Street Village is a 24 unit sober, supportive, permanent, family housing program.
Jackson street village consists of 24-two, three, and four bedroom units. Jackson Street Village includes a community building with a computer lab, tenant lounge, playground, and staff offices. Jackson street village serves homeless and long-term homeless, economically disadvantaged families who experience a disability.
Lindquist Apartments

Lindquist Apartments are located in North Minneapolis and provide 25 unit sober, supportive, permanent, singles housing program. Lindquist Apartments consists of 24 studio apartments and 2-one bedroom units designed for Group Residential Housing (GRH) eligible applicants.
Lindquist Apartments includes a community room, computer lab, tenant lounge, staff offices, access to laundry facilities, and a courtyard. Lindquist serves homeless and long-term homeless, economically disadvantaged single individuals who experience a disability.

Reentry Ashland is a community based corrections residential program for men located in the Ramsey Hill neighborhood. Ashland accepts men referred by Minnesota Department of Corrections (state work release and supervised release clients).

Reentry Metro offers a cooperative, structured living environment where women are held responsible for their actions both in and outside the facility. Qualifications:
Reentry Metro is a community corrections residential program for women and their children located in the midway area of St. Paul. This facility provides safe, comfortable and structured housing and support services for up to 26 women and small children. Reentry Metro accepts women referred by the State Department of Corrections, Ramsey and Hennepin County Community Corrections, Federal Pre-trial, and County Child Protection.

Aeon is a nonprofit developer, owner and manager of high-quality, affordable homes in the Minneapolis/St. Paul area. Aeon aims to provide housing options for people with low or moderate incomes. Aeon owns and manages 38 properties and 2,000 units of affordable homes in Minneapolis, St. Paul, Brooklyn Center, Chaska, and Roseville. Aeon's supportive housing serves homeless youth, homeless adults, and homeless adults seeking a sober community.
Aeon provides the following services:
Housing Development: Renovates and constructs apartment buildings and town homes with an emphasis on quality, community participation, and people-centered design.
Asset Management: Provides long-term sustainability planning.
Resident Connections: Helps residents create homes by connecting them to each other and community resources.
Property Management: Makes certain that Aeon’s values and high standards are reflected in the daily operations of each property.
The following requirements apply to all buildings except those serving homeless adults and youth. Please call each building for more information.
The building manager must be able to confirm good rental history.
Current income must be enough to pay rent.
Section 8 vouchers are accepted.
As part of the application process, criminal, credit, and rental history will be checked and may be discussed.

Energy Assistance Program

Modeled after the success of San Marco, River Crest, opened its doors in July 2010. The River Crest facility, located in St. Cloud, Minnesota offers 40 units of group residential housing for homelessness and chronic alcoholics. Center City Housing partners with Recovery Plus to provide a nurse and addiction case management. The property was developed to provide housing and supportive services to Sherburne, Stearns, Benton, and Wright Counties. River Crest is the first of this type of housing in the St. Cloud area.
